【摘要】：目的:觀察低能量放散式體外沖擊波針灸治療原發(fā)性痛經(jīng)(Primary Dysmenorrhea,PD)的療效以及治療后對患者體內(nèi)血清前列腺素E2(PGE2)、前列腺素F2α(PGF2α)濃度的影響或者改變,以期為臨床治療原發(fā)性痛經(jīng)提供新的治療方法、治療思路。方法:招募原發(fā)性痛經(jīng)患者20例作為試驗組,未發(fā)生過痛經(jīng)的女性20例作為提供正常血清前列腺素水平的對照組。試驗組于月經(jīng)排卵期前后1-2日,選取三陰交、足三里、關(guān)元穴位;腹直肌趾骨聯(lián)合附著點、腹外斜肌等部位進行沖擊波針灸治療。將月經(jīng)排卵期左右接受1次治療視為1個療程,治療1個療程后隨訪6個月并評估其治療效果。試驗組分別于治療前后第1次月經(jīng)來潮24-48小時內(nèi)靜脈采血;正常對照組于月經(jīng)來潮24-48小時內(nèi)靜脈采血,測定血清前列腺素E2、血清前列腺素F2α的濃度,并比較二者在試驗組接受治療前后以及與提供正常血清前列腺素水平的對照組的濃度變化,并使用視覺模擬疼痛評分(Visual Analogue Scale,VAS)進行疼痛程度評定。結(jié)果:(1)1個治療周期后試驗組總有效率為80%(16/20)。試驗組治療后視覺模擬疼痛評分均值較治療前降低(p0.001)。治療后血清前列腺素E2濃度中位數(shù)(四分位間距)較治療前顯著增加(p0.05),治療后血清前列腺素F2α濃度中位數(shù)(四分位間距)較治療前明顯降低(p0.05)。治療后血清前列腺素E2、血清前列腺素F2α濃度與提供正常血清前列腺素水平的對照組的中位數(shù)(四分位間距)相比無統(tǒng)計學(xué)差異(p0.05)。結(jié)論:低能量放散式體外沖擊波針灸循經(jīng)取穴治療,使陰陽歸于平衡,降低原發(fā)性痛經(jīng)患者經(jīng)期血清前列腺素F2α濃度,提高血清前列腺素E2濃度,松解痙攣腹部肌肉從而達(dá)到減輕疼痛癥狀的目的,因此此方法值得在臨床上學(xué)習(xí)并推廣。
[Abstract]:Objective: to observe the effect of low energy release extracorporeal shock wave acupuncture and moxibustion on primary dysmenorrhea (PDD) and the effect or change of serum prostaglandin E 2 (PGE 2), prostaglandin F 2 偽 (PGF 2 偽) in patients with dysmenorrhea.In order to provide clinical treatment of primary dysmenorrhea to provide a new treatment, treatment ideas.Methods: twenty patients with primary dysmenorrhea and 20 women without dysmenorrhea were recruited as control group.The experimental group was treated with shock wave acupuncture on 1-2 days before and after menstrual ovulation, including Sanyinjiao, Zusanli, Guanyuan acupoints, the joint attachment point of rectus abdominis phalangeal bone and external oblique abdominal muscle.The period of menstrual ovulation was treated as a course of treatment, followed up for 6 months after one course of treatment, and the therapeutic effect was evaluated.In the experimental group, venous blood was collected within 24-48 hours of the first menstrual cycle before and after treatment, and in the normal control group, the concentration of prostaglandin E _ 2 and prostaglandin F _ 2 偽 was measured within 24-48 hours of menstruation.The changes of serum prostaglandin levels before and after treatment were compared between the two groups, and the pain degree was evaluated by visual analog pain score (VAS).Results after one treatment cycle, the total effective rate of the trial group was 80 / 16 / 20.The mean visual analogue pain score after treatment in the trial group was lower than that before treatment.After treatment, the median serum prostaglandin E _ 2 concentration (quartile spacing) increased significantly compared with that before treatment, and the median serum prostaglandin F _ 2 偽 concentration (quartile spacing) after treatment was significantly lower than that before treatment.After treatment, there was no significant difference in serum prostaglandin E _ 2, serum prostaglandin F _ 2 偽 concentration and the median (quartile spacing) between the normal serum prostaglandin level and the control group (P 0.05).Conclusion: low energy release extracorporeal shock wave acupuncture and moxibustion along meridian points can balance yin and yang, decrease serum prostaglandin F 2 偽 concentration and increase serum prostaglandin E 2 concentration in patients with primary dysmenorrhea.This method is worth studying and popularizing in clinic because of loosening abdominal muscles and relieving pain symptoms.
