網(wǎng)狀Meta分析報(bào)告量表的制定與驗(yàn)證

【摘要】:背景和目的:網(wǎng)狀Meta分析可以比較同一疾病多個(gè)干預(yù)措施的有效性和安全性,基于排序結(jié)果篩選最佳干預(yù)措施。研究顯示網(wǎng)狀Meta分析的報(bào)告質(zhì)量較低,影響其生產(chǎn)證據(jù)的使用。目前網(wǎng)狀Meta分析報(bào)告量表并不全面,不同報(bào)告量表間的差異較大,不能有效規(guī)范網(wǎng)狀Meta分析的報(bào)告。因此有必要制定全面規(guī)范詳細(xì)的網(wǎng)狀Meta分析報(bào)告量表,并采用科學(xué)的方法進(jìn)行驗(yàn)證。方法:基于系統(tǒng)全面的文獻(xiàn)檢索,納入英文網(wǎng)狀Meta分析,采取社會(huì)網(wǎng)絡(luò)分析和文獻(xiàn)計(jì)量學(xué)方法分析其應(yīng)用現(xiàn)狀;比較網(wǎng)狀Meta分析的報(bào)告量表,分析不同量表間的差異,評(píng)估其全面性;納入涉及網(wǎng)狀Meta分析報(bào)告的文獻(xiàn)或量表,整合網(wǎng)狀Meta分析的報(bào)告條目;隨機(jī)抽取25%的網(wǎng)狀Meta分析,以整合后的報(bào)告條目評(píng)估其報(bào)告質(zhì)量,進(jìn)行探索性因子分析制定網(wǎng)狀Meta分析的報(bào)告量表;納入外科學(xué)網(wǎng)狀Meta分析,以該量表評(píng)價(jià)其報(bào)告質(zhì)量,并進(jìn)行信度分析。采用SPSS 21.0軟件進(jìn)行統(tǒng)計(jì)分析,信度分析主要測(cè)量Cronbach α系數(shù)、Spearman-Brown系數(shù)和Guttman分半系數(shù)。Cronbach α系數(shù)0.8,表示量表的內(nèi)部一致性很高,信度較好。Spearman-Brown系數(shù)和Guttman分半系數(shù)大于0.7就認(rèn)為量表信度較高。結(jié)果:納入522篇網(wǎng)狀Meta分析。網(wǎng)狀Meta分析的發(fā)表量逐年上升,大多數(shù)關(guān)注慢性疾病的藥物治療。共納入6個(gè)網(wǎng)狀Meta分析的報(bào)告量表,比較發(fā)現(xiàn),6個(gè)量表均不全面,對(duì)網(wǎng)狀Meta分析的報(bào)告要求并不一致。大部分量表只要求報(bào)告方法和結(jié)果部分,并不要求報(bào)告背景和討論;15篇文獻(xiàn)的系統(tǒng)評(píng)價(jià),共收集47個(gè)網(wǎng)狀Meta分析報(bào)告條目,并與PRISMA報(bào)告條目進(jìn)行整合,最終獲取55個(gè)報(bào)告條目。采用此量表評(píng)估25%的網(wǎng)狀meta分析(113篇),結(jié)果顯示網(wǎng)狀Meta分析的報(bào)告并不充分和全面,尤其是方法學(xué)部分:45篇(39.82%)網(wǎng)狀Meta分析清楚地報(bào)告研究篩選過(guò)程,68篇(60.18%)網(wǎng)狀Meta分析清楚地報(bào)告資料提取過(guò)程,61篇(53.98%)網(wǎng)狀Meta分析清楚地報(bào)告質(zhì)量評(píng)價(jià)過(guò)程。10篇(8.85%)網(wǎng)狀Meta分析清楚地報(bào)告臨床和方法學(xué)異質(zhì)性的評(píng)估,54篇(47.8%)網(wǎng)狀Meta分析清楚地報(bào)告統(tǒng)計(jì)學(xué)異質(zhì)性的評(píng)估和處理方法,7篇(6.19%)網(wǎng)狀Meta分析清楚地報(bào)告相似性評(píng)估方法,18篇(15.93%)網(wǎng)狀Meta分析清楚地報(bào)告不一致性的檢測(cè)和處理方法;趫(bào)告質(zhì)量評(píng)估結(jié)果的探索性因子分析,獲取34個(gè)網(wǎng)狀Meta分析報(bào)告條目,即網(wǎng)狀Meta分析的報(bào)告量表。以73篇外科學(xué)網(wǎng)狀Meta分析(涉及6種治療措施和11種疾病)的報(bào)告質(zhì)量為基礎(chǔ)進(jìn)行信度分析,Cronbach α值為0.84,即該量表可以獲取84%網(wǎng)狀Meta分析真實(shí)的報(bào)告情況;Spearman-Brown系數(shù)為0.82, Guttman分半系數(shù)為0.82。結(jié)論:網(wǎng)狀Meta分析的報(bào)告質(zhì)量較差,特別是關(guān)鍵部分報(bào)告不充分。基于系統(tǒng)評(píng)價(jià)和探索性因子分析制定的網(wǎng)狀Meta分析報(bào)告量表含34個(gè)條目,信度分析顯示其具有較高信度,可以用來(lái)規(guī)范網(wǎng)狀Meta分析。但未來(lái)研究仍需要在其他領(lǐng)域進(jìn)一步驗(yàn)證網(wǎng)狀Meta分析報(bào)告量表的信度和效度。

A mesh meta - analysis of 25 % was randomly selected to assess the quality of its report with the integrated report entry , and a report scale of mesh meta - analysis was developed for exploratory factor analysis ;
A meta - analysis was conducted with SPSS 21 . 0 software . The results showed that the reliability of the scale was high and the reliability was good . The results showed that there were only a total of 47 mesh Meta - analysis reports .
Conclusion : There are 34 entries in the mesh meta - analysis report scale based on systematic evaluation and exploratory factor analysis . The reliability analysis shows that it has higher reliability and can be used to standardize the mesh meta - analysis . However , future research still needs to further verify the reliability and validity of the mesh meta - analysis report scale in other fields .
