結(jié)核菌及其耐藥性不同檢測(cè)方法的臨床應(yīng)用評(píng)價(jià)

【摘要】:目的對(duì)目前臨床上用于檢測(cè)結(jié)核分枝桿菌(MTB)及其耐藥性的傳統(tǒng)及分子生物學(xué)方法進(jìn)行系統(tǒng)評(píng)價(jià),為設(shè)計(jì)合理的診斷路徑提供指導(dǎo)意義。方法對(duì)標(biāo)準(zhǔn)菌株(H37Rv)、敏感、單耐及耐多藥MTB進(jìn)行梯度濃度稀釋后,作為評(píng)估的實(shí)驗(yàn)樣本,從檢測(cè)結(jié)果的準(zhǔn)確性、檢測(cè)周期、最低檢出限方面對(duì)不同方法進(jìn)行評(píng)價(jià)。結(jié)果MTB檢測(cè)方面,XpertMTB/RIF檢測(cè)MTB的檢出下限(3×10~2cfu)與培養(yǎng)法相當(dāng),比基因芯片和GenoType~汶 MTBDRplus的檢出下限低;耐藥性檢測(cè)方面,在MTB含量不足(≤3×10~2cfu)的情況下XpertMTB/RIF對(duì)利福平(RFP)的檢測(cè)存在假陽性結(jié)果,GenoType~汶 MTBDRplus檢測(cè)3×10~4cfu鏈霉素耐藥MTB時(shí)出現(xiàn)RFP和異煙肼(INH)耐藥的假陽性結(jié)果;檢測(cè)周期方面,XpertMTB/RIF在2 h內(nèi)可完成報(bào)告,基因芯片和GenoType~汶 MTBDRplus在當(dāng)天8 h內(nèi)可完成報(bào)告,培養(yǎng)及比例法藥敏需4~8周可報(bào)告結(jié)果。結(jié)論目前不同的MTB及耐藥檢測(cè)方法各有優(yōu)缺點(diǎn),在臨床應(yīng)用方面應(yīng)綜合考慮患者的病情、經(jīng)濟(jì)狀況等選取合理的診斷路徑。
[Abstract]:Objective to systematically evaluate the traditional and molecular biological methods for the detection of Mycobacterium tuberculosis (MTB) and its drug resistance, and to provide guidance for the design of a reasonable diagnostic pathway. Methods the standard strain (H37Rv), sensitive, single resistant and multidrug resistant MTB were diluted with gradient concentration, and evaluated from the accuracy of test results, detection period and minimum detection limit.
