【摘要】：目的準(zhǔn)確定位小腿三頭肌的神經(jīng)入肌點(diǎn)(N點(diǎn))位置,為臨床該肌痙攣神經(jīng)阻滯提供解剖學(xué)基礎(chǔ)。方法 10具20側(cè)成年人尸體下肢,俯臥。緊貼皮膚連接股骨外上髁與內(nèi)上髁和股骨外上髁與外踝的線分別為N點(diǎn)的橫向參考線(H線)和縱向參考線(L線)。解剖暴露小腿三頭肌各神經(jīng)肌支的N點(diǎn),涂抹硫酸鋇,CT掃描。Syngo系統(tǒng)下確定N點(diǎn)在體表的投影點(diǎn)(P點(diǎn));P點(diǎn)通過N點(diǎn)后投射至對(duì)側(cè)皮膚上的P'點(diǎn);經(jīng)P點(diǎn)的垂線與H線、水平線與L線的交點(diǎn)分別記為P_H和P_L。分別測(cè)量P_H和P_L在H和L線上的百分位置及N點(diǎn)的深度。結(jié)果腓腸肌內(nèi)側(cè)頭、外側(cè)頭和比目魚肌的P_H分別位于H線的(46.89±2.73)%、(40.90±3.05)%和(42.56±2.59)%處,P_L分別位于L線的(7.58±2.88)%、(8.15±2.52)%和(17.42±3.31)%處;N點(diǎn)深度分別位于PP'線的(16.32±2.52)%、(13.83±1.77)%和(29.93±2.89)%處。結(jié)論這些參數(shù)可提高小腿三頭肌痙攣神經(jīng)溶解術(shù)的療效和效率。
[Abstract]:Objective to locate the nerve entry point (N point) of triceps calcaneus accurately and to provide anatomic basis for the clinical use of nerve block in the muscle spasm. Methods the lower limbs of 20 adult cadavers of 10 cadavers were supine. The line between the lateral epicondyle and the medial epicondyle of the femur and the lateral malleolus between the lateral condyle and the lateral malleolus of the femur is the transverse reference line (H line) and the longitudinal reference line (L line) of N point respectively. Dissecting the N points of the nerve branches of the triceps muscle of the calf, applying barium sulfate CT scanning. Syngo system to determine the projection point (P point) of the N point on the body surface and projecting the P 'point through the N point to the contralateral skin, the perpendicular line and the H line through the P point. The intersection of horizontal line and L line is recorded as PSP H and PSP L, respectively. The percent position and the depth of N point on the H and L lines of PSP _ H and P _ S _ L were measured respectively. Results the PSP of medial gastrocnemius head, lateral head and soleus muscle were (46.89 鹵2.73) of H line, (40.90 鹵3.05)% and (42.56 鹵2.59)% of that of L line, (7.58 鹵2.88)%, (8.15 鹵2.52)% and (17.42 鹵3.31)% of PP' line, (13.83 鹵1.77)% and (29.93 鹵2.89)%, respectively. Conclusion these parameters can improve the curative effect and efficiency of triceps muscle spasm neurolysis.
