骶外側(cè)動脈第一背側(cè)支的解剖和多普勒超聲探測的初步研究

【摘要】:目的旨在證實骶外側(cè)動脈第一背側(cè)支是骶髂關(guān)節(jié)背側(cè)的供血動脈,應用多普勒超聲探測該血管的正常血流動力學表現(xiàn)。方法對一具尸體進行解剖,明確骶外側(cè)動脈第一背側(cè)支的來源、分支及其供血區(qū)。再對50名志愿者行骶外側(cè)動脈第一背側(cè)支多普勒超聲探測,測量其血流動力學數(shù)據(jù),包括收縮期峰值流速(PSV)、舒張末期流速(EDV)和阻力指數(shù)(RI),比較不同性別間和左右兩側(cè)間骶外側(cè)動脈第一背側(cè)支的PSV、EDV和RI。由兩位超聲科醫(yī)師對30名志愿者進行重復測量,分析測量值的可重復性。結(jié)果在解剖的尸體標本中,骶外側(cè)動脈發(fā)出第一背側(cè)支,經(jīng)第一骶前孔由骶后孔穿出,隨后發(fā)出分支進入骶骨外側(cè)滋養(yǎng)孔。50名志愿者的100根骶外側(cè)動脈第一背側(cè)支均成功被探測到,其頻譜呈高阻單向波形。骶外側(cè)動脈第一背側(cè)支的PSV為(29.3±4.1)cm/s,EDV為(5.3±1.1)cm/s,RI為0.82±0.04。男、女性志愿者間骶外側(cè)動脈第一背側(cè)支的PSV、EDV和RI的差異均無統(tǒng)計學意義(P值均0.05),志愿者左、右兩側(cè)間骶外側(cè)動脈第一背側(cè)支的PSV、EDV和RI的差異均無統(tǒng)計學意義(P值均0.05)。兩位醫(yī)師測得的:PSV、EDV和RI的組內(nèi)相關(guān)系數(shù)分別為0.80、0.80和0.88,Bland-Altman圖顯示,除1名志愿者測得的PSV外,兩位醫(yī)師對骶外側(cè)動脈第一背側(cè)支血流動力學測量值的差值均位于95%的一致性界限內(nèi),表明兩位醫(yī)師的測量值的可重復性較好。結(jié)論骶外側(cè)動脈第一背側(cè)支是骶髂關(guān)節(jié)背側(cè)的供血動脈,多普勒超聲能有效探測該血管血流動力學表現(xiàn)。
[Abstract]:Objective to confirm that the first dorsal branch of the lateral sacral artery is the blood supply artery of the sacroiliac joint. The origin, branch and blood supply area of the first dorsal branch of the lateral sacral artery were determined. The hemodynamic data of the first dorsal branch of the lateral sacral artery were measured by Doppler ultrasound in 50 volunteers. Including peak systolic flow velocity (PSV), end-diastolic flow velocity (EDV) and resistance index (RI), the PSV of the first dorsal branch of the lateral sacral artery was compared between the sexes and between the left and right sides. EDV and RI. 30 volunteers were measured repeatedly by two ultrasound physicians to analyze the repeatability of the measured values. Results the first dorsal branch of the lateral sacral artery was found in the autopsy cadavers. Through the first anterior sacral foramen through the posterior sacral foramen, the 100 first dorsal branches of the lateral sacral artery were detected successfully in all the 100 volunteers who then issued branches into the lateral sacral trophoblastic foramen. The PSV of the first dorsal branch of the lateral sacral artery was 29.3 鹵4.1 cm / s, and the PSV of the first dorsal branch of the lateral sacral artery was 5.3 鹵1.1 cm / s. RI was 0.82 鹵0.04. There was no significant difference in EDV and RI between male and female volunteers in the first dorsal branch of the lateral sacral artery (P < 0.05). There was no significant difference of PSV EDV and RI in the first dorsal branch of the lateral sacral artery between the right and the right sides (P = 0.05). The intra-group correlation coefficients of EDV and RI were 0.80 and 0.88Bland-Altman, respectively, except for the PSV measured by one volunteer. The difference in hemodynamic measurements of the first dorsal branch of the lateral sacral artery between the two physicians was within the consistent limit of 95%. Conclusion the first dorsal branch of the lateral sacral artery is the blood supply artery of the sacroiliac joint, and Doppler ultrasound can effectively detect the hemodynamic manifestation of the blood vessel.
