【摘要】:隨著信息技術(shù)的發(fā)展,當(dāng)前醫(yī)療機(jī)構(gòu)電子化越來(lái)越普及,為了更好地提供或得到醫(yī)療服務(wù),實(shí)現(xiàn)臨床信息的共享和交換必不可少。但是目前醫(yī)療系統(tǒng)間異構(gòu),數(shù)據(jù)標(biāo)準(zhǔn)、傳輸格式、數(shù)據(jù)格式不統(tǒng)一的問(wèn)題嚴(yán)重阻礙了臨床信息傳遞與交換,患者的臨床信息無(wú)法得到充分利用,造成了醫(yī)療機(jī)構(gòu)之間無(wú)法進(jìn)行溝通。臨床信息的交換與共享,可以使醫(yī)療服務(wù)人員獲得病人任何時(shí)間、任何地點(diǎn)的相關(guān)信息。并且,臨床數(shù)據(jù)的集成是實(shí)現(xiàn)分布式環(huán)境下臨床決策支持系統(tǒng)的前提,各個(gè)醫(yī)療機(jī)構(gòu)的臨床醫(yī)生可以根據(jù)集成的病人臨床信息數(shù)據(jù)對(duì)病人做出更合理的管理決策,從而有效地降低醫(yī)療差錯(cuò),全方位的對(duì)病人提供醫(yī)療服務(wù)。本文基于HL7 CDA標(biāo)準(zhǔn)定義了通用型的臨床文檔結(jié)構(gòu)模板以及CDA和數(shù)據(jù)庫(kù)的映射解決了臨床信息交換這一問(wèn)題。此外,醫(yī)療機(jī)構(gòu)的臨床醫(yī)生存在知識(shí)局限性和人為疏忽的問(wèn)題,臨床決策支持系統(tǒng)的使用可以有效地解決此類問(wèn)題,并且臨床決策支持系統(tǒng)的應(yīng)用可以提供更好的醫(yī)療服務(wù),同時(shí)降低醫(yī)療費(fèi)用。因此,在醫(yī)療服務(wù)中,充分發(fā)揮知識(shí)庫(kù)和臨床決策支持的優(yōu)勢(shì),可以提高臨床醫(yī)生使用臨床支持系統(tǒng)積極性,提高醫(yī)療服務(wù)水平。本文以高血壓作為案例,設(shè)計(jì)了高血壓用藥的知識(shí)庫(kù),研究并設(shè)計(jì)了分布式環(huán)境下高血壓患者用藥的臨床決策支持系統(tǒng)。本文首先通過(guò)研究HL7醫(yī)療信息傳輸標(biāo)準(zhǔn)和臨床文檔結(jié)構(gòu)CDA標(biāo)準(zhǔn),用CDA標(biāo)準(zhǔn)對(duì)臨床信息文檔進(jìn)行包裝,基于CDA和數(shù)據(jù)庫(kù)的雙向轉(zhuǎn)換思想,實(shí)現(xiàn)了CDA文檔的生成與解析,從而實(shí)現(xiàn)了異構(gòu)系統(tǒng)中臨床信息的交換。其次,本文在實(shí)現(xiàn)異構(gòu)系統(tǒng)數(shù)據(jù)集成和互操作的基礎(chǔ)上,設(shè)計(jì)了一種通用的分布式環(huán)境下的臨床決策支持系統(tǒng)的整體框架,并對(duì)其中的關(guān)鍵組成模塊進(jìn)行了介紹。最后,為了驗(yàn)證設(shè)計(jì)的正確性,本文在湖北省人口健康信息平臺(tái)的基礎(chǔ)上,使用多代理技術(shù)協(xié)調(diào)各個(gè)醫(yī)療機(jī)構(gòu)之間的工作,選取高血壓作為案例,對(duì)知識(shí)庫(kù)和數(shù)據(jù)庫(kù)進(jìn)行了定義,對(duì)患者的臨床場(chǎng)景進(jìn)行了描述,實(shí)現(xiàn)了分布式環(huán)境下的臨床信息交換和臨床決策支持。實(shí)現(xiàn)的結(jié)果證明了本文提出的方法可以為分布式環(huán)境下的臨床決策支持系統(tǒng)提供病人的集成數(shù)據(jù),結(jié)合知識(shí)庫(kù)做出更準(zhǔn)確的決策,同時(shí),對(duì)于解決分布式環(huán)境下臨床決策過(guò)程中的臨床信息的獲取的問(wèn)題具有重大的意義。
[Abstract]:With the development of information technology, electronic medical institutions are becoming more and more popular. In order to better provide or get medical services, it is necessary to share and exchange clinical information. But at present, medical systems are heterogeneous. The problem of data standard, transmission format and data format is not uniform, which seriously hinders the transmission and exchange of clinical information, and the patient's clinical information can not be fully utilized. The exchange and sharing of clinical information enables medical service personnel to obtain relevant information about the patient at any time, anywhere. And. The integration of clinical data is the premise of realizing the clinical decision support system in the distributed environment. The clinicians in various medical institutions can make more reasonable management decisions on patients according to the integrated patient clinical information data. In order to effectively reduce medical errors. To provide medical services to patients. This paper is based on HL7. The CDA standard defines the generic clinical document structure template and the mapping between CDA and database to solve the problem of clinical information exchange. Clinical doctors in medical institutions have the problems of limited knowledge and human negligence. The use of clinical decision support system can effectively solve such problems. And the application of clinical decision support system can provide better medical services and reduce medical costs. Therefore, in medical services, the advantages of knowledge base and clinical decision support can be brought into full play. It can improve the enthusiasm of clinical doctors to use the clinical support system and improve the level of medical service. This paper studies and designs the clinical decision support system for patients with hypertension under distributed environment. Firstly, this paper studies the HL7 medical information transmission standard and the clinical document structure CDA standard. The clinical information document is packaged with CDA standard. Based on the idea of bidirectional conversion between CDA and database, the generation and parsing of CDA document are realized. In order to achieve the heterogeneous system of clinical information exchange. Secondly, this paper realizes the heterogeneous system data integration and interoperability on the basis of. A general framework of clinical decision support system in distributed environment is designed, and the key modules are introduced. Finally, in order to verify the correctness of the design. On the basis of population health information platform in Hubei province, this paper uses multi-agent technology to coordinate the work among medical institutions, selects hypertension as a case, and defines the knowledge base and database. The clinical scene of the patient was described. The clinical information exchange and clinical decision support in the distributed environment are realized. The results show that the proposed method can provide integrated patient data for the clinical decision support system in the distributed environment. It is of great significance to make more accurate decisions based on the knowledge base and to solve the problem of obtaining clinical information in the process of clinical decision in distributed environment.
