豬脊髓栓系綜合征神經(jīng)軸性牽拉模型的建立

【摘要】:目的探討建立豬脊髓、神經(jīng)根軸性牽拉損傷模型模擬脊髓栓系綜合征的方法。方法將18只成年家豬隨機分為3組,每組6只。假手術(shù)組:暴露L1~5脊髓;對照組:暴露L1~5脊髓后于L1~5雙側(cè)椎弓根置入螺釘;實驗組:暴露L1~5脊髓并于雙側(cè)椎弓根置入螺釘后,均勻撐開L1~5椎間隙。于各時間點對3組動物行X線片、后肢行為學、尿流動力學、體感誘發(fā)電位、激光散斑血流成像及組織病理學檢查并對結(jié)果進行比較分析。結(jié)果假手術(shù)組和對照組術(shù)前、術(shù)后L1~5椎體間高度、后肢運動神經(jīng)功能評分、尿流動力學、體感誘發(fā)電位及激光散斑血流成像檢查結(jié)果均無明顯變化,組織學觀察脊髓、神經(jīng)結(jié)構(gòu)基本正常。實驗組動物撐開后L1~5的椎體間高度較撐開前增高,后肢運動神經(jīng)功能評分降低,體感誘發(fā)電位P波的潛伏期延長、波幅下降,脊髓表面的血流減少(P0.05);撐開后出現(xiàn)低順應性膀胱、逼尿肌-括約肌協(xié)同功能障礙;脊髓組織可見不同程度的神經(jīng)細胞壞死,胞質(zhì)內(nèi)可見顆粒變性和空泡變性;神經(jīng)根組織可見組織水腫、細胞結(jié)構(gòu)疏松,部分神經(jīng)束膜損傷。結(jié)論通過松解豬腰椎后柱結(jié)構(gòu)均勻撐開椎間隙的方法可以建立脊髓、神經(jīng)根軸性牽拉損傷模型,該模型能夠模擬臨床脊髓栓系綜合征患者的下肢感覺、運動功能和括約肌功能障礙,為深入探討脊髓栓系綜合征的病理機制以及手術(shù)治療方式奠定了實驗基礎(chǔ)。
[Abstract]:Objective to establish pig spinal cord, nerve root axial stretch injury model simulation method of tethered cord syndrome. Methods 18 adult pigs were randomly divided into 3 groups, 6 rats in each group. Sham operation group: exposure of L1~5 spinal cord; control group: exposure of L1~5 spinal cord at L1~5 after bilateral pedicle screw placement; experimental group: the exposure of L1~5 spinal cord and bilateral pedicle screw, uniform distraction L1~5 intervertebral space. At each time point of 3 groups of animal X-ray, hindlimb behavior, urodynamics, somatosensory evoked potential, laser speckle imaging and histopathological examination and the results were compared. Results the sham operation group and the control group before surgery, postoperative L1~5 intervertebral height, hindlimb motor function scores, urodynamics, somatosensory evoked potentials and laser speckle imaging examination results showed no significant changes, histological observation of spinal cord nerve, the normal structure. The experimental group of animals Open the L1~5 material interbody distraction height is increased before, hind limb motor function scores decreased, somatosensory evoked potential P wave latency, amplitude decreased, reduce the surface of the spinal cord blood flow (P0.05); after the unfolding of low compliance bladder, detrusor sphincter coordination dysfunction; spinal cord tissue showed different degrees of nerve cell necrosis in the cytoplasm of granular degeneration and vacuolar degeneration; nerve root tissue edema, cell structure is loose, part of perineurial injury. Conclusion by loose column structure solution of porcine lumbar intervertebral disc after uniform distraction can establish the spinal cord, nerve root axial stretch injury model, the model can simulate clinical spinal cord syndrome of patients with lower limb sensory, motor function and sphincter dysfunction, which lays the foundation for further study on pathological mechanism of tethered spinal cord syndrome and surgical treatment.
