1例肺炎患者利奈唑胺耐藥糞腸球菌定植規(guī)律研究

【摘要】:目的研究1例肺部感染患者體內(nèi)利奈唑胺耐藥糞腸球菌株突變以及定植特點(diǎn)。方法從1例反復(fù)發(fā)作肺炎(1年內(nèi)反復(fù)發(fā)作5次)的急性白血病患者的氣管分泌物中分離出10株糞腸球菌株,菌株的MIC值通過E-test測定,經(jīng)PCR擴(kuò)增細(xì)菌4個(gè)拷貝23S r RNA基因V區(qū)基因,測序分析突變位點(diǎn),通過脈沖場凝膠電泳(PFGE)觀察分離菌株同源性。結(jié)果患者5次肺炎發(fā)作從氣管分泌物中分離的10株糞腸球菌株均為利奈唑胺不敏感株(4株為中介耐藥株,6株為耐藥株),通過PCR和測序發(fā)現(xiàn)4株利奈唑胺中介耐藥株含有1個(gè)拷貝23S r RNA基因V區(qū)G2424U突變,而利奈唑胺耐藥菌株同時(shí)含有G2424U和G2576U雙突變。通過PFGE分型檢測提示10株糞腸球菌株具有同源性。通過肉湯稀釋法鑒定這些菌株均對(duì)萬古霉素敏感。結(jié)論利奈唑胺耐藥糞腸球菌株可能長期定植于患者體內(nèi),并導(dǎo)致反復(fù)感染。
[Abstract]:Objective to study the mutagenesis and colonization characteristics of lenazolidomide resistant Enterococcus faecalis strain in a patient with pulmonary infection. Methods the tracheal secretion of a patient with acute leukemia with recurrent pneumonia (5 episodes in one year) was studied. Ten strains of Enterococcus faecalis were isolated. The MIC value of the strain was determined by E-test. The V region of 23S r RNA gene was amplified by PCR, and the mutation site was analyzed by sequencing. Results 10 strains of Enterococcus faecalis isolated from trachea secretions in 5 episodes of pneumonia were all linazolamide insensitive strains and 4 strains were intermediate resistance strains. By PCR and sequencing, it was found that there was one copy of G2424U mutation in V region of 23S r RNA gene. Both G2424U and G2576U were found in linazolamide resistant strains. PFGE typing showed that 10 strains of Enterococcus faecalis were homologous. These strains were all sensitive to vancomycin by broth dilution method. Conclusion Liniazolamide is sensitive to vancomycin. The drug-resistant Enterococcus faecalis strain may be colonized in the patient for a long time. And lead to repeated infections.
