【摘要】：目的:探討免疫檢查點抑制劑(immune checkpoint blockade,ICB)的不良反應(yīng)及其處理原則。方法:采取文獻回顧的方法對ICB出現(xiàn)的不良反應(yīng)及處理原則進行歸納總結(jié)。結(jié)果:ICB的不良反應(yīng)主要是其特有的免疫相關(guān)不良反應(yīng)(immune-related adverse events,ir AEs),可累及全身多個臟器和系統(tǒng),可發(fā)生在治療全過程甚至治療結(jié)束后,其處理原則包括預(yù)防、評估、檢查、治療和監(jiān)測,早期的識別和檢測是管理的關(guān)鍵。結(jié)論:雖然大多數(shù)ir AEs通過延遲給藥及皮質(zhì)內(nèi)固醇激素治療可以控制并且可逆,但有些ICB的不良反應(yīng)可帶來嚴重后果,因此臨床醫(yī)生應(yīng)實行患者教育,早期識別并及時處理,以提高腫瘤免疫治療的安全性。
[Abstract]:Objective: To investigate the immune checkpoint inhibitors (Immune checkpoint, blockade, ICB) the adverse reactions and treatment principles. Methods: summary of adverse reactions and treatment principles adopted the method of literature review of ICB. Results: the main adverse reaction of ICB is its unique immune related adverse reactions (immune-related adverse events, IR AEs), the body can affect multiple organs and systems, can occur in the whole treatment process even after the end of treatment, the treatment principles including prevention, assessment, examination, treatment and monitoring, identification and early detection is the key to management. Conclusion: Although the majority of IR AEs by delaying the administration of corticosteroids and hormone therapy can control and reversible, but the adverse reaction of some ICB can have serious consequences, so clinicians should implement the patient education, early recognition and timely treatment, in order to improve the safety of tumor immunotherapy .
