【摘要】：隨著人口老齡化的加劇,老齡人口對醫(yī)療服務(wù)的需求也日益增加。老年人的健康與疾病狀況直接影響到中國的經(jīng)濟和社會可持續(xù)發(fā)展。就醫(yī)行為和醫(yī)療保障與老年人的生活息息相關(guān),已經(jīng)成為整個社會必須重視的問題。就醫(yī)行為是醫(yī)療服務(wù)需求和醫(yī)療服務(wù)供給研究的重要組成部分,其重點是研究需求方的就醫(yī)取向和方式問題。本研究通過問卷調(diào)查和訪談的方法,著重研究老年人的個人因素、心理因素、社會因素以及醫(yī)療機構(gòu)因素對老年人就醫(yī)行為產(chǎn)生的不同影響。分析老年人在不同個人因素和社會因素的影響下,對醫(yī)療機構(gòu)和就醫(yī)方式的差異性選擇。通過調(diào)研數(shù)據(jù)分析老年人因不同的保險類型、學歷、收入等而做出的不同的醫(yī)療機構(gòu)、就診方式的選擇。分析醫(yī)生和醫(yī)院提供的服務(wù)對老年人就醫(yī)體驗的影響。探究老年人醫(yī)療衛(wèi)生服務(wù)的需求與利用情況。找出影響老年人就醫(yī)行為的主要因素,對于就醫(yī)行為中存在的問題進行討論,為老年人的醫(yī)療和就醫(yī)行為提供方便。方法與對象:以哈爾濱市道里區(qū)60歲及以上老年人為研究對象,采用隨機發(fā)放問卷的方式在哈爾濱市道里區(qū)穆斯林小區(qū)、迎賓小區(qū)、新城花園小區(qū)等8個小區(qū)發(fā)放問卷500份,剔除無效問卷后使用統(tǒng)計軟件SPSS 20進行數(shù)據(jù)錄入與分析。影響因素分析運用Pearson卡方檢驗和相關(guān)系數(shù)分析的方法,通過卡方值和相關(guān)系數(shù)值來分析哈爾濱市道里區(qū)老年人就醫(yī)行為的影響因素。結(jié)果:1、哈爾濱市道里區(qū)老年人的婚姻狀況、學歷、收入狀況及醫(yī)保類型等因素影響其對就診方式、就醫(yī)地點的選擇。2、哈爾濱市道里區(qū)老年人的婚姻狀況、學歷、收入狀況及醫(yī)保類型等因素在"自感病重"時對就診方式的選擇均具有差異性。不同文化程度和不同收入狀況的老年人在"自感病輕"和"自感病重"時對就診方式選擇的都具有顯著差異。性別與老年人"自感病輕"和"自感病重"時對就診方式的選擇無顯著性差異。3、哈爾濱市道里區(qū)老年人的婚姻狀況、學歷、收入及醫(yī)保類型等個人因素與其選擇的就醫(yī)地點存在相關(guān)性,其相關(guān)性因素由強到弱依次是婚姻狀況、收入、醫(yī)保類型和學歷。而老年人的性別與就醫(yī)地點的選擇并無明顯的相關(guān)性。結(jié)論:老年人的就醫(yī)及時性和合理性存在問題,就醫(yī)體驗不夠理想;疾病預(yù)防意識較為薄弱;對于保健品的作用存在一定的理解誤區(qū);個人醫(yī)療費用支出較高。此外,三級醫(yī)院的醫(yī)療衛(wèi)生資源配置明顯不足,醫(yī)療機構(gòu)的服務(wù)質(zhì)量和服務(wù)水平也有待提高。建議應(yīng)引導(dǎo)老年人及時、合理就醫(yī)并降低老年人個人醫(yī)療費用;改善和優(yōu)化醫(yī)藥保健品的市場環(huán)境,提高醫(yī)療機構(gòu)的服務(wù)質(zhì)量;合理配置醫(yī)療衛(wèi)生資源。
[Abstract]:As the population ages, The health and disease status of the elderly has a direct impact on the sustainable economic and social development of China. The behavior of seeking medical care and medical security are closely related to the lives of the elderly. The behavior of seeking medical care is an important part of the research on the demand for medical services and the supply of medical services. The focus of this study is to study the orientation and methods of seeking medical treatment on the demand side. By means of questionnaires and interviews, this study focuses on the individual and psychological factors of the elderly. Social factors and medical institution factors have different effects on the elderly's medical behavior. The analysis of the elderly under the influence of different personal factors and social factors, The different choice of medical institutions and ways of visiting medical care. Through the research data, we analyze the different medical institutions made by the elderly because of different types of insurance, educational background, income, etc., To analyze the influence of the service provided by doctors and hospitals on the medical experience of the elderly, to explore the demand and utilization of medical and health services for the elderly, and to find out the main factors that affect the behavior of the elderly in seeking medical treatment. The problems existing in the behavior of seeking medical treatment are discussed in order to provide convenience for the medical treatment and medical behavior of the elderly. Methods and objects: taking the elderly aged 60 and above in Daoli District of Harbin City as the research object, Using random questionnaires, 500 questionnaires were distributed in 8 districts, such as the Muslim District, the Yingbin District, the New City Garden District, etc., in Daoli District, Harbin City. After eliminating the invalid questionnaire, the data input and analysis were carried out with the statistical software SPSS 20. The influencing factors were analyzed by Pearson chi-square test and correlation coefficient analysis. Through chi-square value and correlation coefficient value, this paper analyzes the influencing factors of the elderly's medical behavior in Daoli District, Harbin City. Results the marriage status, educational background, income status and the type of medical insurance of the elderly in Daoli District, Harbin City, affect the way of visiting a doctor, such as marriage status, educational background, income status and the type of medical insurance, and so on. The choice of the place of medical treatment. 2. The marital status and educational background of the elderly in Daoli District, Harbin City. The factors such as income status and medical insurance type have differences in the choice of the way of seeing a doctor when the patients are "seriously ill". The elderly with different education and different income status visit the patients when they are "mild" and "seriously ill". There was no significant difference between gender and the elderly when they were "self-infected" or "seriously ill". There was no significant difference between them. 3. The marriage status of the elderly in Daoli District, Harbin City, was not significantly different. Personal factors, such as education, income and type of medical insurance, were correlated with the places they chose to seek medical treatment, and the related factors were marital status and income in the order of strong to weak. There is no obvious correlation between the sex of the elderly and the choice of the place of medical treatment. Conclusion: there are some problems in the timeliness and rationality of the medical visit, the experience is not good enough, the consciousness of disease prevention is relatively weak. There are some misunderstandings about the role of health care products; personal medical expenses are relatively high; in addition, the allocation of medical and health resources in tertiary hospitals is obviously inadequate. The service quality and service level of medical institutions should also be improved. It is suggested that the elderly should be guided to seek medical treatment in a timely manner and the personal medical expenses should be reduced, the market environment of medical and health products should be improved and optimized, and the service quality of medical institutions should be improved. Reasonable allocation of medical and health resources.
