【摘要】：目的:探討實(shí)脾消水膏外敷治療原發(fā)性肝癌腹水的臨床療效及安全性。方法:將符合納入標(biāo)準(zhǔn)的63例原發(fā)性肝癌腹水患者隨機(jī)分為試驗(yàn)組(西醫(yī)常規(guī)治療+實(shí)脾消水膏外敷)31例,對(duì)照組(西醫(yī)常規(guī)治療)32例,療程為10天。試驗(yàn)結(jié)束后評(píng)價(jià)兩組患者腹水分度、腹水療效、中醫(yī)癥狀評(píng)分、卡氏評(píng)分、平均24小時(shí)尿量、腹圍、體重、量生存質(zhì)的改善情況,并評(píng)價(jià)其安全性,比較兩組治療前后有無統(tǒng)計(jì)學(xué)意義。結(jié)果:(1)兩組患者治療前后腹水深度差值、治療后腹水深度組間比較有統(tǒng)計(jì)學(xué)意義(P0.05),說明在改善腹水深度方面試驗(yàn)組優(yōu)于對(duì)照組;(2)在改善平均24小時(shí)尿量、腹圍、體重差值方面,兩組治療前后差值組間比較有顯著統(tǒng)計(jì)學(xué)意義(P0.01),試驗(yàn)組明顯優(yōu)于對(duì)照組;(3)在改善生活質(zhì)量方面:試驗(yàn)組總改善率為83.3%,對(duì)照組總改善率為63.3%,治療后兩組比較有統(tǒng)計(jì)學(xué)意義(P0.05),試驗(yàn)組優(yōu)于對(duì)照組;(4)在常見臨床癥狀方面:兩組患者比較無統(tǒng)計(jì)學(xué)意義(P0.05),其中腹脹、納呆、下肢水腫方面兩組治療后組間比較有統(tǒng)計(jì)學(xué)意義(P0.05),試驗(yàn)組優(yōu)于對(duì)照組;在脅痛、惡心嘔吐、乏力方面兩組治療后組間比較無統(tǒng)計(jì)學(xué)意義(P0.05)。在臨床觀察期間,兩組患者均未出現(xiàn)明顯不良毒副反應(yīng)。結(jié)論:實(shí)脾消水膏外敷治療原發(fā)性肝癌腹水有效、安全,能改善患者生存質(zhì)量,值得臨床推廣。
[Abstract]:Objective: to investigate the clinical efficacy and safety of shipixiaoshui ointment in the treatment of ascites of primary liver cancer. Methods: 63 patients with ascites of primary liver cancer were randomly divided into experimental group (western medicine routine treatment of solid spleen). 31 cases of external application of Xiaoshui ointment, In the control group, 32 patients were treated with routine western medicine for 10 days. The improvement of ascites grade, ascites curative effect, TCM symptom score, Karschoff score, average 24 hours urine volume, abdominal circumference, weight and quality of life were evaluated after the trial. The safety of the two groups was evaluated and the difference of ascites depth before and after treatment was compared between the two groups. There was statistically significant difference in the depth of ascites between the two groups after treatment, which indicated that the experimental group was superior to the control group in improving the depth of ascites (P 0.05)) in improving the average 24-hour urine volume, abdominal circumference and weight difference. The difference between the two groups before and after treatment was statistically significant (P 0.01), the experimental group was significantly better than the control group in improving the quality of life: the total improvement rate of the trial group was 83.3%, and the total improvement rate of the control group was 63.3%. After treatment, there were statistics between the two groups. The clinical symptoms in the experimental group were better than those in the control group. There was no significant difference between the two groups in terms of clinical symptoms, in which abdominal distension was found. After treatment, there was no significant difference between the two groups in terms of hypochondriac pain, nausea, vomiting and fatigue. During the period of clinical observation, there was no significant difference between the two groups. Conclusion: Shipixiaoshui ointment is effective and safe in the treatment of ascites of primary liver cancer and can improve the quality of life of patients and is worthy of clinical promotion.
