【摘要】：目的:本研究旨在觀察三參四黃降糖膠囊對早期糖尿病腎病(氣陰兩虛夾瘀證)的臨床療效及對血清腫瘤壞死因子(TNF-α)的干預。方法:將選取符合納入標準的患者60例,隨機分為兩組(對照組、治療組),每組30例。兩組均進行常規(guī)基礎治療及糖尿病宣教,對照組加用厄貝沙坦治療,治療組加用三參四黃降糖膠囊治療。總療程為12周,療程結束后檢測兩組患者的尿微量白蛋白排泄率(UAER)、血清TNF-α等生化指標,分析治療前后臨床療效及中醫(yī)證候積分變化,并結合安全性指標,對兩組進行綜合比較。結果:1.治療12周后觀察,兩組總有效率比較:治療組為82.1%,對照組總有效率25.0%,差異顯著,有統(tǒng)計學意義(p0.05),說明三參四黃降糖膠囊治療早期糖尿病腎病作用突出,區(qū)別于傳統(tǒng)治療具有明顯優(yōu)勢。2.治療12周后觀察,在中醫(yī)證候總積分及癥狀積分方面:兩組間比較,治療組較對照組下降明顯(p0.05);治療組組內比較,較治療前顯著下降(p0.05)在中醫(yī)有效率方面:治療組92.0%,對照組17.9%,兩組比較差異顯著(p0.05),有統(tǒng)計學意義。表明,三參四黃降糖膠囊在改善中醫(yī)證候、癥狀方面具有獨特療效。3.治療12周后觀察,在降低UAER方面:治療組較治療前明顯降低(p0.05);治療組較對照組明顯下降(p0.05),差異顯著,具有統(tǒng)計學意義。證實,三參四黃降糖膠囊可顯著降低早期糖尿病腎病UAER水平。4.治療12周后觀察,在降低血清TNF-α方面:治療組較治療前明顯降低(p0.05);對照組較治療前輕微升高;治療組較對照組明顯下降(p0.05)差異顯著,具有統(tǒng)計學意義。說明三參四黃降糖膠囊可顯著降低早期糖尿病腎病患者血清TNF-α水平。結論:1.三參四黃降糖膠囊可降低早期糖尿病腎病患者UAER水平,說明可降低腎小球損傷及蛋白尿的產生。2.三參四黃降糖膠囊可明顯改善早期糖尿病腎病患者中醫(yī)臨床癥狀及體征,降低證候積分。3.三參四黃降糖膠囊可通過降低患者血清TNF-α的水平,延緩早期糖尿病腎病病情進展。4.三參四黃降糖膠囊安全性高,藥物及使用劑量可靠。
[Abstract]:Objective: This study aimed to investigate the three ginseng four yellow Jiangtang Capsule on early diabetic nephropathy (Qi Yin deficiency with blood stasis syndrome) clinical efficacy and factors on serum tumor necrosis (TNF- alpha) intervention. Methods: 60 patients were enrolled according to inclusion criteria, were randomly divided into two groups (control group, treatment group), 30 cases in each group. Two groups were received routine treatment and diabetes education, the control group treated with irbesartan, the treatment group was treated with three parameters of four yellow Jiangtang Capsule treatment. The total treatment duration was 12 weeks after the end of treatment, detection of urinary albumin excretion rate in two groups (UAER), serum TNF- biochemical indicator. Analysis of the changes of the integral clinical curative effect and TCM syndrome before and after treatment, and combined with the safety indexes of the two groups were compared. Results: the observation of 1. after 12 weeks of treatment, the total efficiency of two groups: the treatment group was 82.1%, control group total effective rate was 25%, there was significant difference. (P0.05), meaning that three and four yellow Jiangtang Capsule in treating early diabetic nephropathy have outstanding effect, different from the traditional treatment has the obvious advantages of.2. after 12 weeks of treatment, the TCM syndrome total score and symptom score: comparison between the two groups, the treatment group significantly decreased compared with the control group (P0.05); treatment group in comparison, significantly lower than before treatment (P0.05) efficiency in traditional Chinese medicine: the treatment group 92%, control group 17.9%, there was significant difference between two groups (P0.05), there was statistical significance. Three parameters showed that four yellow Jiangtang Capsule in improving TCM syndrome, the symptoms have unique curative effect observation of.3. after 12 weeks of treatment and the reduction of UAER in treatment group were significantly lower than those before treatment (P0.05); the treatment group decreased significantly compared with the control group (P0.05), significant difference was statistically significant. Three confirmed, four yellow ginseng Jiangtang Capsule can significantly reduce the level of UAER.4. in early diabetic nephropathy after 12 weeks of treatment Observation on serum alpha TNF- treatment group was significantly lower than before treatment (P0.05); the control group is slightly higher than before treatment; the treatment group decreased significantly compared with the control group (P0.05) had significant difference, with statistical significance. Three refs four yellow Jiangtang Capsule could significantly decrease the serum TNF- in patients with early diabetic nephropathy alpha level. Conclusion: 1. three ginseng four yellow Jiangtang Capsule can reduce the level of UAER in patients with early diabetic nephropathy, that can reduce glomerular injury and proteinuria in.2. three ginseng four yellow Jiangtang Capsule can obviously improve the traditional Chinese medicine for early diabetic nephropathy patients clinical symptoms and signs, reduce the symptom score of.3. three. Four yellow Jiangtang Capsule can reduce patients serum TNF- level, delay the disease progression of diabetic nephropathy early.4. three ginseng four yellow Jiangtang Capsule with high safety, drugs and dosage.
