目的研究腹腔鏡輔助直腸前切除術(shù)后發(fā)生吻合口出血的相關(guān)因素,并制定預防策略。方法回顧性分析2011年1月至2018年12月在瑞金醫(yī)院胃腸外科行腹腔鏡輔助直腸前切除術(shù)的病例的臨床資料,結(jié)合單因素及多因素分析,探究術(shù)后吻合口出血發(fā)生的相關(guān)因素,并探討不同的術(shù)中預防措施的效果。結(jié)果335例術(shù)中未行預防措施的腹腔鏡輔助直腸前切除術(shù)患者平均年齡為63.6±11.9歲（26-91歲）,其中男性患者230例（68.66%）,女性患者105例（31.34%）。腹腔鏡輔助直腸前切除術(shù)后吻合口出血的發(fā)生率為5.37%（18/335）。單因素分析結(jié)果顯示,腹腔鏡輔助直腸前切除術(shù)后吻合口出血的發(fā)生與吻合口距離肛門的距離有關(guān)（P<0.05）,與性別、年齡、BMI、術(shù)前合并腸梗阻、術(shù)前新輔助放化療、術(shù)前血清白蛋白水平、術(shù)前癌胚抗原（carcinoembryonic antigen,CEA）水平、保留左結(jié)腸動脈、保護性造口、腫瘤直徑、腫瘤分期、圓形吻合器直徑無明顯關(guān)系。多因素分析結(jié)果顯示,低位吻合（P=0.020,OR=5.983,95%CI:1.331-26.885）是吻合口出血的獨立危險因素。吻合口漏的發(fā)生...
