【摘要】： 肝十二指腸韌帶位于肝門與十二指腸上部之間,不論在肝膽外科還是在胃腸外科,均占有極其重要的地位,有非常多的手術(shù)均需對此結(jié)構(gòu)進行解剖。其內(nèi)有進出肝門的肝外膽道、肝固有動脈、門靜脈等重要結(jié)構(gòu),以及肝神經(jīng)叢、淋巴管和為數(shù)不多的淋巴結(jié)等。本文利用35具防腐成人腹部標本解剖,觀測肝十二指腸韌帶內(nèi)主要結(jié)構(gòu)及其重要分支的結(jié)構(gòu)關(guān)系,進行量化分析,同時對各重要結(jié)構(gòu)的起源及與其關(guān)系密切的血管也進行了觀察,結(jié)果發(fā)現(xiàn),其內(nèi)主要三大結(jié)構(gòu)相對關(guān)系比較固定,但其在肝十二指腸內(nèi)分叉的高低及距離差異較大,以膽道最為明顯,它們與各自的上級管通及下級分支之間也存在較多變異。因此詳細了解該區(qū)的解剖結(jié)構(gòu),熟悉相關(guān)的各種變異,對于此部位的手術(shù)具有重要的作用。
[Abstract]:The hepatoduodenal ligament is located between the hepatic hilum and the upper part of the duodenum, and plays an extremely important role both in hepatobiliary surgery and in gastrointestinal surgery. There are a lot of surgeries that need to be dissected. There are important structures in and out of the hilum of the liver, including the extrahepatic bile duct, the proper hepatic artery, the portal vein, and the hepatic nerve plexus. The lymphatic vessels and a few lymph nodes were dissected from 35 antiseptic adult abdominal specimens. The main structures and important branches of the hepatoduodenal ligament were observed and analyzed quantitatively. At the same time, the origin of the important structures and the vessels closely related to them were also observed. The results showed that the relative relations of the three major structures were relatively fixed, but the height and distance of the bifurcation in the liver and duodenum were quite different. The bile duct is the most obvious, and there are many variations between them and their superior canal and their subordinate branches. Therefore, it is important to understand the anatomical structure of the area in detail and to be familiar with the related variations.
