【摘要】：研究目的本論文主要包含兩大部分內(nèi)容:其一、葛根黃芩黃連湯方證的文獻(xiàn)綜述、理論研究、現(xiàn)代臨床應(yīng)用與實(shí)驗(yàn)室研究。其二、葛根黃芩黃連湯的循證醫(yī)學(xué)與系統(tǒng)研究。全面檢索、收集、閱讀、統(tǒng)計(jì)、篩選出葛根黃芩黃連湯的隨機(jī)對(duì)照試驗(yàn)臨床研究文獻(xiàn),從結(jié)果中發(fā)現(xiàn)葛根黃芩黃連湯在臨床上主要用來(lái)治療小兒輪狀病毒性腸炎。在此結(jié)果基礎(chǔ)上,具有相同研究?jī)?nèi)容與目地之研究文獻(xiàn)進(jìn)行分類(lèi),依據(jù)文獻(xiàn)資料共分為2組。使用Cochrane系統(tǒng)提供的Meta-analysis統(tǒng)計(jì)軟體對(duì)2組數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。最后,遵循循證醫(yī)學(xué)對(duì)每個(gè)疾病提出最佳的治療方案原則,小兒輪狀病毒性腸炎設(shè)計(jì)出一套建議治療方案。研究對(duì)象1.發(fā)表于1949年-2017年03月之間的葛根黃芩黃連湯的期刊文獻(xiàn)。2.葛根黃芩黃連湯、中西醫(yī)結(jié)合治療小兒輪狀病毒性腸炎的隨機(jī)對(duì)照試驗(yàn)期刊文獻(xiàn)。研究方法1.題目確立:基于中醫(yī)學(xué)辨證論治之基礎(chǔ)與西醫(yī)內(nèi)科學(xué)對(duì)小兒輪狀病毒的定義,研究題目確立為"葛根黃芩黃連湯治療小兒輪狀病毒性腸炎隨機(jī)對(duì)照試驗(yàn)之RevMan Meta分析評(píng)價(jià)"。2.文獻(xiàn)資料檢索方法:6個(gè)主題詞:葛根黃芩黃連湯、葛根芩連湯、葛根黃芩黃連加減方、復(fù)方葛根黃芩黃連湯、葛根芩連湯加減方、復(fù)方葛根芩連湯。2個(gè)關(guān)鍵詞:小兒輪狀病毒、小兒輪狀病毒治療。3.數(shù)據(jù)處理:使用Cochrane系統(tǒng)提供的Meta-analysis統(tǒng)計(jì)軟體對(duì)2組數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。研究?jī)?nèi)容使用Cochrane系統(tǒng)提供的Meta-analysis統(tǒng)計(jì)軟體分析葛根黃芩黃連湯治療小兒輪狀病毒性腸炎之二分類(lèi)變量(dichotomous)、連續(xù)性變量(continuous)數(shù)據(jù)分析。1.二分類(lèi)變量:1.1葛根黃芩黃連湯vs單純西醫(yī)基礎(chǔ)療法1.2葛根黃芩黃連湯+西醫(yī)基礎(chǔ)療法VS西醫(yī)基礎(chǔ)療法2.連續(xù)性變量:中西醫(yī)結(jié)合治療VS西醫(yī)基礎(chǔ)療法之小兒輪狀病毒性腸炎的臨床癥狀治愈時(shí)間比較分析:2.1止瀉時(shí)間SMD2.2退熱時(shí)間SMD2.3嘔吐消失時(shí)間SMD2.4脫水糾正時(shí)間SMD2.5腹脹消失時(shí)間SMD2.6總治療療程時(shí)間SMD研究結(jié)果使用Cochrane系統(tǒng)提供的Meta-analysis統(tǒng)計(jì)軟體分析葛根黃芩黃連湯治療小兒輪狀病毒性腸炎之治療療效,二分類(lèi)變量(dichotomous)、連續(xù)性變量(continuous)數(shù)據(jù)分析統(tǒng)計(jì)結(jié)果如下:1.二分類(lèi)變量:1.1葛根黃芩黃連湯單純西醫(yī)基礎(chǔ)療法1.2葛根黃芩黃連湯+西醫(yī)基礎(chǔ)療法西醫(yī)基礎(chǔ)療法2.連續(xù)性變量:2.1中西醫(yī)結(jié)合治療之止瀉時(shí)間SMD西醫(yī)基礎(chǔ)療法之止瀉時(shí)間SMD2.2中西醫(yī)結(jié)合治療之退熱時(shí)間SMD西醫(yī)基礎(chǔ)療法之退熱時(shí)間SMD2.3中西醫(yī)結(jié)合治療之嘔吐消失時(shí)間SMD西醫(yī)基礎(chǔ)療法之嘔吐消失時(shí)間SMD2.4中西醫(yī)結(jié)合治療之脫水糾正時(shí)間SMD西醫(yī)基礎(chǔ)療法之脫水糾正時(shí)間SMD2.5中西醫(yī)結(jié)合治療之腹脹消失時(shí)間SMD西醫(yī)基礎(chǔ)療法之腹脹消失時(shí)間SMD2.6中西醫(yī)結(jié)合治療之總治療療程時(shí)間SMD西醫(yī)基礎(chǔ)療法之總治療療程時(shí)間SMD研究成果與小兒輪狀病毒性腸炎建議治療方案結(jié)論就此統(tǒng)計(jì)結(jié)果,筆者與臨床經(jīng)驗(yàn)豐富且治療療效卓越導(dǎo)師陳明教授對(duì)小兒輪狀病毒設(shè)計(jì)出一套建議治療方案,方案如下:1.中藥處方:葛根黃芩黃連湯(葛根、黃芩、黃連、甘草)2.兼證配伍:2.1脾虛:白術(shù)、山藥、白扁豆2.2濕勝:蒼術(shù)、澤瀉、茯苓、車(chē)前子2.3脾胃氣滯:木香、柴胡2.4脾胃虛寒:白術(shù)、干姜2.5嘔惡苔膩:半夏、藿香2.6熱毒血痢:金銀花炭3.西醫(yī)基礎(chǔ)療法:糾正電解質(zhì)及酸堿平衡紊亂、營(yíng)養(yǎng)支持。
[Abstract]:This paper mainly contains two main parts: first, the literature review, the theoretical study, the modern clinical application and the laboratory study of Gegen Huanglian decoction, the second, the evidence-based medicine and the systematic study of Gegen Huanglian decoction. The random control trial of the Radix Scutellariae Decoction of Radix Scutellariae is selected. The results of the results showed that kudzu Scutellaria Huanglian soup was mainly used to treat children with rotavirus enteritis. On the basis of the results, 2 groups were divided into 2 groups according to the literature. The Meta-analysis software provided by the Cochrane system was used for the 2 groups of data. Finally, according to the principle of the best treatment plan for each disease by evidence-based medicine, a set of recommended treatment scheme was designed for children with rotavirus enteritis.
