等離子體射流聯(lián)合刮治術(shù)治療犬種植體周圍炎的實驗研究

【摘要】:目的:評價低溫等離子體射流聯(lián)合刮治術(shù)治療犬種植體周圍炎的臨床、微生物、放射學和組織學成果。 方法:拔除六只比格犬雙側(cè)下頜第一磨牙;三個月后種牙;三個月后埋線引導種植體周圍炎形成,三個月后當種植體支持骨吸收40%時,除去引導線;采用樹脂刮治器和0.2%氯己定葡萄糖酸鹽治療對照組,采用樹脂刮治器、0.2%氯己定葡萄糖酸鹽和等離子體噴流治療實驗組;在治療前和治療后三個月分別作齦溝出血指數(shù)、探診深度、牙槽骨喪失深度和CT檢查;在治療后三個月作Micro CT和硬組織切磨片檢查;在治療前和治療后1、2和3個月時,取微生物樣本并作聚合酶鏈式反應(yīng)(Polymerase chain reaction, PCR)分析。 結(jié)果:在治療后牙齦卟啉單胞菌(Porphyromonas gingivalis, Pg)和福賽擬桿菌(Bacteroides forsythus, Bf)的檢出率有顯著性下降(P0.05),實驗組和對照組之間的差異有統(tǒng)計學意義(P0.05);實驗組的齦溝出血指數(shù)(sulcus bleeding index, SBI)和探診深度(probing depth, PD)有顯著性下降(P0.05),分別從4.42±0.58和4.77±1.45mm下降到1.21±0.58和2.52±0.7 mm,實驗組和對照組之間的差異有統(tǒng)計學意義(P0.05);實驗組骨喪失深度(The distance from the shoulder of the implant to the bottom of the bony defect, DSB)有顯著性下降(P0.05),從3.35±0.70mm下降到2.21±0.90 mm,實驗組和對照組之間的差異有統(tǒng)計學意義(P0.05)。實驗組新生骨大量形成,并且與種植體發(fā)生骨整合,對照組新生骨形成較少。 結(jié)論:低溫等離子體噴流聯(lián)合刮治術(shù)治療犬種植體周圍炎取得了很好的治療效果,改善了臨床癥狀,減少了種植體周圍的細菌,促進了種植體和牙槽骨的再結(jié)合。
[Abstract]:Objective: to evaluate the clinical, microbiological, radiological and histological results of low temperature plasma jet combined curettage in the treatment of canine periimplant inflammation. Methods: the first mandibular molars were extracted from six Beagle dogs; the first mandibular molars were planted after 3 months; the implant thread implantation led to the formation of periimplant inflammation three months later and the guiding line was removed when the implant support bone resorption was 40 months later. The control group was treated with resin curette and 0.2% chlorhexidine gluconate, and the experimental group was treated with 0.2% chlorhexidine gluconate and plasma jet. The depth of diagnosis, the depth of alveolar bone loss and CT were examined. Micro CT and hard tissue cutting and grinding were performed three months after treatment, and microorganism samples were collected and polymerase chain reaction (PCR) chain reaction, PCR) analysis was performed before treatment and 1 and 3 months after treatment. Results: after treatment, the detection rate of Porphyromonas gingivalis (Pgg) and Bacteroides forsythus (BfF) decreased significantly (P 0.05), the difference between the experimental group and the control group was statistically significant (P 0.05), and the gingival sulcus bleeding index, SBI) of the experimental group was significantly lower than that of the control group. There was a significant decrease in the depth of probing (P 0.05), from 4.42 鹵0.58 and 4.77 鹵1.45mm to 1.21 鹵0.58 and 2.52 鹵0.7 mm, respectively. There was significant difference between the experimental group and the control group (P 0.05), and the depth of bone loss in the experimental group was significantly lower than that in the control group (P < 0.05). DSBs decreased significantly from 3.35 鹵0.70mm to 2.21 鹵0.90mm. The difference between the experimental group and the control group was statistically significant (P 0.05). In the experimental group, a large number of new bone formation, and bone integration with the implant, the control group new bone formation was less. Conclusion: low temperature plasma jet combined with curettage is effective in the treatment of dog periimplant inflammation, improves clinical symptoms, reduces bacteria around implant, and promotes the recombination of implant and alveolar bone.
