【摘要】：研究目的:本課題基于督脈理論與臨床,研究適用于針灸臨床的督脈診察方法,并對其診察方法的規(guī)范與流程進(jìn)一步深入研究,使診察方法更貼近針灸臨床,更易于指導(dǎo)診療。同時,通過研究督脈診察方法與規(guī)程,將進(jìn)一步推動針灸標(biāo)準(zhǔn)化的研究,尤其是針灸臨床診療標(biāo)準(zhǔn)化與規(guī)范化的研究進(jìn)程。研究內(nèi)容:本課題主要研究與分析了下列督脈診察方法,并對其操作步驟、操作要點(diǎn),診察要素以及操作流程與規(guī)范分別進(jìn)行論述:(1)視診:頸部視診、背部視診。(2)觸診:單手拇指觸診、單手雙指滑觸診、單手三指滑觸診、雙手四指觸診、雙手拇指觸診、小魚際觸診、小魚際加壓觸診。(3)罐診。研究意義:督脈診察方法與規(guī)程的研究,初步形成了一套較為完整的督脈診察體系,為臨床診治脊柱病以及脊柱相關(guān)病提供了新的思路,并且填補(bǔ)了中醫(yī)經(jīng)絡(luò)診察中有關(guān)督脈診察的空白。醫(yī)者進(jìn)行督脈診察時不再局限于只運(yùn)用傳統(tǒng)的脊柱檢查,醫(yī)者在診察過程中更有主導(dǎo)性,更有利于醫(yī)者對患者督脈以及病癥的把握。另外,本課題也詳細(xì)描述了針灸臨床中經(jīng)常見到的診察要素,通過規(guī)范化的診察手法將會獲取更多有效的診察要素,指導(dǎo)針灸取穴。最后,本課題認(rèn)為,壓痛點(diǎn)不是督脈的唯一陽性反應(yīng)點(diǎn)。醫(yī)者對于各類疾病在督脈背段陽性反應(yīng)點(diǎn)規(guī)律性研究,可以不再拘泥于探尋壓痛點(diǎn)。本課題中的督脈診察要素提供了更加豐富和易于獲得的陽性反應(yīng)點(diǎn),課題組認(rèn)為,督脈背段陽性反應(yīng)點(diǎn)的規(guī)律性可以從壓痛點(diǎn)延伸到棘突的偏歪、棘突間隙的變化以及罐印的變化等等。
[Abstract]:Objective: based on the theory and clinic of du pulse, this paper studies the diagnosis method of du pulse which is suitable for acupuncture and moxibustion clinic, and makes further research on the standard and flow of diagnosis method, so that the diagnosis method is more close to acupuncture clinic, and it is easier to guide the diagnosis and treatment. At the same time, the research on the standardization of acupuncture and moxibustion, especially the standardization of clinical diagnosis and treatment of acupuncture and moxibustion, will be promoted by studying the methods and procedures of du pulse diagnosis. Research content: this subject mainly studies and analyzes the following methods of monitoring pulse diagnosis, and discusses the operation steps, operating points, diagnostic elements, operation flow and standard, respectively. Palpation: one hand thumb palpation, one hand two fingers sliding palpation, one hand three fingers sliding palpation, two hands four fingers palpation, two hands thumb palpation, little thenar pressure palpation. Research significance: the study on the methods and procedures of du pulse diagnosis has preliminarily formed a set of relatively complete monitoring system, which provides a new way of thinking for clinical diagnosis and treatment of spinal diseases and spinal related diseases. And filled in the meridian diagnosis of traditional Chinese medicine about the inspection of the gap. The traditional spinal examination is no longer limited when the doctor carries on the inspector pulse examination, the doctor is more dominant in the process of diagnosis, which is more helpful for the doctor to grasp the patient's blood vessel and the disease. In addition, this subject also describes in detail the diagnostic elements that are often seen in acupuncture and moxibustion clinic. Through standardized diagnostic techniques, more effective diagnostic elements will be obtained to guide acupuncture and moxibustion to obtain acupoints. Finally, this subject believes that tenderness point is not the only positive reaction point of du pulse. The study on the regularity of positive reaction points in the dorsal segment of the du meridian can not be confined to the search for tenderness points. The examination elements of du pulse in this subject provide more abundant and easy to obtain positive reaction points. The study group believes that the regularity of positive reaction points in the dorsal segment of du veins can be extended from tenderness points to skewness of spinous process. The change of spinous process gap and the change of jar seal, etc.
