【摘要】:目的:探討Ensite Navx三維標測系統(tǒng)指導下射頻消融治療左側旁道的有效性及安全性。方法:選取2014年6月至2016年12月期間經房間隔穿刺行射頻消融治療的左側旁道患者91例,將其分為觀察組和對照組,觀察組45例患者在Ensite Navx三維標測系統(tǒng)引導下,必要時結合短暫X透視進行射頻消融治療;對照組46例患者在傳統(tǒng)X線透視下行射頻消融治療。觀察指標:(1)放置標測導管過程中X線照射時間和曝光劑量;(2)操作消融導管進行標測消融過程中X線照射時間和曝光劑量;(3)手術時間;(4)即刻成功率;(5)并發(fā)癥。結果:(1)放置標測導管過程中兩組間X線照射時間及照射劑量:觀察組X線照射時間(2.4±1.4)min VS對照組(2.5±1.2)min,兩組間無顯著差異,(P0.05);觀察組X線曝光劑量(1.6±1.1)mGy與對照組(1.5±1.0)mGy相比,兩組間也無顯著差異,(P0.05)。(2)消融導管操作過程中兩組間X線照射時間及照射劑量:在操作消融導管過程中,觀察組的X線曝光時間(5.1±2.3)min VS對照組(13.1±3.1)min,觀察組的X線曝光時間明顯低于對照組,觀察組X線曝光時間較對照組縮短61%(P0.05);同時觀察組X線曝光劑量(5.7±2.6)mGy較對照組X線曝光劑量(17.8±4.3)mGy也顯著減少,觀察組X線曝光劑量較對照組下降68%,(P0.05)。(3)手術時間:觀察組(59.1±18.8)min VS對照組(62.3±20.6)min,兩組間屋顯著差異(P0.05)。(4)即刻成功率:觀察組全部消融成功,對照組1例左后游離壁旁道未消融成功,后借助Ensite Navx三維系統(tǒng)消融成功,術后隨訪3個月觀察組無復發(fā)病例。(5)并發(fā)癥:其中觀察組出現單純穿刺口血腫1例,對照組出現氣胸1例,兩組訴心前區(qū)或后背部不適患者4例,分別予相應處理。結論:Ensite Navx三維系統(tǒng)引導下行左側旁道射頻消融,可明顯縮短操作消融導管過程中X線照射時間,減少X線曝光劑量,進而減少手術全程的X線透視時間和曝光劑量;手術成功率與傳統(tǒng)X線透視下射頻消融無差異;并不增加手術相關并發(fā)癥的發(fā)生風險。
[Abstract]:Objective: To investigate the Ensite Navx RF carto guided ablation is effective and safety in the treatment of left accessory pathways. Methods: during the period from June 2014 to December 2016 with atrial septal puncture underwent radiofrequency ablation treatment in patients with 91 left accessory pathways were divided into observation group and control group, 45 patients in observation group were Ensite Navx three mapping system under the guidance, when necessary and short-term perspective X radiofrequency ablation therapy; 46 patients in the control group had conventional X-ray fluoroscopy radiofrequency ablation treatment. Observation index: (1) X-ray irradiation time and exposure dose mapping catheter placed in the process; (2) the operation of the ablation catheter of X-ray radiation exposure and mapping the ablation process dose; (3) the operation time; (4) immediate success rate; (5) complications.
