【摘要】:背景忠誠一詞可以被定義為表征人們?cè)诟杏X、情感或習(xí)慣上始終如一的附屬物。2004年法國引入"全科醫(yī)生宣言(首選醫(yī)生宣言)",因?yàn)槠湫枰ㄟ^國家醫(yī)療保險(xiǎn)實(shí)現(xiàn)醫(yī)療服務(wù)的最優(yōu)覆蓋,所以法國采用了一種正式激勵(lì)機(jī)制來鼓勵(lì)患者忠實(shí)于他們自己的醫(yī)生。關(guān)于評(píng)估該措施的影響并確定醫(yī)患間忠誠度的組成部分的研究還是一個(gè)空白。目的探討如何建立并維持患者對(duì)其全科醫(yī)生的忠誠度。設(shè)計(jì)與場所基于對(duì)臨近法國巴黎地區(qū)(伊夫林省的行政部門)的半結(jié)構(gòu)化訪談,進(jìn)行定性研究。方法在5個(gè)具有不同人口統(tǒng)計(jì)學(xué)特征的個(gè)體經(jīng)營的全科醫(yī)生的診療室中,對(duì)28例患者進(jìn)行訪談。轉(zhuǎn)錄訪談內(nèi)容,并對(duì)數(shù)據(jù)進(jìn)行主題分析及分類。采用現(xiàn)象學(xué)分析法分析轉(zhuǎn)錄本。結(jié)果患者忠誠度主要基于信任。信任可以通過某些令人欣慰的因素得以加強(qiáng),如傾聽的能力、細(xì)心的意識(shí)和護(hù)理的質(zhì)量。忠誠既是一種動(dòng)態(tài)構(gòu)建,又是一種關(guān)系的轉(zhuǎn)換,受多種因素影響。患者發(fā)現(xiàn)保持忠誠的優(yōu)勢。"家庭醫(yī)生"的模式一直是一個(gè)家族幾代人對(duì)于患者忠實(shí)于自己的醫(yī)生的原型。全科醫(yī)生不具備滿足患者所有要求的能力不一定是打破患者忠誠的決定因素。結(jié)論患者對(duì)于醫(yī)生的忠誠度比通常假設(shè)的要復(fù)雜的多,其涉及信任、傾聽、護(hù)理質(zhì)量,有效性和熟悉程度等多個(gè)維度。從本研究中得出的觀察結(jié)論值得進(jìn)行更大規(guī)模的調(diào)查。
[Abstract]:The word background loyalty can be defined as a representation of people feeling, Affective or habitually consistent appendages. In 2004 France introduced the General Practitioners' Declaration (the Declaration of doctors of first Choice) because it requires optimal coverage of health services through national health insurance. Therefore, France has adopted a formal incentive mechanism to encourage patients to be faithful to their own doctors. There is still no research on assessing the impact of the measure and determining the components of patient-doctor loyalty. How to establish and maintain patients' loyalty to their general practitioners. Design and location are based on semi-structured interviews with the administrative department in the Evelyn region, near Paris, France. Methods 28 patients were interviewed in 5 self-employed general practitioners with different demographic characteristics. Subject analysis and classification of the data. Analysis of transcripts by phenomenological analysis. Results patients' loyalty is mainly based on trust. Trust can be enhanced by some comforting factors, such as listening ability. Careful consciousness and the quality of care. Loyalty is both a dynamic construction and a transformation of relationships, The pattern of "family doctor" has always been the prototype of a family of generations loyal to their own doctor. General practitioners do not have to meet all the needs of patients. Ability is not necessarily a determinant of patient loyalty. Conclusion patients' loyalty to their doctors is much more complex than is usually assumed. It involves multiple dimensions of trust, listening, quality of care, effectiveness and familiarity. The findings from this study merit a larger survey.
