【摘要】：目的:探討完全型心內(nèi)膜墊缺損(ECD)尸檢解剖的難點和技巧。方法:選取安貞醫(yī)院病理科尸檢診斷完全型ECD的14例病例,與產(chǎn)前超聲檢查診斷結(jié)果對比,觀察心臟結(jié)構,并按Rastelli進行分型,明確房、室間隔缺損的范圍,房室瓣發(fā)育、瓣膜及腱索附著情況,觀察是否合并其他心內(nèi)畸形及其他臟器畸形;并測量心臟各項指標。結(jié)果:14例完全型ECD:1按Rastelli分型,A型12例,B型1例,C型1例;2單一完全型ECD 2例,僅合并其他心內(nèi)畸形者5例,僅合并其他臟器異常或畸形者2例,同時合并其他心內(nèi)畸形及其他臟器異�；蚧握�5例。313例與超聲診斷相同,1例不符。結(jié)論:掌握完全型ECD的分步解剖技巧,提高尸檢診斷準確率,對臨床及超聲診斷有一定的意義。
[Abstract]:Objective: to investigate the anatomic difficulties and techniques of complete endocardial cushion defect (ECD).Methods: fourteen cases of complete ECD diagnosed by autopsy in Department of Pathology of Anzhen Hospital were selected and compared with the results of prenatal ultrasonography. The cardiac structure was observed and classified according to Rastelli. The range of atrial and ventricular septal defect and the development of atrioventricular valve were determined.The attachment of valves and chordae tendineae was observed, and other cardiac deformities and other organ deformities were observed.Results according to Rastelli classification of 14 cases of complete type ECD:1, 12 cases of type A were classified as type A, 1 case of type C type and 1 case of single complete type ECD, only 5 cases were complicated with other intracardiac malformations, and only 2 cases were complicated with other organ abnormalities or deformities.5 cases with other heart malformations and other organ abnormalities or deformities were found to be inconsistent with ultrasound diagnosis in 1 case.Conclusion: mastering the anatomical skills of complete ECD and improving the accuracy of autopsy diagnosis are of certain significance for clinical and ultrasonic diagnosis.
