【摘要】：研究目的:根據(jù)佛山市名中醫(yī)丘青中教授30多年臨床經(jīng)驗(yàn)自擬方劑補(bǔ)血祛瘀湯用于PFNA術(shù)后治療股骨粗隆間骨折(轉(zhuǎn)子間骨折;IFF)導(dǎo)致隱性失血的療效,探討補(bǔ)血祛瘀湯對PFNA術(shù)治療股骨粗隆間骨折圍手術(shù)期隱性失血的影響,為隱性失血的預(yù)防及治療提供參考。研究方法:本研究采納隨機(jī)數(shù)字法將40名符合本研究中相關(guān)標(biāo)準(zhǔn)的患者分為中藥組、對照組兩組,每組20個,中藥組在入院后至術(shù)后第5d服用補(bǔ)血祛瘀湯,對照組只作一般常規(guī)術(shù)后抗感染、補(bǔ)液處理等處理,入院后第ld,術(shù)后第1、3、5d復(fù)查血常規(guī),記下血紅蛋白及紅細(xì)胞壓積,用Gross法計(jì)算出隱性失血量,采用Harris髖關(guān)節(jié)評分系統(tǒng)對術(shù)后1周、2周、1月患側(cè)髖關(guān)節(jié)進(jìn)行評分。研究結(jié)果:(1)與對照組相比,中藥組術(shù)后第3天,術(shù)后第5天血紅蛋白較高,差異存在統(tǒng)計(jì)學(xué)意義(P0.05)。(2)與對照組相比,中藥組術(shù)后第3天,術(shù)后第5天血紅細(xì)胞壓積(Hct)較高,差異存在統(tǒng)計(jì)學(xué)意義(P0.05)。(3)與對照組對比,在Harris髖關(guān)節(jié)評分評分中,補(bǔ)血祛瘀湯對髖關(guān)節(jié)功能恢復(fù)無統(tǒng)計(jì)學(xué)意義(P0.05)(4)與對照組相比,中藥組術(shù)后第3天,術(shù)后第5天隱性失血量減少,差異存在統(tǒng)計(jì)學(xué)意義(P0.05)。研究結(jié)論:(1)PFNA術(shù)治療老年股骨粗隆間骨折可導(dǎo)致發(fā)生大量的隱性失血。(2)補(bǔ)血祛瘀湯能有效地提高血紅蛋白量,減少隱性失血。
[Abstract]:Objective: according to more than 30 years clinical experience of Professor Qiu Qing Zhong of Foshan City, the effect of self-made prescription Buxue Quyu decoction for treating recessive blood loss caused by intertrochanteric fracture of femur (IFF) after PFNA operation was studied. To explore the effect of Bu Xue Quyu decoction (BXQT) on recessive blood loss in perioperative period of femoral intertrochanteric fracture treated by PFNA, and to provide reference for prevention and treatment of recessive blood loss. Methods: in this study, 40 patients who met the relevant criteria in this study were randomly divided into traditional Chinese medicine group and control group with 20 in each group. The traditional Chinese medicine group was treated with Buxue Quyu decoction from admission to the 5th day after operation. The control group was only treated with general routine anti-infection and fluid resuscitation. The blood routine was re-examined on the first day of admission and the first 3th day after operation. Hemoglobin and hematocrit were recorded and recessive blood loss was calculated by Gross's method. Harris hip scoring system was used to evaluate the affected hip at 1 week, 2 weeks and 1 month after operation. Results: (1) compared with the control group, the hemoglobin in the Chinese medicine group was higher on the 3rd day and 5th day after operation (P0.05). (2), compared with the control group, the hematocrit (HCT) was higher in the traditional Chinese medicine group on the 3rd and 5th day after operation. The difference was statistically significant (P0.05). (3) compared with the control group. In the score of). (hip joint, there was no significant difference in the recovery of hip joint function by BXQYD (P0.05) (4) compared with the control group, the recessive blood loss decreased on the 3rd day and the 5th day after operation in the TCM group. The difference was statistically significant (P0.05). Conclusion: (1) PFNA can cause a large amount of recessive blood loss in the treatment of senile femoral intertrochanteric fracture. (2) Buxue Quyu decoction can effectively increase hemoglobin content and reduce recessive blood loss.
