經(jīng)口氣管插管的定位評估防逆流裝置的設(shè)計與效用評價

【摘要】:目的改進實驗大鼠經(jīng)口氣管插管定位技術(shù),提高氣管插管效率,減少并發(fā)癥。方法采用臨床套管針和靜脈留置針組合,設(shè)計自制的氣管插管定位防逆流裝置。60只成年雄性SD大鼠隨機分成A、B兩組,A組40只,用于定位效果測試,B組20只用于防逆流測試。A組進一步分為A1組采用自行設(shè)計的定位裝置,A2組采用霧氣法,A3組采取棉纖維法進行定位判斷。B組分為B1、B2兩個亞組,均采取氣泡逸出作為定位觀察手段,不同的是B1使用圓臺狀防逆流裝置,B2組不使用。采用透光法經(jīng)口氣管插管,觀察并記錄定位時間、呼氣相定位效果、吸氣相逆流水柱長度。A1組進一步行氣管切開術(shù)在直視下明確定位狀況。結(jié)果在呼氣周期內(nèi),觀察出泡口連續(xù)逸出3個以上的氣泡,可以確定插管在位良好并通暢。定位時間:A1組需要(1.75±1.02)個呼吸周期,A2組需要(3.30±0.95)個呼吸周期,A3組需要(4.10±0.99)個呼吸周期。A2組與A3組兩組間比較差異無顯著性(P0.05),A1組明確定位需要的時間明顯少于A2、A3組,差異有顯著性(P0.01)。B1組最長逆流水柱長度為(3.23±0.53)cm,B2組最長逆流水柱長度為(8.48±1.01)cm。結(jié)論自制實驗大鼠經(jīng)口氣管插管的定位評估防逆流裝置結(jié)構(gòu)簡單,使用方便,能有效提高氣管插管的定位效率,有較好的實驗應(yīng)用價值。
[Abstract]:Objective to improve the localization technique of endotracheal intubation in rats, improve the efficiency of tracheal intubation and reduce the complications. Methods 60 adult male SD rats were randomly divided into two groups: group A (n = 40), and group A (n = 40), who were divided into two groups: group A (n = 40), group A (n = 40) and group A (n = 40). Group A was further divided into A1 group with self-designed positioning device, A2 group with fog method and A3 group with cotton fiber method. Group B was divided into two subgroups: group B (B1) and group B2 (group B), and group B (group B) was divided into two subgroups: group B (B _ 1) and group B (B _ 2). Bubble escape was used as the location observation method, but B1 used round counter-current protection device, B2 group did not use it. The time of location, the effect of expiratory localization and the length of inspiratory counter-current column were observed and recorded by transtracheal intubation. Group A 1 was further performed tracheotomy to determine the localization status under direct vision. Results during the expiratory cycle, more than three bubbles were observed to escape from the vesicles continuously, and the catheterization was good and unobstructed. Location time: (1.75 鹵1.02) breathing cycles in A1 group and (3.30 鹵0.95) respiratory cycles in A2 group. There was no significant difference between A2 group and A3 group (P0.05), and the time required for clear location in A1 group was significantly shorter than that in A2 group (A3 group), and there was no significant difference between A2 group and A3 group (P0.05), but there was no significant difference between A2 group and A3 group (P0.05). The longest countercurrent column length in B1 group was (3.23 鹵0.53) cm,B2, and the longest countercurrent column length in B1 group was (8.48 鹵1.01) cm.. Conclusion the localization evaluation of tracheal intubation in self-made experimental rats is simple in structure and convenient in use.
