【摘要】：目的:比較養(yǎng)血定風(fēng)湯聯(lián)合自血療法與單純使用西藥在治療慢性蕁麻疹(血虛風(fēng)燥型)的臨床療效。觀察治療過(guò)程中的不良反應(yīng)和治療前后對(duì)患者生活質(zhì)量的影響,探討中西結(jié)合治療在臨床上的優(yōu)勢(shì)。方法:選取門(mén)診患者中符合納入標(biāo)準(zhǔn)的80例慢性蕁麻疹患者隨機(jī)分為治療組和對(duì)照組,每組40例。其中治療組給予養(yǎng)血定風(fēng)湯合并自血療法;對(duì)照組給予口服依巴斯汀片。記錄治療前兩組患者的各項(xiàng)癥狀積分、生活質(zhì)量評(píng)分及血清學(xué)指標(biāo)。治療4周后再次記錄觀察這些指標(biāo)及治療過(guò)程中的不良反應(yīng)情況。對(duì)比兩組患者治療前后各癥狀積分有無(wú)明顯變化、對(duì)生活質(zhì)量的影響及不良反應(yīng)等情況。比較治療后兩組患者各項(xiàng)對(duì)比有無(wú)顯著差異。觀察患者血清總IgE、IL-4、IFN--γ的水平變化。結(jié)果:治療后治療組12人臨床癥狀消失,18人癥狀明顯改善,5人癥狀得到緩解;對(duì)照組6人癥狀消失,10人癥狀明顯改善,15人緩解。治療組痊愈率高于對(duì)照組。經(jīng)過(guò)4周療程后,治療組和對(duì)照組臨床各癥狀積分較治療前明顯降低,患者癥狀明顯減輕。治療后的兩組間積分差異顯著,有統(tǒng)計(jì)學(xué)意義,說(shuō)明治療組的療效優(yōu)于對(duì)照組。根據(jù)生活質(zhì)量調(diào)查表發(fā)現(xiàn),治療后兩組生活質(zhì)量均得到改善。組間比較,治療后治療組生活質(zhì)量積分明顯低于對(duì)照組,兩組比較差異具有顯著性(P0.05)。養(yǎng)血定風(fēng)湯聯(lián)合自血療法能夠降低蕁麻疹對(duì)患者生活質(zhì)量的影響。觀察血清學(xué)指標(biāo)檢驗(yàn)發(fā)現(xiàn),治療后兩組患者血清總IgE、IL-4水平較治療前降低,IFN-γ較治療前升高。治療后組間比較,養(yǎng)血定風(fēng)湯聯(lián)合自血療法對(duì)血清指標(biāo)的影響更大,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:1.養(yǎng)血定風(fēng)湯聯(lián)合自血療法相對(duì)于單純使用西藥能更好緩解慢性蕁麻疹患者(血虛風(fēng)燥型)的痛苦,改善臨床癥狀。2.養(yǎng)血定風(fēng)湯聯(lián)合自血療法可降低慢性蕁麻疹對(duì)患者生活質(zhì)量的影響。3.養(yǎng)血定風(fēng)湯聯(lián)合自血療法可降低患者血清總IgE、IL-4水平,提高IFN-γ水平,對(duì)免疫功能起到一定干預(yù)作用。
[Abstract]:Objective: to compare the clinical efficacy of Yangxue Dingfeng decoction combined with self-blood therapy and western medicine alone in the treatment of chronic urticaria (blood deficiency and wind dryness).To observe the adverse reactions in the course of treatment and the influence on the quality of life of the patients before and after treatment, and to explore the clinical advantages of the combination of Chinese and Western treatment.Methods: 80 patients with chronic urticaria were randomly divided into treatment group (n = 40) and control group (n = 40).The treatment group was treated with Yangxue Dingfeng decoction combined with self-blood therapy, while the control group was given oral Ebastine tablets.Symptom score, quality of life score and serological index were recorded before treatment.These indexes and adverse reactions were observed 4 weeks after treatment.The symptom scores before and after treatment, the effects on quality of life and adverse reactions were compared between the two groups.There were significant differences between the two groups after treatment.To observe the level of IFN- 緯 in serum of patients with total IgE and IL-4.Results: after treatment, the clinical symptoms of 12 patients in the treatment group disappeared and the symptoms of 18 patients improved significantly. In the control group, the symptoms disappeared in 6 patients and 10 patients in the control group.The cure rate in treatment group was higher than that in control group.After 4 weeks of treatment, the clinical symptom scores of the treatment group and the control group were significantly lower than those before treatment, and the symptoms of the patients were obviously alleviated.There was significant difference between the two groups after treatment, indicating that the curative effect of the treatment group was better than that of the control group.According to the quality of life questionnaire, both groups were improved after treatment.The scores of quality of life in the treatment group were significantly lower than those in the control group after treatment, and there was a significant difference between the two groups (P 0.05).Yangxue Dingfeng decoction combined with self-blood therapy can reduce the quality of life of patients with urticaria.The serum levels of total IgEN IL-4 in the two groups were lower than those before treatment and IFN- 緯 was higher than that before treatment.After treatment, the effect of Yangxue Dingfeng decoction combined with self-blood therapy on serum indexes was greater, and the difference was statistically significant (P 0.05).Conclusion 1.Yangxue Dingfeng decoction combined with self-blood therapy can relieve the pain of patients with chronic urticaria (blood deficiency wind dryness) and improve clinical symptoms. 2.Yangxue Dingfeng decoction combined with self-blood therapy can reduce the effect of chronic urticaria on the quality of life of patients. 3.Yangxue Dingfeng decoction combined with self-blood therapy can reduce the level of serum IgE IL-4, increase the level of IFN- 緯, and play an intervention role in immune function.
