【摘要】：研究目的:本文通過對朱佳教授配伍蟲類藥治療慢性咳嗽的臨床醫(yī)案進行數據挖掘,系統(tǒng)總結和歸納朱佳教授治療慢性咳嗽的總體思想和靈活運用蟲類藥配伍的特色,為慢性咳嗽的中醫(yī)診療提供思路和方法。研究方法:收集、整理朱佳教授2011年1月至2016年11月臨床配伍蟲類藥診治慢性咳嗽的醫(yī)案,共涉及364例病例,累計386診次。采用頻數分析、關聯(lián)規(guī)則分析的數據挖掘方法,結合朱佳教授的臨證經驗對挖掘結果進行分析、歸納和總結。結果:數據挖掘頻數分析結果顯示386診次中男性105例108診次,女性259例278次。慢性咳嗽的主要臨床表現(xiàn)為咳嗽、咽喉癢、痰白、陣咳、痰少、干咳,在所有診次中分別占 91.19%、48.96%、34.46%、31.87%、29.27%、27.20%;病機多為風邪戀肺、風痰阻肺、肺脾兩虛等,在所有診次中分別占36.27%、24.61%、11.14%;關聯(lián)規(guī)則分析結果顯示蟲類藥(蟬蛻、僵蠶、蜈蚣、全蝎、地龍)與臨床癥狀、舌象、脈象、病機外關聯(lián)都存在著較強的關聯(lián)性。結論:通過研究發(fā)現(xiàn),慢性咳嗽以風痰阻肺、風邪戀肺為主要病機,久咳不愈常出現(xiàn)肺脾兩虛等虛證,屬于本虛標實。蟲類藥(蟬蛻、僵蠶、蜈蚣、全蝎、地龍)既可祛外風,又善息內風,還可化痰散結,主要應用在咽喉癢、陣咳、干咳、痰少、痰白、夜間咳甚,舌淡紅或紅,苔薄或薄黃,脈細弦或細滑,辨證為風邪戀肺證、風痰阻肺證的慢性咳嗽患者。
[Abstract]:Objective: through the data mining of professor Zhu Jia's clinical records of treating chronic cough, the author summarized and summarized the general thought of professor Zhu Jia's treatment of chronic cough and the characteristics of flexible application of insect medicine compatibility To provide ideas and methods for the diagnosis and treatment of chronic cough. Methods: a total of 364 cases involving 364 cases of chronic cough were collected and sorted out by Professor Zhu Jia from January 2011 to November 2016. The data mining method based on frequency analysis and association rule analysis is used to analyze and summarize the mining results combined with Professor Zhu Jia's clinical experience. Results: the data mining frequency analysis showed that 105 cases (108 times) were male and 259 cases (278 times) were women. The main clinical manifestations of chronic cough are cough, itching of throat, phlegm white, phlegm, dry cough, which account for 91.199,48.96 and 34.8729.277.20 respectively in all visits. The pathogenesis of chronic cough is mostly wind evil love of lung, wind phlegm blocking lung, deficiency of lung and spleen, etc. The results of association rule analysis showed that insect drugs (cicada slough, Bombyx bassiana, centipede, scorpion, Geosaurus) had strong correlation with clinical symptoms, tongue, pulse and pathogenesis. Conclusion: the main pathogenesis of chronic cough is wind phlegm blocking lung and wind evil falling in love with lung. Deficiency syndrome such as deficiency of lung and spleen and deficiency of lung and spleen are often found in chronic cough. Insect medicine (cicada slough, silkworm, centipede, scorpion, earth dragon) can not only quell the outside wind, but also soothe the internal wind, but also can dissipate phlegm and disperse the knot. It is mainly used in throat itching, coughing, dry cough, phlegm less, phlegm white, night cough very, tongue pale red or red, thin or thin yellow moss. Pulse fine string or fine smooth, syndrome differentiation is wind evil love lung syndrome, wind phlegm block lung chronic cough patients.
