【摘要】：研究目的:本研究旨在通過對國家、北京市醫(yī)藥衛(wèi)生體制改革等相關政策文件中的主要目標集中點進行提煉與歸類,以世界衛(wèi)生組織最新制定的衛(wèi)生費用核算體系(SHA2011)中核心維度的二維矩陣交叉表為基礎,進一步探討形成分析北京市醫(yī)療機構中可反映各主要政策目標集中點實施情況的框架,并初步實現(xiàn)對2014-2015年北京市醫(yī)療機構經常性衛(wèi)生費用的結果分析。研究方法:本研究重點搜索2009-2016年國內及北京市醫(yī)藥衛(wèi)生體制改革相關政策、方案、實施意見等文件,通過專家咨詢討論確定政策主要目標集中點并形成基于政策目標的各角度分析框架;通過分析2014-2015年北京市醫(yī)療機構經常性衛(wèi)生費用的數(shù)據(jù)進行分析框架的實際應用。研究內容:本研究主要通過歸納總結醫(yī)藥衛(wèi)生體制改革政策主要目標集中點即推進建設醫(yī)療保障體系、推進公立醫(yī)院改革試點、建設基本公共衛(wèi)生服務體系、完善政府衛(wèi)生投入機制和建設完善醫(yī)療服務體系,結合SHA2011的核算維度提出從社會醫(yī)療保險籌資、政府籌資、醫(yī)療機構資源配置和預防服務費用四個角度形成分析框架,并分析2014-2015年北京市醫(yī)療機構經常性衛(wèi)生費用的結果。結論:為了科學制定相關衛(wèi)生政策,迫切需要新的核算手段來適應如今衛(wèi)生系統(tǒng)的諸多變化。利用SHA2011核算體系,可以彌補以前所應用的來源法、機構法和功能法等單維度核算衛(wèi)生總費用時部分所不能進行解釋的內容缺失;總結歸納2009年新醫(yī)改以來的主要政策目標集中點,結合SHA2011的二維矩陣平衡交叉表以形成分析框架,可用于衛(wèi)生政策的分析并且能夠實現(xiàn)深入分析政策所要求的結果;2014-2015年北京市醫(yī)療機構經常性衛(wèi)生費用結果反映出建立完善政府衛(wèi)生投入機制、推進建設醫(yī)療保障體系和建設完善醫(yī)療服務體系政策實施均反映取得一定效果,建設基本公共衛(wèi)生服務體系政策實施情況反映較好。
[Abstract]:Objective: This study aims to state, the main target of Beijing city health system reform and other related policy documents focus on the extraction and classification, the WHO to develop the latest health cost accounting system (SHA2011) two-dimensional matrix in the core dimensions of cross table as the basis, to further explore the formation of analysis can reflect the framework the main policy focus point of the implementation of the medical institutions in Beijing City, and the initial realization of the medical institutions in Beijing city in 2014-2015 current health expenditure analysis. Methods: This study focus search 2009-2016 China and Beijing City medical and health system reform policies, programs, opinions and other documents, through expert consultation and discussion to determine the main target point and the formation of policy analysis framework based on the angle of policy objectives; through the analysis of medical institutions in Beijing city in 2014-2015 Current health expenditure data analysis application framework. The research content of this research is to summarize the policies of health care system reform is the main focus of promoting the construction of medical security system, promote the reform of public hospitals, the construction of basic public health service system, improve the government health investment mechanism and improve the construction of medical service system. With accounting dimensions of SHA2011 made from the social medical insurance financing, government financing, analyses form four aspects of medical resources allocation and prevention services, and analysis of medical institutions in Beijing city in 2014-2015 current health expenditure results. Conclusion: in order to scientifically formulate relevant health policy, there is an urgent need for new accounting means to adapt the present health the many changes. The use of SHA2011 accounting system, can be used to make up the source of law, law and power mechanism Methods the single dimension calculation of total health expenses when part cannot explain the contents of the deletion; summarize the main policy goal since 2009 summed up new medical reform focus, combined with the SHA2011 matrix to form a two-dimensional balance cross table analysis framework can be used for the analysis of health policy and to achieve in-depth analysis of policy required medical results; institutions in Beijing city in 2014-2015 current health expenditure results reflect the establishment of government health investment mechanism to improve and promote the construction of medical security system construction and improve the medical service system of policy implementation are made to reflect the effect of the construction of basic public health service system of policy implementation to reflect better.
