【摘要】：目的:探討血清補(bǔ)體C1q水平對(duì)類風(fēng)濕關(guān)節(jié)炎活動(dòng)度的診斷價(jià)值。方法:收集160例RA患者(活動(dòng)組110例,緩解組50例)的外周靜脈血,測(cè)定其血清補(bǔ)體C1q水平,同時(shí)檢測(cè)與RA疾病活動(dòng)度相關(guān)的指標(biāo)ESR、hs-CRP、RF以及CCP的表達(dá),并采用SPSS19.0軟件進(jìn)行比較分析。結(jié)果:RA患者活動(dòng)組C1q、ESR、hs-CRP水平高于緩解組,且差異具有統(tǒng)計(jì)學(xué)意義(P0.05),ESR、hs-CRP與C1q具有相關(guān)性(P0.05);RA患者活動(dòng)組RF陽(yáng)性率高于緩解組(P0.05),CCP陽(yáng)性率與緩解組無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),C1q與RF具有相關(guān)性(P0.05)而與CCP沒(méi)有相關(guān)性(P0.05);C1q、ESR、hs-CRP、RF與DAS28具有顯著相關(guān)性(P0.05),CCP與DAS28相關(guān)性不顯著。補(bǔ)體C1q診斷RA患者活動(dòng)期的ROC曲線下面積為0.87,最佳截?cái)嘀禐?24.5mg/l,此時(shí)靈敏度為0.81,特異度為0.80。C1q診斷RA患者活動(dòng)期的靈敏度為0.71,特異度為0.80,Youden指數(shù)為50.9,陽(yáng)性似然比為3.5,陰性似然比為0.36。將C1q、ESR、hs-CRP、RF四項(xiàng)指標(biāo)聯(lián)合試驗(yàn),得出靈敏度為0.87,特異度為0.92,正確指數(shù)為0.79,Kappa值0.65。結(jié)論:血清補(bǔ)體C1q可以作為評(píng)價(jià)RA活動(dòng)度的指標(biāo),C1q、ESR、hs-CRP、RF四項(xiàng)指標(biāo)聯(lián)合檢測(cè)對(duì)診斷RA活動(dòng)度的價(jià)值比單獨(dú)任一指標(biāo)的診斷價(jià)值高。
[Abstract]:Objective: to investigate the diagnostic value of serum complement C 1q level in rheumatoid arthritis (RA). Methods: peripheral venous blood was collected from 160 RA patients (active group 110 cases, remission group 50 cases), and serum complement C1q level was measured. The expression of ESRHs-CRPnP RF and CCP related to disease activity of RA was detected at the same time. SPSS19.0 software is used to compare and analyze. Results the levels of ESRHs-CRP in the active group of patients with 0: RA were higher than those in the remission group. The positive rate of RF in the active group was higher than that in the remission group. There was no significant difference between P0.05C1q and RF (P0.05). There was no correlation between P0.05Hs-CRP and CCP, but there was no significant correlation between P0.05ESRHs-CRP1q and DAS28 in patients with RA. The correlation between CCP and DAS28 was not significant. The area under the ROC curve of complement C1q was 0.87, the best truncation value was 224.5 mg / l, the sensitivity was 0.81, the specificity was 0.71, the specificity was 0.80 Youden index 50.9, the positive likelihood ratio was 3.5, and the negative likelihood ratio was 0.366.The specificity was 0.71, the specificity was 0.80 Youden index, the positive likelihood ratio was 3.5, and the negative likelihood ratio was 0.361.The sensitivity was 0.71, the specificity was 0.80 Youden index, the positive likelihood ratio was 3.5, and the negative likelihood ratio was 0.36. Conclusion: serum complement C1q can be used as an index to evaluate the activity of RA. The diagnostic value of serum complement C1q in diagnosing RA activity is higher than that of any single index.
