【摘要】：本研究旨在了解橋小腦角的顯微解剖；探討枕下乙狀竇后入路及乙狀竇后-內(nèi)耳道上入路的優(yōu)缺點(diǎn)。本實(shí)驗(yàn)采用15例(30側(cè))干標(biāo)本和10例(20側(cè))濕標(biāo)本，按照枕下乙狀竇后入路和乙狀竇后-內(nèi)耳道上入路的所涉及的解剖區(qū)域，解剖觀測(cè)面神經(jīng)、中間神經(jīng)、小腦前下動(dòng)脈及巖靜脈等諸多結(jié)構(gòu)，并對(duì)神經(jīng)外科手術(shù)乙狀竇后入路及乙狀竇后。內(nèi)耳道上入路作初步探討，主要實(shí)驗(yàn)結(jié)果如下： 1．面神經(jīng)橋小腦角池段長(zhǎng)為11.13士1.35mm，內(nèi)耳道段長(zhǎng)為10.69±0.90mm。面神經(jīng)在橋延溝的外端起自腦干，與其外側(cè)的前庭蝸神經(jīng)進(jìn)入腦干處相距2.38+0.65mm，距外側(cè)展神經(jīng)6.62±0.41mm，距三叉神經(jīng)7.59±1.08mm。由交匯角內(nèi)前方沿巖骨探查32mm，即可發(fā)現(xiàn)面聽(tīng)神經(jīng)。 2．中間神經(jīng)單根束居多，共12側(cè)，，兩根束6側(cè)，三根束2側(cè)。未見(jiàn)四根束。中間神經(jīng)前庭部長(zhǎng)為7.62±0.69mm，中間部長(zhǎng)為10.05±1.12mm。 3．小腦前下動(dòng)脈 小腦前下動(dòng)脈(AICA)出現(xiàn)率為100％,20側(cè)共23支，單干18側(cè)，雙干1側(cè)，三干1側(cè)，未見(jiàn)缺失。20支起自基底動(dòng)脈(BA)下段，2支起自BA中段，1支起自BA上段。副小腦前下動(dòng)脈共出現(xiàn)3支，主要分支有迷路動(dòng)脈、回返穿通動(dòng)脈、弓狀下動(dòng)脈。 4．10例(20側(cè))標(biāo)本共有27支巖靜脈，6側(cè)中存在有2～3支巖靜脈。巖靜脈在腦橋小腦角內(nèi)各屬支的匯合點(diǎn)至巖上竇入口處的距離為3.46±1.87mm。巖靜脈在注入巖上竇入口處的壓扁外徑為2.26±0.82mm�？稍谇巴ノ伾窠�(jīng)正上方71mm
[Abstract]:The purpose of this study was to investigate the microanatomy of the cerebellopontine angle, and to explore the advantages and disadvantages of the retrosigmoid sinus approach and the retrosigmoid approach to the inner ear. 15 cases (30 sides) of dry specimens and 10 cases (20 sides) of wet specimens were used. According to the anatomical regions involved in the retrosigmoid suboccipital approach and the retrosigmoid approach and the superior approach to the internal auditory canal, the structures of the facial nerve, the intermediate nerve, the anterior inferior cerebellar artery and the petrosal vein were anatomically observed. The retrosigmoid approach, retrosigmoid sinus approach and suprasigmoid approach were discussed. The main results were as follows:. 1. The length of cerebellopontine cistern was 11.13 鹵1.35 mm, and the length of internal auditory canal was 10.69 鹵0.90 mm. The distance from the lateral vestibulocochlear nerve to the brainstem was 2.38 0.65 mm, 6.62 鹵0.41 mm from the lateral abductor nerve and 7.59 鹵1.08 mm from the trigeminal nerve. 2. There were 12, 6 and 2 bundles of the medial nerve, respectively. There were no four bundles. The vestibular part of the medial nerve was 7.62 鹵0.69 mm, and the middle part was 10.05 鹵1.12 mm. 3. The occurrence rate of AICA in 20 sides of anterior inferior cerebellar artery (AICA) was 100 and 20, 18 in single trunk, 1 in double trunk and 1 in third trunk. No missing .20 was found in the inferior segment of the basilar artery (BAA) and 2 branches in the middle segment of BA and 1 branch in the upper segment of BA. There were 3 branches of the anterior inferior accessory cerebellar artery, the labyrinthine artery, the return perforating artery, and the arcuate inferior artery. 4. There were 27 petrosal veins in 20 sides of 10 cases. The distance between the confluence of petrosal veins in the cerebellopontine angle and the entrance of the superior petrosal sinus was 3.46 鹵1.87 mm. the petrosal vein was compressed at the entrance of the superior petrosal sinus. The outer diameter was 2.26 鹵0.82mm. it was 71mm above the vestibulocochlear nerve.
