鼻咽部EB病毒核酸標(biāo)志物檢測在鼻咽癌分子診斷中的應(yīng)用研究

目的:鼻咽癌(Nasopharyngeal carcinoma,NPC)是一種高侵襲性、高轉(zhuǎn)移性的惡性腫瘤,在中國南方和東南亞地區(qū)廣泛流行。臨床上鼻咽癌的早診早治成為提高患者生存率、改善預(yù)后的關(guān)鍵突破口。尋找和建立簡單可靠的生物學(xué)標(biāo)志物應(yīng)用于高發(fā)區(qū)鼻咽癌的早期診斷和篩查是亟待解決的重要問題。已有充分的研究表明,EB病毒(Epstein-barr virus,EBV)感染在鼻咽癌的發(fā)病中扮演著非常重要的角色,鼻咽刷取樣結(jié)合EB病毒核酸標(biāo)志物檢測可能會為高發(fā)區(qū)鼻咽癌的診斷和篩查提供新的研究思路。我們開展該項(xiàng)研究的目的主要有:(1)探討鼻咽刷取樣結(jié)合EBV DNA檢測對中國華南區(qū)高發(fā)的鼻咽癌是否具有較好的診斷應(yīng)用價(jià)值。(2)明確鼻咽刷樣本中EBV的產(chǎn)生來源,以確定是否是腫瘤細(xì)胞來源反應(yīng)鼻咽癌的發(fā)生,為鼻咽部EB病毒核酸標(biāo)志物的發(fā)掘和利用提供清晰的理論基礎(chǔ)。(3)發(fā)掘適用于輔助鼻咽癌診斷的核酸標(biāo)志物,尤其適用于在高發(fā)現(xiàn)場不依賴臨床醫(yī)生和醫(yī)療設(shè)備的盲刷取樣條件下的標(biāo)志物,以利于在高發(fā)現(xiàn)場人群篩查中的應(yīng)用。方法:本項(xiàng)研究以鼻咽癌作為研究疾病模型,通過與地處中國華南地區(qū)鼻咽癌高發(fā)區(qū)的中山大學(xué)腫瘤防治...

鼻咽活檢組織的EBERs原位雜交染色代表性結(jié)果
為了確認(rèn)鼻咽刷取樣能獲得鼻咽部的腫瘤細(xì)胞或正常鼻咽上皮細(xì)胞,我們開展了10例(5例來自鼻咽癌患者,5例來自鼻咽黏膜炎病例)鼻咽刷的涂片實(shí)驗(yàn),并進(jìn)行HE染色。從實(shí)驗(yàn)結(jié)果中看到鼻咽癌患者的鼻咽刷樣本中,刷取的主要有腫瘤細(xì)胞和淋巴細(xì)胞組份,而正常對照中或鼻咽黏膜炎中,刷取到的是正常的鼻咽上皮細(xì)胞和大量的淋巴細(xì)胞,這與活檢組織獲得的細(xì)胞學(xué)類型基本保持一致,提示鼻咽刷取樣的可行性。為了進(jìn)一步確定鼻咽刷取樣和鼻咽活檢組織具有一致性,開展了10例鼻咽刷取樣和活檢組織的EBERs原位雜交比對實(shí)驗(yàn),結(jié)果顯示無論是在鼻咽癌患者還是對照組中,EBERs原位雜交的結(jié)果在鼻咽刷涂片和活檢組織兩組間都保持一致性,即在鼻咽癌患者中,EBERs原位雜交結(jié)果顯示強(qiáng)陽性,提示鼻咽活檢組織和鼻咽刷涂片細(xì)胞中有EBV的感染,而在對照組樣本中,原位雜交結(jié)果都顯示陰性,進(jìn)一步說明了鼻咽刷取樣的可行性。
