【摘要】：日本長期照護(hù)保險(xiǎn)制度是日本特定社會(huì)、制度、經(jīng)濟(jì)、政治等因素影響的產(chǎn)物。人口老齡化、長期照護(hù)需求攀升、家庭長期照護(hù)功能弱化、老人福利制度與老人保健制度問題凸顯、社會(huì)保障財(cái)政窘迫與經(jīng)濟(jì)長期低迷、女權(quán)主義運(yùn)動(dòng)以及政黨博弈共同影響了日本長期照護(hù)保險(xiǎn)制度的建立。文章利用Gilbert社會(huì)福利政策分析架構(gòu)對日本長期照護(hù)保險(xiǎn)制度設(shè)計(jì)進(jìn)行了分析,發(fā)現(xiàn)日本長期照護(hù)保險(xiǎn)制度設(shè)計(jì)無論是分配基礎(chǔ)、分配內(nèi)容、服務(wù)輸送還是財(cái)務(wù)模式都力求精良,并通過多次改革不斷完善,其制度設(shè)計(jì)也顯現(xiàn)出“在地老化”、“地區(qū)主義”、“福利多元主義”等理念在日本的發(fā)展和應(yīng)用。制度實(shí)施十五年來,不僅在較大程度上解決了社會(huì)性長期照護(hù)難題、完善了老年社會(huì)保障制度、實(shí)現(xiàn)全社會(huì)共同支撐老齡社會(huì),而且促進(jìn)了長期照護(hù)產(chǎn)業(yè)的發(fā)展、創(chuàng)造了新的就業(yè)崗位。但是日本長期保險(xiǎn)制度也面臨著財(cái)政缺乏可持續(xù)性、統(tǒng)籌層次過低導(dǎo)致地區(qū)負(fù)擔(dān)不公平、服務(wù)供給失衡、家庭成員照顧負(fù)擔(dān)仍然沉重、照護(hù)人力不足等問題。中國人口老齡化進(jìn)程加快,老年長期照護(hù)需求日漸攀升,發(fā)展長期照護(hù)保險(xiǎn)制度是中國應(yīng)對長期照護(hù)社會(huì)風(fēng)險(xiǎn)的重要選擇。借鑒日本經(jīng)驗(yàn),我國長期照護(hù)保險(xiǎn)制度設(shè)計(jì)要力求簡潔、努力實(shí)現(xiàn)應(yīng)保盡保,要明確保險(xiǎn)給付內(nèi)容與給付辦法,同時(shí)服務(wù)輸送要注重可及性,籌資機(jī)制要注重可持續(xù)性。同時(shí)需要完善相關(guān)政策法規(guī)體系、完善長期照護(hù)服務(wù)體系、建立健全需求評估機(jī)制、建構(gòu)跨專業(yè)整合型人才隊(duì)伍、并通過宣傳教育增強(qiáng)民眾的長期照護(hù)保險(xiǎn)意識,保障長期照護(hù)保險(xiǎn)制度的健康運(yùn)行。
[Abstract]:Japan's long-term care insurance system is the product of specific social, institutional, economic, political and other factors in Japan. The problems of the welfare system for the elderly and the health care system for the elderly are highlighted, and the social security system is in financial straits and the economy is in a long downturn. The feminist movement and the political party game influence the establishment of the long-term care insurance system in Japan. This paper analyzes the design of the long-term care insurance system in Japan by using the Gilbert social welfare policy analysis framework. It is found that the design of Japan's long-term care insurance system, whether it is on the basis of distribution, the content of distribution, the delivery of services or the financial model, is striving to be excellent, and it has been continuously improved through many reforms. Its system design also shows the development and application of the concepts of "local aging", "regionalism" and "welfare pluralism" in Japan. In the past 15 years, the system has not only solved the social long-term care problem to a large extent. It has improved the social security system for the elderly, realized that the whole society supports the aging society, promoted the development of the long-term care industry, and created new jobs. However, Japan's long-term insurance system also faces a lack of financial sustainability. The low level of co-ordination leads to unfair regional burden, unbalanced service supply, heavy care burden for family members and insufficient care manpower. The aging process of China's population is accelerating, and the demand for long-term care for the elderly is rising day by day. The development of long-term care insurance system is an important choice for China to deal with the social risks of long-term care. It is necessary to clarify the contents and methods of insurance payment, at the same time, to pay attention to accessibility and financing mechanism for service delivery. At the same time, it is necessary to perfect the relevant policies and regulations, perfect the long-term care service system, and establish and improve the needs assessment mechanism. To build a cross-professional integrated talent team and strengthen the awareness of long-term care insurance through propaganda and education to ensure the healthy operation of long-term care insurance system.
