【摘要】：背景和目的：網(wǎng)狀Meta分析可以比較同一疾病多個干預(yù)措施的有效性和安全性,基于排序結(jié)果篩選最佳干預(yù)措施。研究顯示網(wǎng)狀Meta分析的報告質(zhì)量較低,影響其生產(chǎn)證據(jù)的使用。目前網(wǎng)狀Meta分析報告量表并不全面,不同報告量表間的差異較大,不能有效規(guī)范網(wǎng)狀Meta分析的報告。因此有必要制定全面規(guī)范詳細的網(wǎng)狀Meta分析報告量表,并采用科學(xué)的方法進行驗證。方法：基于系統(tǒng)全面的文獻檢索,納入英文網(wǎng)狀Meta分析,采取社會網(wǎng)絡(luò)分析和文獻計量學(xué)方法分析其應(yīng)用現(xiàn)狀；比較網(wǎng)狀Meta分析的報告量表,分析不同量表間的差異,評估其全面性；納入涉及網(wǎng)狀Meta分析報告的文獻或量表,整合網(wǎng)狀Meta分析的報告條目；隨機抽取25%的網(wǎng)狀Meta分析,以整合后的報告條目評估其報告質(zhì)量,進行探索性因子分析制定網(wǎng)狀Meta分析的報告量表；納入外科學(xué)網(wǎng)狀Meta分析,以該量表評價其報告質(zhì)量,并進行信度分析。采用SPSS 21.0軟件進行統(tǒng)計分析,信度分析主要測量Cronbach α系數(shù)、Spearman-Brown系數(shù)和Guttman分半系數(shù)。Cronbach α系數(shù)0.8,表示量表的內(nèi)部一致性很高,信度較好。Spearman-Brown系數(shù)和Guttman分半系數(shù)大于0.7就認為量表信度較高。結(jié)果：納入522篇網(wǎng)狀Meta分析。網(wǎng)狀Meta分析的發(fā)表量逐年上升,大多數(shù)關(guān)注慢性疾病的藥物治療。共納入6個網(wǎng)狀Meta分析的報告量表,比較發(fā)現(xiàn),6個量表均不全面,對網(wǎng)狀Meta分析的報告要求并不一致。大部分量表只要求報告方法和結(jié)果部分,并不要求報告背景和討論�；�15篇文獻的系統(tǒng)評價,共收集47個網(wǎng)狀Meta分析報告條目,并與PRISMA報告條目進行整合,最終獲取55個報告條目。采用此量表評估25%的網(wǎng)狀meta分析(113篇),結(jié)果顯示網(wǎng)狀Meta分析的報告并不充分和全面,尤其是方法學(xué)部分：45篇(39.82%)網(wǎng)狀Meta分析清楚地報告研究篩選過程,68篇(60.18%)網(wǎng)狀Meta分析清楚地報告資料提取過程,61篇(53.98%)網(wǎng)狀Meta分析清楚地報告質(zhì)量評價過程。10篇(8.85%)網(wǎng)狀Meta分析清楚地報告臨床和方法學(xué)異質(zhì)性的評估,54篇(47.8%)網(wǎng)狀Meta分析清楚地報告統(tǒng)計學(xué)異質(zhì)性的評估和處理方法,7篇(6.19%)網(wǎng)狀Meta分析清楚地報告相似性評估方法,18篇(15.93%)網(wǎng)狀Meta分析清楚地報告不一致性的檢測和處理方法�；趫蟾尜|(zhì)量評估結(jié)果的探索性因子分析,獲取34個網(wǎng)狀Meta分析報告條目,即網(wǎng)狀Meta分析的報告量表。以73篇外科學(xué)網(wǎng)狀Meta分析(涉及6種治療措施和11種疾病)的報告質(zhì)量為基礎(chǔ)進行信度分析,Cronbach α值為0.84,即該量表可以獲取84%網(wǎng)狀Meta分析真實的報告情況；Spearman-Brown系數(shù)為0.82, Guttman分半系數(shù)為0.82。結(jié)論：網(wǎng)狀Meta分析的報告質(zhì)量較差,特別是關(guān)鍵部分報告不充分�；谙到y(tǒng)評價和探索性因子分析制定的網(wǎng)狀Meta分析報告量表含34個條目,信度分析顯示其具有較高信度,可以用來規(guī)范網(wǎng)狀Meta分析。但未來研究仍需要在其他領(lǐng)域進一步驗證網(wǎng)狀Meta分析報告量表的信度和效度。
[Abstract]:BACKGROUND & OBJECTIVE : A meta - analysis of mesh Meta - analysis can compare the effectiveness and safety of multiple intervention measures of the same disease , and screen best interventions based on ranking results .
To compare the report scale of mesh meta - analysis , to analyze the difference between different scales and to evaluate its comprehensiveness ;
include a literature or scale related to a mesh meta - analysis report , an integrated mesh meta - analysis report entry ;
A mesh meta - analysis of 25 % was randomly selected to assess the quality of its report with the integrated report entry , and a report scale of mesh meta - analysis was developed for exploratory factor analysis ;
A meta - analysis was conducted with SPSS 21 . 0 software . The results showed that the reliability of the scale was high and the reliability was good . The results showed that there were only a total of 47 mesh Meta - analysis reports .
Conclusion : There are 34 entries in the mesh meta - analysis report scale based on systematic evaluation and exploratory factor analysis . The reliability analysis shows that it has higher reliability and can be used to standardize the mesh meta - analysis . However , future research still needs to further verify the reliability and validity of the mesh meta - analysis report scale in other fields .
