【摘要】：目的通過隨機對照試驗,觀察揉捏牽轉法結合斜方肌按揉對小兒先天性肌性斜頸的療效,以期為本病提供更有效的治療方案。方法將符合納入標準的100例患兒隨機分為治療組和對照組。治療組采用揉捏牽轉法結合斜方肌按揉治療,對照組采用揉捏牽轉法治療,每周治療5次,5次為1個療程,共5個療程。治療后觀察患兒頭部傾斜、旋轉及頸椎活動功能。結果兩組病例在性別、分型、病位、月齡分布上無顯著差異,治療前耳肩距、健側頜肩距、患側頜肩距方面比較均無顯著差異,治療組和對照組的患兒頭部傾斜、旋轉及頸椎活動功能在治療后均有改善,與同組治療前相比,差異顯著,有統(tǒng)計學意義(P0.05)。治療后治療組的患兒頭部傾斜、旋轉及頸椎活動功能與對照組相比,治療組優(yōu)于對照組,差異有統(tǒng)計學意義(P0.05)。在臨床療效方面,治療組總有效率為97.87%,對照組總有效率為93.33%,治療組總有效率優(yōu)于對照組,但差異無統(tǒng)計學意義(P0.05);治療組愈顯率為65.96%,對照組愈顯率為42.22%,差異有統(tǒng)計學意義(P0.05)。結論揉捏牽轉法結合斜方肌按揉與揉捏牽轉法均能改善肌性斜頸癥狀,但揉捏牽轉法結合斜方肌按揉優(yōu)于揉捏牽轉法。揉捏牽轉法結合斜方肌按揉安全有效,值得臨床推廣。
[Abstract]:Objective to observe the effect of kneading and retraction combined with trapezius on congenital muscular torticollis in children through a randomized controlled trial. In order to provide a more effective treatment for the disease. Methods 100 children who met the inclusion criteria were randomly divided into the treatment group and the control group. The treatment group was treated by kneading method combined with trapezius. The patients in the control group were treated by kneading and retraction, 5 times a week for a course of five courses. After the treatment, the head tilt, rotation and cervical vertebrae function were observed. Results the patients in the two groups were classified according to sex. There was no significant difference in the distribution of disease position and age. There was no significant difference in the anterior ear shoulder distance, the healthy side shoulder distance and the affected side shoulder distance. The head tilt of the children in the treatment group and the control group was not significant. Rotation and cervical movement were improved after treatment, compared with the same group before treatment, the difference was significant (P 0.05). After treatment, the head of the children in the treatment group was tilted. Compared with the control group, the treatment group was better than the control group, the difference was statistically significant (P 0.05). In clinical effect, the total effective rate of the treatment group was 97.87%. The total effective rate of the control group was 93.33, the total effective rate of the treatment group was better than that of the control group, but the difference was not statistically significant (P 0.05). The effective rate was 65.96 in the treatment group and 42.22% in the control group. The difference was statistically significant (P 0.05). Conclusion the kneading method combined with trapezius method can improve the symptoms of muscular torticollis. But kneading traction combined with trapezius is better than kneading. Kneading combined with trapezius is safe and effective, which is worth popularizing in clinic.
