【摘要】：目的:本次研究以了解目前南京市開展兒童保健服務(wù)的現(xiàn)狀為主要目的。為不斷調(diào)整和完善目前的兒童保健衛(wèi)生服務(wù)體系提供科學(xué)依據(jù)。方法:對《2013-2015年江蘇省衛(wèi)生統(tǒng)計年鑒》中涉及南京市兒童保健服務(wù)狀況的數(shù)據(jù)進(jìn)行描述性分析,結(jié)合分析情況進(jìn)行實證研究。選取建鄴區(qū)、鼓樓區(qū)、江寧區(qū)、六合區(qū)4個社區(qū)服務(wù)中心及南京市婦幼保健院、江蘇省婦幼保健院作為調(diào)查現(xiàn)場,共對329名7歲以下兒童家長進(jìn)行了調(diào)查訪問。問卷內(nèi)容涉及三方面:基于模糊綜合評價法的兒童保健服務(wù)滿意度評價,家長對兒童保健服務(wù)利用率的影響因素研究,南京市社區(qū)兒童保健服務(wù)項目推行情況。研究的數(shù)據(jù)資料采用單因素方差和Logistic回歸分析(檢驗水準(zhǔn)α=0.05)。對南京市兒童保健服務(wù)滿意度的整體評價采用模糊綜合評價法。對兒童保健的服務(wù)組織、服務(wù)技術(shù)、服務(wù)態(tài)度、服務(wù)連續(xù)性、服務(wù)費用、服務(wù)效率6個一級指標(biāo)分別進(jìn)行評分。結(jié)合各級指標(biāo)權(quán)重結(jié)果得出南京市兒童保健服務(wù)滿意度分值。結(jié)果:對兒童保健信息關(guān)注情況做有序Logistic回歸分析中我們發(fā)現(xiàn)文化程度、年齡特點成為影響兒童保健信息關(guān)注程度的重要影響因素。對社區(qū)兒童保健服務(wù)利用率做單因素及非條件Logistic逐步回歸分析,分析結(jié)果提示從需方角度看,家庭收入、文化程度、對社區(qū)衛(wèi)生服務(wù)中心兒童保健服務(wù)知曉度成為影響社區(qū)兒童保健服務(wù)利用率的顯著因素(p0.05)。從供方角度看,醫(yī)師水平、專業(yè)設(shè)備、報銷比例、就醫(yī)距離成為制約社區(qū)兒童保健服務(wù)利用的重要因素。南京市兒童保健服務(wù)的滿意度評價的總體分值為71分介于"一般"和"滿意"之間。在兒童保健服務(wù)滿意度指標(biāo)測評中,服務(wù)組織、服務(wù)技術(shù)、服務(wù)態(tài)度、服務(wù)連續(xù)性、服務(wù)價格、服務(wù)效率測評的滿意度分值分別為67.96,80.22,71.33,74.95,69.01,74.00。結(jié)論:從研究的結(jié)論來看近三年的南京市兒童服務(wù)建設(shè)取得了一定進(jìn)展,兒童保健水平不斷提高。但在基層社區(qū)兒童保健服務(wù)的推行和利用上普及度不高,一定程度上影響了對整體兒童服務(wù)保健的滿意度評價。在評價的二級指標(biāo)中,服務(wù)組織和服務(wù)費用兩方面家長滿意度較低,提示需要相關(guān)部門應(yīng)進(jìn)一步完善收費制度,豐富服務(wù)項目,落實推行分級診療制度。
[Abstract]:Objective: to understand the present situation of child health service in Nanjing. To continuously adjust and improve the current child health care service system to provide scientific basis. Methods: the data of Jiangsu Provincial Health Statistics Yearbook from 2013 to 2015 were analyzed by descriptive analysis and the empirical study was carried out in combination with the situation of Nanjing children's health care service. Four community service centers in Jianye District, Gulou District, Jiangning District, Liuhe District, Nanjing Maternal and Child Health Hospital and Jiangsu Provincial Maternal and Child Health Hospital were selected as the investigation site. A total of 329 parents of children under 7 years old were interviewed. The contents of the questionnaire involved three aspects: the satisfaction evaluation of child health care based on fuzzy comprehensive evaluation, the study on the influencing factors of parents' utilization rate of child health care service, and the implementation of the project of community child health service in Nanjing. The data of the study were analyzed by single factor variance and Logistic regression analysis (test level 偽 0. 05). The fuzzy comprehensive evaluation method was used to evaluate the satisfaction degree of children's health care service in Nanjing. Children's health care service organization, service technology, service attitude, service continuity, service cost, service efficiency were evaluated. According to the results of index weight at all levels, the satisfaction degree of Nanjing child health care service was obtained. Results: in an ordered Logistic regression analysis, we found that the educational level and age characteristics were the important factors influencing the attention level of children's health information. Univariate and unconditioned Logistic stepwise regression analysis was performed on the utilization rate of community child health care services. The results indicated that, from the perspective of demand side, family income and education level, The awareness of child health care services in community health service centers was a significant factor affecting the utilization rate of community child health care services. From the point of view of suppliers, the level of doctors, professional equipment, reimbursement ratio and distance from medical treatment are the important factors restricting the utilization of community children's health care services. The overall score of satisfaction evaluation of child health care in Nanjing was 71 between "average" and "satisfactory". In the evaluation of children's health care service satisfaction index, the satisfaction scores of service organization, service technology, service attitude, service continuity, service price and service efficiency were 67.966 / 80.222.71.33 / 74.951 / 69.01 / 74.00 respectively. Conclusion: according to the conclusion of the study, the construction of children's services in Nanjing has made some progress in recent three years, and the level of children's health care has been improved. However, the implementation and utilization of child health care services in the grass-roots community is not high, which to some extent affects the evaluation of the overall child health care satisfaction. In the secondary index of evaluation, the satisfaction of parents in service organization and service cost is low, which suggests that the relevant departments should further improve the charging system, enrich the service items, and implement the classified diagnosis and treatment system.
