【摘要】：目的 了解湘雅系統(tǒng)大型綜合醫(yī)院青年臨床醫(yī)師的心理健康水平、積極心理狀況及相關(guān)影響因素,探討青年臨床醫(yī)師心理健康與積極心理之間的關(guān)系。 方法 對湘雅系統(tǒng)3家大型綜合醫(yī)院青年臨床醫(yī)師進行問卷調(diào)查,調(diào)查問卷包括：基本資料問卷、一般健康問卷、生活滿意度量表、人生意義問卷、六項感恩問卷、寬恕傾向量表、心理資本問卷。 結(jié)果 共發(fā)放測試問卷535份,回收問卷501份,有效問卷493份,問卷的有效率為98.40%。結(jié)果顯示：1、總體上,約25.15%的青年醫(yī)師存在心理問題,青年醫(yī)師在生活滿意度、人生意義、感恩、寬恕、心理資本上的得分處于中等稍偏上的水平。2、不同性別、年齡、婚姻狀況、學(xué)歷、職稱、工作時間、科室在各量表上的得分存在差異：(1)男女醫(yī)師心理健康得分無顯著差異；女醫(yī)生生活滿意度,寬恕他人及寬恕情境得分較男醫(yī)生高(p0.05),男醫(yī)生韌性得分較高(p0.05)；(2)已婚醫(yī)師在心理健康、生活滿意度、人生意義體驗、寬恕、心理資本上得分高于未婚醫(yī)生(p0.01)；(3)碩士研究生在心理健康及積極心理各問卷上得分較博士研究生低(p0.05)；(4)各科室心理健康得分無顯著差異；內(nèi)科醫(yī)師在生活滿意度、人生意義體驗、寬恕、心理資本上得分較高,而外科醫(yī)師較低(p0.05)；腫瘤科醫(yī)師感恩得分較高,而韌性得分較低(p0.05)；(5)隨著青年醫(yī)師年齡的增長,工作時間的延長以及職稱的晉升,在心理健康、生活滿意度、人生意義體驗、寬恕傾向及心理資本上得分呈現(xiàn)出逐漸增高的趨勢,而在人生意義尋找、感恩傾向上得分則呈現(xiàn)出逐漸降低的趨勢。3、心理健康得分與生活滿意度、人生意義感、感恩、寬恕自己、寬恕、心理資本得分之間存在顯著負相關(guān)關(guān)系(p0.01)；各積極心理因素之間存在正相關(guān)關(guān)系(絕大部加0.01)。4、對心理健康的多元逐步回歸分析顯示：青年醫(yī)師心理健康與生活滿意度、寬恕他人、人生意義體驗、寬恕自己、自我效能、感恩之間存在線性回歸關(guān)系,且均為負相關(guān)(p0.01)。 結(jié)論 1、湖南省湘雅系統(tǒng)大型綜合醫(yī)院青年醫(yī)師的心理健康水平較差,生活滿意度、人生意義感、感恩、寬恕、心理資本總體上處于中等偏上水平； 2、青年醫(yī)師的心理健康及積極心理狀況在不同性別、年齡、婚姻狀況、學(xué)歷、職稱、工作時間、科室上存在一定的差異。本調(diào)查主要顯示出如下特點：(1)男女醫(yī)師心理健康水平無明顯差異；女醫(yī)生生活滿意度,寬恕他人及寬恕情境傾向較男醫(yī)生高,男醫(yī)生韌性較女醫(yī)師強；(2)已婚青年醫(yī)師心理健康及積極心理狀況較未婚醫(yī)師好；(3)碩士研究生的心理健康及積極心理狀況較博士研究生差；(4)各科室青年臨床醫(yī)師心理健康水平無顯著差異；內(nèi)科醫(yī)師積極心理狀況較好,外科醫(yī)師較差；腫瘤科醫(yī)師感恩傾向較高,韌性較低；(5)隨著青年醫(yī)師年齡的增長,工作時間的延長以及職稱的晉升,在心理健康、生活滿意度、人生意義體驗、寬恕傾向及心理資本上呈現(xiàn)出逐漸上升、增強的趨勢,而在人生意義尋找、感恩傾向上則呈現(xiàn)出逐漸下降、減弱的趨勢。 3、心理健康水平與生活滿意度、人生意義感、感恩、寬恕、心理資本水平呈顯著正相關(guān),其中生活滿意度、寬恕他人、人生意義體驗、寬恕自己、自我效能、感恩是影響青年醫(yī)師心理健康的主要因素,并可在一定程度上預(yù)測心理健康；各積極心理因素之間以及它們與心理健康之間相互影響、相互促進。
[Abstract]:Purpose

To understand the mental health level , positive psychological condition and relevant influencing factors of young clinician in large - scale comprehensive hospital in Xiangya system , and discuss the relationship between mental health and positive psychology of young clinician .

method

Questionnaire , general health questionnaire , life satisfaction scale , life meaning questionnaire , six Thanksgiving questionnaires , forgiveness tendency scale and psychological capital questionnaire were investigated .

Results

Results : 1 . Overall , about 25.15 % of young doctors had psychological problems .
The scores of life satisfaction , forgiveness and forgiveness of women doctors were higher than those of male doctors ( p < 0.05 ) , and the toughness of male doctors was higher ( p < 0.05 ) .
( 2 ) The married physicians scored higher scores in mental health , life satisfaction , life meaning experience , forgiveness and psychological capital than unmarried doctors ( p0.01 ) ;
( 3 ) The master ' s post - graduate student scored less than the doctor ' s post - graduate degree in mental health and positive psychological questionnaire ( p . 05 ) ;
( 4 ) There was no significant difference in mental health scores of the departments ;
The physician scored higher in life satisfaction , life experience , forgiveness , psychological capital , while the surgeon was low ( p . 05 ) ;
There was a higher Thanksgiving score in the oncologist , while the toughness score was lower ( p < 0.05 ) .
( 5 ) With the increase of the age of the young doctor , the extension of the working time and the promotion of the job title , the scores of mental health , life satisfaction , life meaning experience , forgiveness tendency and psychological capital have gradually increased , while in the search for the meaning of life , the scores of gratitude tend to decrease gradually . 3 . There is a significant negative correlation between mental health score and life satisfaction , life sense , gratitude , forgiveness , forgiveness and psychological capital score ( p0.01 ) ;
There is positive correlation between the positive psychological factors ( absolute addition 0.01 ) . 4 . The multiple stepwise regression analysis of mental health shows that the mental health and life satisfaction of the young physician , the forgiveness of others , the life meaning experience , the forgiveness of oneself , the self - efficacy , and the feeling of the linear regression relationship , and the negative correlation ( p0.01 ) .

Conclusion

1 . The mental health level of young doctors in large - scale comprehensive hospitals in Xiangya System of Hunan Province is poor , life satisfaction , life sense , gratitude , forgiveness and psychological capital are generally in the middle level ;


2 . The mental health and the positive mental state of the young doctor are different in different sex , age , marital status , educational background , professional title , working time and department . This investigation mainly shows the following characteristics : ( 1 ) There is no obvious difference between the mental health level of male and female physicians ;
The female doctors ' life satisfaction , forgiveness of others and forgiveness situations tend to be higher than those of the male doctors , and the toughness of the male doctors is stronger than that of the female doctors ;
( 2 ) The mental health and positive mental status of married young doctors are better than those of unmarried physicians ;
( 3 ) The mental health and the positive mental state of the master ' s graduate students are worse than those of the doctoral students ;
( 4 ) There was no significant difference in mental health level among young clinical doctors in each department ;
The physician ' s positive mental condition is good , and the surgeon is poor ;
The physicians of the oncology department have higher sensitivity and lower toughness ;
( 5 ) With the increase of the age of the young doctor , the extension of the working time and the promotion of the job title , in the psychological health , the life satisfaction , the life meaning experience , the forgiveness tendency and the psychological capital , the trend of gradual rise and enhancement is presented .

3 . Mental health level is positively correlated with life satisfaction , life sense , gratitude , forgiveness and psychological capital , among which life satisfaction , forgiveness of others , life meaning experience , forgiveness of oneself , self - efficacy , gratitude are the main factors influencing the mental health of young doctors , and mental health can be predicted to some extent ;
The interaction between positive psychological factors and their relationship with mental health promotes each other .
