【摘要】：目的運用循證醫(yī)學(xué)探討我國極低出生體質(zhì)量早產(chǎn)兒并發(fā)支氣管肺發(fā)育不良的危險因素。方法系統(tǒng)檢索Pub Med、Embase、萬方數(shù)據(jù)庫、中國期刊全文數(shù)據(jù)、中國知網(wǎng)(CNKI)、中文科技期刊全文數(shù)據(jù)、Cochrane臨床對照試驗資料庫、中國生物醫(yī)學(xué)文獻數(shù)據(jù)庫(CBM),檢索時間均從建庫至2016年5月。按納入排除標(biāo)準(zhǔn)進行隨機對照試驗的篩選、資料提取和文獻質(zhì)量評價,應(yīng)用Stata 11.0軟件進行meta分析。結(jié)果共納入15篇對照試驗研究,mate分析結(jié)果顯示,胎膜早破(OR=1.90,95%CI:1.16~3.11),肺透明膜病(OR=6.46,95%CI:4.47~9.35),吸氧時間(RR=3.01,95%CI:1.34~4.69),肺部感染(OR=15.76,95%CI:7.51~33.07),生后窒息(OR=2.23,95%CI:1.09~4.57),產(chǎn)前感染(OR=3.41,95%CI:2.40~4.84),動脈導(dǎo)管未閉(OR=3.39,95%CI:2.04~5.64),胎齡(RR=-2.34,95%CI:-3.42~-1.27),出生體質(zhì)量(RR=-2.08,95%CI:-2.91~-1.21)是極低出生體質(zhì)量早產(chǎn)兒并發(fā)支氣管肺發(fā)育不良的危險因素;產(chǎn)后使用肺表面活性物質(zhì)(OR=4.09,95%CI:1.65~10.12)是其保護因素。結(jié)論應(yīng)積極對極低出生體質(zhì)量早產(chǎn)兒并發(fā)支氣管肺發(fā)育不良危險因素采取預(yù)防措施,從而降低支氣管肺發(fā)育不良的發(fā)生率,提高生存率及改善遠(yuǎn)期生活質(zhì)量。
[Abstract]:Objective to explore the risk factors of bronchopulmonary dysplasia in very low birth weight preterm infants in China. Methods Pub Medbase, Wanfang database, full text data of Chinese journals, CNKII of China, Cochrane clinical controlled trial database of Chinese scientific and technological journals, CBMN of Chinese biomedical literature database were searched systematically. The retrieval time was from the establishment of the database to May 2016. According to exclusion criteria, random controlled trial screening, data extraction and literature quality evaluation were carried out, and meta analysis was carried out with Stata 11.0 software.
