【摘要】：目的比較木瓜蛋白酶關(guān)節(jié)腔注射與關(guān)節(jié)石膏伸直位制動(dòng)建立兔膝骨關(guān)節(jié)炎模型的效果。方法將21只實(shí)驗(yàn)兔采用隨機(jī)區(qū)組分組法分為正常組、木瓜蛋白酶組及石膏組,每組7只。木瓜蛋白酶組予6%木瓜蛋白酶生理鹽水溶液于實(shí)驗(yàn)開(kāi)始第1、4、7天行右膝關(guān)節(jié)腔注射,注射結(jié)束4周后造模完成;石膏組予膝關(guān)節(jié)伸直位固定6周;記錄實(shí)驗(yàn)期間動(dòng)物不良情況發(fā)生率,造模完成后處死實(shí)驗(yàn)兔,取軟骨標(biāo)本進(jìn)行大體觀(guān)察、Pelletier評(píng)分、Mankin's評(píng)分,免疫組化測(cè)定軟骨Ⅱ型膠原蛋白含量,ELISA法檢測(cè)血清和關(guān)節(jié)液中腫瘤壞死因子-α(TNF-α)、軟骨寡聚基質(zhì)蛋白(COMP)以及基質(zhì)金屬蛋白酶-3(MMP-3)的含量。結(jié)果大體觀(guān)察顯示木瓜蛋白酶組與石膏組均出現(xiàn)關(guān)節(jié)腫脹、關(guān)節(jié)間隙變窄、軟骨缺損、滑膜增生硬化、炎癥改變、骨贅形成;Pelletier評(píng)分、Mankin's評(píng)分、Ⅱ型膠原蛋白含量、血清和關(guān)節(jié)液中TNF-α、COMP以及MMP-3的含量,木瓜蛋白酶組、石膏組與正常組比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);木瓜蛋白酶組與石膏組比較,上述指標(biāo)差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。木瓜蛋白酶組動(dòng)物不良情況發(fā)生率明顯低于石膏組,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論木瓜蛋白酶關(guān)節(jié)腔注射與石膏關(guān)節(jié)伸直位制動(dòng)均能成功建立膝骨關(guān)節(jié)炎模型,但木瓜蛋白酶關(guān)節(jié)腔注射法更為快捷、簡(jiǎn)便,并發(fā)情況少,可行性好。
[Abstract]:Objective to compare the effect of papain injection and plaster immobilization on rabbit knee osteoarthritis model. Methods 21 rabbits were randomly divided into normal group, papain group and gypsum group with 7 rabbits in each group. In the papain group, 6% papain saline solution was injected into the right knee joint on the 1st and 4th day of the experiment, and the model was made after 4 weeks of injection, while the gypsum group was fixed in the straight position of the knee joint for 6 weeks.
