【摘要】：第一部分 骨性頸靜脈孔區(qū)的顯微解剖 目的 通過(guò)研究頸靜脈孔(Jugular Foramen，JF)的位置、組成、形態(tài)、大小及其與周圍骨性結(jié)構(gòu)的關(guān)系，尋找臨床手術(shù)解剖學(xué)標(biāo)志，為臨床手術(shù)提供解剖學(xué)基礎(chǔ)。 方法 顯微鏡下觀察20例成人頭顱顱骨標(biāo)本JF的位置、組成、形態(tài)，并測(cè)量其大小及與周圍骨性結(jié)構(gòu)的距離關(guān)系。對(duì)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)處理，并對(duì)所觀察的解剖結(jié)構(gòu)用數(shù)碼相機(jī)拍照記錄。 結(jié)果 JF位于顱底枕髁的前外側(cè)和巖骨的后內(nèi)側(cè)，，左右各一。JF呈不規(guī)則三角形。內(nèi)口75％右側(cè)大于左側(cè)，外口85％右側(cè)大于左側(cè)。巖骨和枕骨向JF內(nèi)發(fā)出頸靜脈內(nèi)突(intrajugular press，IJP)并凸入孔內(nèi)，10％JF中兩側(cè)的IJP相互融合形成骨橋。JF的前外側(cè)壁有三角形的隱窩—錐形窩，后外側(cè)是膨大的骨性穹隆陷窩—頸靜脈窩，73.9％髁管內(nèi)口位于JF后外部的后內(nèi)側(cè)緣。 JF的毗鄰結(jié)構(gòu)：JF外口的前方有頸動(dòng)脈管外口和巖斜下間隙；外側(cè)為莖突、莖乳孔以及稍遠(yuǎn)處的乳突。乳突、莖突、頸動(dòng)脈管外口幾乎成一直線；內(nèi)側(cè)緊鄰舌下神經(jīng)管外口和枕髁。枕髁后緣至舌下神經(jīng)孔內(nèi)口的距離約8.15 mm；后方為枕骨頸突，形成了JF的后緣。JF內(nèi)口的前上端接巖下竇溝，后外側(cè)端接乙狀竇溝。上有內(nèi)耳門；外上有前庭水管外口；內(nèi)側(cè)有頸靜脈結(jié)節(jié)。頸靜脈結(jié)節(jié)下有舌下神經(jīng)管內(nèi)口。 結(jié)論 1、JF是由枕骨和顳骨巖部合圍的一個(gè)骨性管道，多數(shù)是不規(guī)則的三角形，多數(shù)右側(cè)大于左側(cè)。 2、鼓乳裂可以定位莖乳孔及JF外側(cè)壁；枕骨頸突是確定JF后壁的標(biāo)志。 3、熟悉JF及其周圍結(jié)構(gòu)的解剖關(guān)系，可以很好的理解和治療該區(qū)疾病。
[Abstract]:Part I microanatomy of the osseous jugular foramen. Objective to study the location, composition, shape, size of jugular Foramenus (JF) and its relationship with the surrounding bone structure in order to provide anatomic basis for clinical surgery. Methods the position, composition and morphology of JF in 20 adult skull specimens were observed under microscope. The size of JF and the distance between JF and the surrounding bone structure were measured. The observed anatomical structure was recorded with digital camera. Results JF was located on the anterolateral side of the skull base occipital condyle and the posterior medial side of the petrosal bone. The right side of the external orifice 85% is larger than the left. The petrosal bone and occipital bone send out intrajugular intrajugular intrajugular protruding to JF and protrude into the internal foramen of 10 JF. The IJP on both sides of the osseous bridge. JF has a triangular recess-cone fossa on the anterolateral wall of the bone bridge. At the posterolateral side, 73.9% of the medial orifice of the condylar canal was located at the posterior medial margin of JF. The adjoining structure of JF is the external orifice of carotid artery and the space of inferior petrosal obliquity in front of the outer orifice of JF, the lateral part is styloid process, cauline foramen and mastoid process, mastoid process, styloid process and external orifice of carotid artery are almost in a straight line. The distance between the posterior edge of the occipital condyle and the internal orifice of the foramen of hypoglossal nerve is about 8.15 mm. The posterior occipital neck process forms the posterior edge of JF. The anterior superior end of the JF medial orifice is connected with the sulcus of the inferior petrosal sinus, and the posterior lateral end is connected with the sigmoid sinus sulcus. External vestibular aqueduct external orifice; medial jugular vein nodule; jugular vein nodule have hypoglossal nerve canal internal orifice below jugular vein nodule. Conclusion (1) JF is an osseous canal surrounded by occipital bone and temporal bone, most of which are irregular triangles, and most of the right are larger than the left. 2. Tympanic fissure can locate the lateral wall of JF and the lateral wall of JF, and the occipital cervical process is a marker for determining the posterior wall of JF. 3. Familiar with the anatomical relationship of JF and its surrounding structures, it can be well understood and treated in this area.
