雙側(cè)小關(guān)節(jié)切除制作大鼠頸椎間盤退變模型的可行性

【摘要】:目的:探討雙側(cè)小關(guān)節(jié)切除建立大鼠頸椎間盤退變模型的可行性。方法 :16只3月齡雌性SD大鼠隨機(jī)均分為實(shí)驗(yàn)組和對(duì)照組。實(shí)驗(yàn)組大鼠C4/5和C5/6雙側(cè)上、下關(guān)節(jié)突采用磨鉆切除。術(shù)后12周時(shí)獲取大鼠C4~C6標(biāo)本。顯微CT掃描C5/6節(jié)段,測量椎間高度及軟骨終板的缺損率,并觀測C5椎體微結(jié)構(gòu)的變化;番紅O快綠染色后觀察髓核和纖維環(huán)的形態(tài),并對(duì)椎間盤退變程度評(píng)分。采用RT-PCR法檢測C4/5椎間盤組織中蛋白聚糖、Ⅱ型膠原以及基質(zhì)金屬蛋白酶(MMP)3和13的m RNA表達(dá)水平。組間定量指標(biāo)行獨(dú)立樣本t檢驗(yàn),顯著性水平為P0.05。結(jié)果:術(shù)后12周,實(shí)驗(yàn)組椎間高度為0.51±0.04mm,顯著低于對(duì)照組(0.55±0.02mm)(P0.05)。實(shí)驗(yàn)組軟骨終板出現(xiàn)明顯的缺損,下終板缺損主要出現(xiàn)在腹側(cè),而上終板四周及中央均出現(xiàn)缺損;實(shí)驗(yàn)組的缺損率為(11.5±2.0)%,顯著大于對(duì)照組的(6.9±1.0)%(P0.05)。在椎體微結(jié)構(gòu)中,實(shí)驗(yàn)組骨體積分?jǐn)?shù)和骨小梁間隙分別為(53.0±6.0)%和170±2μm,而對(duì)照組分別為(46.4±3.0)%和195±1μm,兩組間的差異均有統(tǒng)計(jì)學(xué)意義。實(shí)驗(yàn)組的骨小梁數(shù)目和厚度與對(duì)照組無統(tǒng)計(jì)學(xué)差異。組織學(xué)觀察到實(shí)驗(yàn)組椎間盤的軟骨終板形態(tài)不規(guī)則、出現(xiàn)缺損及少許鈣化,髓核細(xì)胞出現(xiàn)聚集、數(shù)量減少,纖維環(huán)排列紊亂。實(shí)驗(yàn)組椎間盤退變?cè)u(píng)分為8.6±0.8分,顯著高于對(duì)照組的5.8±0.5分。在實(shí)驗(yàn)組椎間盤組織中,蛋白聚糖和Ⅱ型膠原的m RNA表達(dá)水平明顯低于對(duì)照組,MMP-13的m RNA表達(dá)水平顯著高于對(duì)照組,MMP-3則呈現(xiàn)上升趨勢(shì)。結(jié)論:大鼠頸椎雙側(cè)小關(guān)節(jié)切除可導(dǎo)致切除節(jié)段椎間盤在形態(tài)學(xué)、組織學(xué)和分子生物學(xué)上的退變,是建立椎間盤退變模型的一種可行方法。
[Abstract]:Objective: to investigate the feasibility of establishing cervical disc degeneration model by bilateral facet joint resection in rats. Methods: sixteen three-month-old female Sprague-Dawley rats were randomly divided into experimental group and control group. The lower articular process was resected by grinding drill. The C4~C6 specimens were obtained 12 weeks after operation. C5 / 6 segments were scanned by microCT. The height of intervertebral and the defect rate of cartilage endplate were measured, and the changes of microstructures of C5 vertebrae were observed. The morphology of nucleus pulposus and fibrous ring was observed after fast green staining, and the degeneration degree of intervertebral disc was graded.
