經(jīng)皮逆行髖臼前柱螺釘內(nèi)固定的計(jì)算機(jī)輔助應(yīng)用解剖學(xué)研究

【摘要】:[目的]測(cè)量經(jīng)皮逆行髖臼前柱螺釘內(nèi)固定的應(yīng)用解剖參數(shù)。[方法]收集本院2015年6月~2016年1月完整骨盆CT掃描數(shù)據(jù)男女各15例(男女各30側(cè)半骨盆),應(yīng)用Mimics計(jì)算機(jī)輔助外科軟件,重建骨盆三維模型。模擬手術(shù)逆行置入髖臼前柱虛擬螺釘,調(diào)整好螺釘位置后測(cè)量螺釘長(zhǎng)度、進(jìn)釘點(diǎn)、進(jìn)釘方向;通過(guò)軟件的重切割功能,制作35層髖臼前柱系列截面,測(cè)量截面中螺釘所通過(guò)位置(即釘?shù)?的直徑。[結(jié)果]髖臼前柱可置入螺釘?shù)淖畲蠊ぷ鏖L(zhǎng)度男女分別為(119.79±8.71)mm、(104.10±5.84)mm;進(jìn)釘點(diǎn)(P)到恥骨上支上緣(A)、恥骨聯(lián)合(B)的垂直距離男性分別為(19.69±2.79)mm、(18.80±3.35)mm,女性分別為(18.27±2.13)mm、(22.92±3.34)mm;螺釘與冠狀面、矢狀面、橫切面的夾角男性依次為(16.18±5.37)°、(41.76±4.78)°、(43.30±5.55)°,女性依次為(17.16±5.53)°、(43.12±3.90)°、(41.36±4.59)°;螺釘長(zhǎng)度和PB男女之間的差異具有統(tǒng)計(jì)學(xué)意義(P0.05),進(jìn)釘方向和PA男女之間的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。前柱釘?shù)雷瞠M窄處位于第5層,該層截面中釘?shù)乐睆侥信謩e為(11.43±1.24)mm、(8.75±0.95)mm,男女之間差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。[結(jié)論]作者建議國(guó)人男性使用直徑為6.5 mm的螺釘,女性使用直徑為5.5 mm的螺釘;進(jìn)釘點(diǎn)男女稍有不同,在螺釘經(jīng)過(guò)恥骨上支的中部和髂窩平面處兩個(gè)狹窄地方時(shí),應(yīng)加強(qiáng)監(jiān)視。
[Abstract]:[objective] to measure the applied anatomical parameters of percutaneous retrograde anterior column screw fixation of acetabular anterior column. [methods] 15 male and 15 male patients (30 sides of hemipelvis, male and female) with complete pelvic CT scan data from June 2015 to January 2016 were collected from our hospital. Computer aided surgery software, Reconstruction of the pelvis 3D model. Simulated surgery retrograde placement of the acetabular anterior column virtual screw, adjust the screw position after measuring the screw length, nail point, the direction of the nail, through the software recutting function, make 35 layers of acetabular anterior column series section, The diameter of the screw through the cross section was measured. [results] the maximum working length of the screw was 119.79 鹵8.71 mm for males and 104.10 鹵5.84 mm for males and females, respectively. It was 19.69 鹵2.79 mm and 18.80 鹵3.35 mm respectively, and that of female was 18.27 鹵2.13 mm and 22.92 鹵3.34 mm respectively. In sagittal plane and transverse plane, the angle between male and female was 16.18 鹵5.37 擄/ 41.76 鹵4.78 擄/ 43.30 鹵5.55 擄respectively, and that of female was 43.12 鹵3.90 擄/ 41.36 鹵4.59 擄respectively. The diameter of the nail passage in this layer was 11.43 鹵1.24 mm and 8.75 鹵0.95mm, respectively. The difference between male and female was statistically significant (P 0.05). [conclusion] the author suggests that the male should use the screw with the diameter of 6.5 mm and the female with the diameter of 5.5 mm, and the point of insertion is slightly different between the male and the female. Surveillance should be strengthened when screws pass through the middle of the superior pubic branch and at the level of the iliac fossa.
