【摘要】：目的:探討內(nèi)鏡下逆行胰膽管造影術(shù)(Endoscopic retrograde cholangiopancreatography,ERCP)所致的常見并發(fā)癥術(shù)后胰腺炎(Post-ERCP Pancreatitis,PEP)和高淀粉酶血癥(Hyperamylasemia)臨床危險(xiǎn)因素。方法:對(duì)2014年1月—2016年10月于廣西醫(yī)科大學(xué)第一附屬醫(yī)院行ERCP且符合納入標(biāo)準(zhǔn)的共919例行回顧性分析,篩選出8種因素進(jìn)行X~2檢驗(yàn)及Logistics回歸分析。結(jié)果:本文研究納入病例共919例,其中正常組病人722例,PEP組44例,發(fā)生率為4.79%(44/919);術(shù)后高淀粉酶血癥組153例,發(fā)生率為16.64%(153/919)。PEP的單因素分析得出的危險(xiǎn)因素為:行EST、ENBD、EPBD術(shù)、既往有胰腺炎病史;多因素分析得出的危險(xiǎn)因素為:既往有胰腺炎病史。術(shù)后高淀粉酶血癥的單因素分析得出危險(xiǎn)因素為:存在EST、EPBD、既往有胰腺炎病史;多因素分析得出危險(xiǎn)因素為:男性、存在EST、EPBD、膽總管支架植入術(shù)、既往有胰腺炎史。結(jié)論:1.PEP的危險(xiǎn)因素為:有胰腺炎病史;2.ERCP術(shù)后高淀粉酶血癥的危險(xiǎn)因素為:男性、有胰腺炎病史、行EST、行EPBD、行膽總管支架植入術(shù)。
[Abstract]:Objective: to investigate the clinical risk factors of postoperative pancreatitis (Post-ERCP Pancreatitis,PEP) and hyperamylase (Hyperamylasemia) induced by endoscopic retrograde cholangiopancreatography (Endoscopic retrograde cholangiopancreatography,ERCP). Methods: a total of 919 cases of ERCP from January 2014 to October 2016 in the first affiliated Hospital of Guangxi Medical University were retrospectively analyzed and 8 factors were selected for Xan2 test and Logistics regression analysis. Results: a total of 919 cases were included in this study, of which 44 cases (4.79%) were in normal group and 153 cases in hyperamylase group (16.64% (153 / 919). The risk factors were as follows: EST,ENBD,EPBD was performed. The previous history of pancreatitis and the risk factors of multivariate analysis were as follows: previous history of pancreatitis. Univariate analysis of postoperative hyperamylase showed that EST,EPBD, had a history of pancreatitis in the past, and multivariate analysis showed that the risk factors were: male, EST,EPBD, common bile duct stent implantation, and history of pancreatitis. Conclusions 1. The risk factors of PEP were as follows: having history of pancreatitis. 2. The risk factors of hyperamylase after ERCP were: male, history of pancreatitis, EST, with EPBD, and common bile duct stent implantation.
