【摘要】：農(nóng)村衛(wèi)生服務(wù)體系由政府、集體、社會(huì)和個(gè)人舉辦的縣、鄉(xiāng)、村三級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)組成,以縣級(jí)醫(yī)療衛(wèi)生機(jī)構(gòu)為龍頭,鄉(xiāng)(鎮(zhèn))衛(wèi)生院為中心,村衛(wèi)生室為基礎(chǔ)。主要包括縣醫(yī)院、縣中醫(yī)(民族醫(yī))醫(yī)院、縣疾病預(yù)防控制機(jī)構(gòu)、縣衛(wèi)生執(zhí)法監(jiān)督機(jī)構(gòu)、縣婦幼保健機(jī)構(gòu)、鄉(xiāng)(鎮(zhèn))衛(wèi)生院、村衛(wèi)生室及其他衛(wèi)生機(jī)構(gòu)等,是我國農(nóng)村居民獲得基本醫(yī)療服務(wù)和公共衛(wèi)生服務(wù)的重要支柱。國內(nèi)研究學(xué)者主要是針對(duì)農(nóng)村衛(wèi)生服務(wù)的政府投入方面、人力資源管理方面、績效評(píng)價(jià)體系方面、組織建設(shè)方面進(jìn)行研究,對(duì)農(nóng)村衛(wèi)生服務(wù)體系建設(shè)方面的詳細(xì)研究相對(duì)較少。因此研究農(nóng)村衛(wèi)生服務(wù)體系建設(shè)對(duì)于目前農(nóng)村醫(yī)療衛(wèi)生服務(wù)體系建設(shè)有著重要意義。本文主要通過對(duì)醫(yī)療衛(wèi)生服務(wù)體系相關(guān)理論的研究,闡述了農(nóng)村衛(wèi)生服務(wù)體系的框架構(gòu)成,在運(yùn)用新公共管理理論和社會(huì)保障理論的基礎(chǔ)上,結(jié)合中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系建設(shè)的實(shí)際情況,采用文獻(xiàn)法及訪談法對(duì)中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系建設(shè)進(jìn)行深入研究。首先,文章介紹了中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系建設(shè)的基本現(xiàn)狀,指出當(dāng)前中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系建設(shè)方面取得的成效,比如在基礎(chǔ)設(shè)施建設(shè)、公立醫(yī)院改革、衛(wèi)生信息化等方面有了長足的發(fā)展。其次,從機(jī)構(gòu)設(shè)置、人才隊(duì)伍建設(shè)、服務(wù)能力、專科建設(shè)等方面介紹了中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系的基本情況;同時(shí)指出了中衛(wèi)市農(nóng)村衛(wèi)生服務(wù)體系目前存在的問題,諸如對(duì)農(nóng)村衛(wèi)生服務(wù)體系重要性認(rèn)識(shí)不足、基層服務(wù)能力有待加強(qiáng)、醫(yī)院債務(wù)負(fù)擔(dān)較重、醫(yī)療市場競爭日益加劇等問題,針對(duì)這些問題產(chǎn)生的原因進(jìn)行了深入分析,存在著對(duì)農(nóng)村衛(wèi)生服務(wù)體系建設(shè)的重要性認(rèn)識(shí)不足、責(zé)權(quán)不明晰、投入不足、機(jī)制不健全以及人才管理政策等幾個(gè)方面。在對(duì)國內(nèi)外先進(jìn)地區(qū)農(nóng)村衛(wèi)生服務(wù)體系建設(shè)的經(jīng)驗(yàn)分析的基礎(chǔ)上從以下幾個(gè)方面提出了相應(yīng)的措施:編制好“十三五”發(fā)展規(guī)劃、深化醫(yī)藥衛(wèi)生體制改革、大力推進(jìn)衛(wèi)生信息化建設(shè)、加大人才隊(duì)伍建設(shè)、加強(qiáng)基層醫(yī)療服務(wù)體系建設(shè)、全面提升診療服務(wù)水平等。
[Abstract]:The rural health service system is composed of county, township and village medical and health institutions organized by the government, collective, society and individuals, with county-level medical and health institutions as the lead, township (town) health centers as the center and village clinics as the basis. Mainly including county hospitals, county traditional Chinese medicine (ethnic medicine) hospitals, county disease prevention and control agencies, county health enforcement supervision agencies, county maternal and child health care institutions, township (town) hospitals, village clinics and other health institutions, etc. It is an important pillar of rural residents' access to basic medical services and public health services. Domestic scholars mainly focus on the government input of rural health services, human resources management, performance evaluation system, organizational construction research, rural health service system construction of detailed research is relatively small. Therefore, it is of great significance to study the construction of rural health service system. This article mainly through the research to the medical and health service system related theory, elaborated the rural health service system frame constitution, in the use of the new public management theory and the social security theory foundation, Combined with the actual situation of the construction of rural health service system in Zhongwei city, the paper makes a deep study on the construction of rural health service system in Zhongwei city by using the method of literature and interview. First of all, the article introduces the basic status quo of the construction of rural health service system in Zhongwei City, and points out the achievements in the construction of rural health service system in Zhongwei City, such as the construction of infrastructure, the reform of public hospitals, Health informatization and other aspects have made great progress. Secondly, this paper introduces the basic situation of the rural health service system in Zhongwei City from the aspects of institution setting, talent team building, service ability and specialty construction, and points out the existing problems of the rural health service system in Zhongwei City. Such problems as insufficient understanding of the importance of rural health service system, need to be strengthened in basic level service capacity, heavy hospital debt burden, increasing competition in the medical market, and so on, are analyzed in depth in view of the causes of these problems. There are some aspects such as insufficient understanding of the importance of the construction of rural health service system, unclear responsibility and power, insufficient investment, imperfect mechanism and talent management policy. Based on the analysis of the experience in the construction of rural health service system in advanced areas at home and abroad, the corresponding measures are put forward in the following aspects: making a good development plan for the 13th Five-Year Plan and deepening the reform of the medical and health system. Vigorously promote the construction of health information, strengthen the construction of talent team, strengthen the construction of grass-roots medical service system, and improve the level of diagnosis and treatment services in an all-round way.
