【摘要】：目的:本研究通過觀察臨床中采用芒針透刺的方法治療周圍性面癱的療效,旨在為醫(yī)師在臨床治療本病時(shí)提供一個(gè)不僅安全有用,又簡(jiǎn)便適用的新方式。方法:在臨床中選取的60例患者,均依照周圍性面癱的納入和診斷標(biāo)準(zhǔn)進(jìn)行診斷符合后,由隨機(jī)數(shù)字表法隨機(jī)分配,為治療組30例和對(duì)照組30例。治療組采用本臨床觀察之特色針法,即為芒針透刺,在患側(cè)選取兩組透穴:太陽(yáng)透地倉(cāng),夾承漿透頰車。針刺時(shí)應(yīng)深入肌層透刺,透刺過程中不可穿出皮膚。其余穴位隨癥加減:眼瞼閉合不全或下垂時(shí),取絲竹空;鼻唇溝消失者,口角歪斜,取迎香,顴毼,人中等穴,其他型加承漿,鳳池,風(fēng)府。針刺采取每日1次,針6休1的原則,而芒針透刺,根據(jù)患者的耐受程度,可采取每日或隔日1次,療程均為4周。對(duì)照組為不用芒針透刺為普通針刺、選穴太陽(yáng),陽(yáng)白,四白,顴毼,下關(guān),頰車,迎香,地倉(cāng),翳風(fēng),合谷。針刺采取每日1次,針6休1的原則,療程仍為4周。整理并統(tǒng)計(jì)患者前后H-B分級(jí)法、治療后評(píng)分量化轉(zhuǎn)化表、多倫多評(píng)分體系分級(jí)計(jì)分量表等分?jǐn)?shù),進(jìn)行治療效果的評(píng)價(jià)。結(jié)果:兩組患者的一般資料經(jīng)統(tǒng)計(jì)分析后無顯著性的差別(P0.05),其中包括年齡、性別、病程等,說明組間具有可比性。兩組患者在觀察評(píng)分量表的分析中分?jǐn)?shù)均有下降(p0.05),但治療組與對(duì)照組之間比較有明顯差異(P0.05),可見治療組療效優(yōu)于對(duì)照組。兩組方法治療周圍性面癱的總有效率分別為90.0%、76.7%,其差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:芒針透刺的特種針法與常規(guī)針刺療法在治療周圍性面癱時(shí)均發(fā)揮了療效,通過分析兩種方法治療周圍性面癱的功能量表評(píng)分,芒針透刺的新方法高出一籌,對(duì)面神經(jīng)功能的恢復(fù)具有明顯的促進(jìn)作用。
[Abstract]:Objective: the purpose of this study was to provide a safe and effective method for the treatment of peripheral facial palsy by the treatment of facial palsy with Awang-needle penetration in clinic. Methods: 60 patients selected in clinic were randomly assigned according to the inclusion of peripheral facial paralysis and diagnostic criteria. In the treatment group (30 cases) and the control group (30 cases), the treatment group was treated with the characteristic acupuncture method of the clinical observation, that is, the awn-needle penetration. Two groups of penetrating acupoints were selected on the affected side: the sun through the ground chamber, the plaster through the buccal car, and the acupuncture should go deep into the muscle layer. The other acupoints are added or subtracted with the symptoms: if the eyelid is not completely closed or drooping, the silk bamboo is empty; if the nasolabial groove disappears, the mouth angle is skewed, the incense is taken, the zygomaticus, the person medium point, the other type adds the paste, the Phoenix pond, Fengfu. Acupuncture takes the principle of 1 time a day, 6 times a day, and the principle of Awn acupuncture penetration, according to the patient's tolerance, can be taken daily or every other day, the course of treatment is 4 weeks. Yangbai, Sibai, zygomaticus, Xiaguan, buccal car, Yingxiang, Dikang, Yifeng, Hegu. The principle of acupuncture was taken once a day and 6 times a day. The course of treatment was still 4 weeks. Results: there was no significant difference in general data between the two groups after statistical analysis, including age, sex, course of disease, etc. The scores of the two groups were decreased in the analysis of the observation scale, but there was a significant difference between the treatment group and the control group, which showed that the curative effect of the treatment group was better than that of the control group. The total effective rates of peripheral facial palsy were 90.07 and 76.7, respectively. The difference was statistically significant (P 0.05). Conclusion: the special needling method and the routine acupuncture therapy have played an important role in the treatment of peripheral facial paralysis. By analyzing the scores of the two methods in the treatment of peripheral facial paralysis, the new method of Acupuncture is superior, and it can promote the recovery of facial nerve function.
