【摘要】：目的:修訂臨床記憶量表中的詞語(yǔ)配對(duì)聯(lián)想學(xué)習(xí)測(cè)驗(yàn)(VPALT)的老年常模并檢驗(yàn)其效度和信度。方法:在北京取60~85歲正常老人825名(用于建立常模和信度檢驗(yàn))和輕度認(rèn)知損害(MCI)患者91名(用于效度檢驗(yàn)),完成VPALT、瑞氏聽(tīng)覺(jué)詞語(yǔ)學(xué)習(xí)測(cè)驗(yàn)(RAVLT)、蒙特利爾認(rèn)知評(píng)估量表(MoCA)等神經(jīng)心理測(cè)驗(yàn),并選取其中25名正常老人于2~5周后進(jìn)行VPALT重測(cè)。把被試按年齡和受教育程度分組以建立常模。效度檢驗(yàn)采用實(shí)證效度和效標(biāo)效度,效標(biāo)工具包括RAVLT和MoCA,并進(jìn)行ROC曲線(xiàn)分析。信度檢驗(yàn)使用內(nèi)部一致性系數(shù)和重測(cè)信度。結(jié)果:不同年齡組和受教育程度組VPALT得分存在差異(P0.001),年齡越低或受教育程度越高VPALT得分越高。效度檢驗(yàn)發(fā)現(xiàn),VPALT得分與RAVLT的即時(shí)回憶總分、MoCA的延遲回憶得分、MoCA總分均呈正相關(guān)(r=0.56、0.46、0.49,均P0.001);正常老年人群的VPALT得分高于MCI患者[(6.9±4.2)vs.(4.2±3.2),P0.001];根據(jù)ROC曲線(xiàn)結(jié)果,VPALT的敏感度為81.4%,特異度為64.4%。信度檢驗(yàn)發(fā)現(xiàn),VPALT的Cronbachα系數(shù)為0.86,其中容易詞對(duì)和困難詞對(duì)兩個(gè)分測(cè)驗(yàn)的α系數(shù)分別為0.78和0.83;VPALT的重測(cè)信度為0.80,其中容易詞對(duì)和困難詞對(duì)兩個(gè)分測(cè)驗(yàn)的重測(cè)信度分別為0.77和0.71。結(jié)論:本研究修訂了詞語(yǔ)配對(duì)聯(lián)想學(xué)習(xí)測(cè)驗(yàn)60~85歲健康老人的年齡和受教育程度常模,其效度、信度檢驗(yàn)符合心理測(cè)量學(xué)要求,可作為老年人記憶能力檢測(cè)和MCI臨床診斷的定量參照指標(biāo)。
[Abstract]:Objective: to revise the geriatric norm of word pair associative learning test (VPALTT) in clinical memory scale and to test its validity and reliability. Methods: 825 normal people aged 60 to 85 years old (used to establish norm and reliability test) and mild degree were selected in Beijing. 91 patients with cognitive impairment (MCI) received neuropsychological tests such as validity test, VPALTT, RAVLTT, Montreal Cognitive Assessment scale (MOCA), etc. 25 of them were selected to retest VPALT after 2 weeks. The subjects were divided into two groups according to their age and education level to establish the norm. The validity test was conducted by empirical validity and standard validity. The reliability test uses internal consistency coefficient and retest reliability. Results: there are differences in VPALT scores of different age groups and educated groups (P 0.001), the lower the age or the educational level, the lower the age or the educational level. The higher the score of VPALT was, the higher the score of VPALT was. The validity test showed that the score of VPALT was positively correlated with the total score of delayed recall of RAVLT and the total score of RAVLT was 0.560.46 鹵0.49, all of which was P 0.0010.The VPALT score of normal elderly people was higher than that of MCI patients [6.9 鹵4.2)vs.(4.2 鹵3.2P0.001], and according to the result of ROC curve, VPALT was significantly higher than that of MCI patients. The sensitivity was 81.4 and the specificity was 64.4. The reliability test found that the Cronbach 偽 coefficient of VPALT was 0.86, and the 偽 coefficient of easy word pair and difficult word pair subtest was 0.78 and 0.83VPALT, respectively, and the re-test reliability of easy word pair and difficult word pair was 0.80. The test-retest reliability of the test was 0.77 and 0.71 respectively. Conclusion: this study revised the norm of age and education level of the healthy people aged 6085 years old in the word pair associative learning test. The test of validity and reliability meets the requirement of psychometrics and can be used as a quantitative reference index for the detection of memory ability and the clinical diagnosis of MCI in the elderly.
