【摘要】：為尺側(cè)上副動脈穿支皮瓣提供解剖學(xué)基礎(chǔ)。通過對14側(cè)新鮮成人上肢標(biāo)本的顯微解剖學(xué)研究發(fā)現(xiàn):尺側(cè)上副動脈全程均有穿支皮動脈發(fā)出,其中遠(yuǎn)近兩端的穿支位置恒定,并與尺側(cè)下副動脈、尺側(cè)返動脈及臂后皮動脈所發(fā)出的穿支在上臂內(nèi)側(cè)中、下部皮膚各層次形成可靠的血管吻合網(wǎng)。本研究為上臂內(nèi)側(cè)皮膚的血管構(gòu)筑提供了更為詳盡的解剖學(xué)數(shù)據(jù),并以此設(shè)計(jì)了以尺側(cè)上副動脈穿支為血管蒂的上臂內(nèi)側(cè)近端蒂及遠(yuǎn)端蒂皮瓣用來修復(fù)腋部、上臂上部及肘部、前臂上部的軟組織缺損。該皮瓣血管蒂血供穩(wěn)定,手術(shù)操作簡單,供區(qū)損傷小,設(shè)計(jì)靈活,易于臨床推廣。
[Abstract]:The microanatomical study of 14 fresh adult upper limb specimens showed that the perforating cutaneous artery of the superior ulnar collateral artery appeared throughout the course of the whole process, and the perforating branches at the far and near ends of the flap were constant, and the microanatomy of 14 fresh adult upper limb specimens showed that the perforating branch of the superior ulnar collateral artery was in a constant position. The perforating branches of the inferior ulnar collateral artery, the recurrent ulnar artery and the posterior cutaneous artery were located in the medial upper arm. A reliable vascular anastomosis network is formed at all levels of the lower skin. This study provides more detailed anatomical data for the vascular architecture of the medial upper arm skin. The medial proximal end pedicle and distal pedicle flap pedicled with the perforating branch of the superior superior ulnar collateral artery were designed to repair the soft tissue defects in the armpit, upper arm, elbow and upper forearm. The blood supply of the vascular pedicle of the flap was stable and the operation was simple. The donor area has the advantages of small injury, flexible design and easy to be popularized in clinic.
