【摘要】:1998年城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)制度建立以來(lái),歷年城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金收入和支出的非均衡增長(zhǎng)(基金收入明顯快于基金支出)導(dǎo)致基金當(dāng)年結(jié)余和累計(jì)結(jié)余規(guī)模不斷擴(kuò)大。截至2013年底,我國(guó)城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金當(dāng)年結(jié)余達(dá)到1447億元,累計(jì)結(jié)余也高達(dá)8130億元。換句話說(shuō),即使在沒(méi)有發(fā)生當(dāng)年基金收入以及來(lái)年的基金支出保持不變的的前提下,單靠累計(jì)結(jié)余的基金就可以保障14個(gè)月的基金支出。巨額的城鎮(zhèn)職工醫(yī);鸾Y(jié)余,成為了中國(guó)社會(huì)保障領(lǐng)域的熱議話題之一。如果基金能夠保持適度的結(jié)余水平,就可以防范未來(lái)醫(yī)療費(fèi)用支付帶來(lái)的風(fēng)險(xiǎn),可是基金的過(guò)多結(jié)余卻可能損害參保者利益,F(xiàn)階段我國(guó)醫(yī)保基金的高結(jié)余會(huì)帶來(lái)哪些影響以及如何去實(shí)現(xiàn)合理化的城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余水平,這是我們目前需要深入研究的問(wèn)題。本文從影響城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金的因素分析出發(fā),深入探究導(dǎo)致職工醫(yī)療保險(xiǎn)基金高結(jié)余的根源,最后針對(duì)產(chǎn)生基金高結(jié)余的根源,相應(yīng)地提出如何實(shí)現(xiàn)合理化的城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余水平政策建議。 本文總共分為六個(gè)部分。第一章導(dǎo)論。本章主要內(nèi)容包括本文的選題背景及意義、文獻(xiàn)綜述(國(guó)內(nèi)和國(guó)外文獻(xiàn)綜述)、研究方法和結(jié)構(gòu)以及可能的創(chuàng)新點(diǎn)與不足之處。 第二章主要對(duì)城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金的現(xiàn)狀進(jìn)行梳理以及指出城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金在運(yùn)行過(guò)程中所存在的問(wèn)題并且應(yīng)該遵循的一般規(guī)律。接著在對(duì)我國(guó)三大基本醫(yī)療保險(xiǎn)基金進(jìn)行了比較的同時(shí),也從區(qū)域出發(fā)對(duì)最近幾年各省市的城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金進(jìn)行比較分析。 第三章首先在對(duì)進(jìn)行城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余的深度分析之后,接著在假設(shè)前提下對(duì)未來(lái)城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金的結(jié)余進(jìn)行預(yù)測(cè)從而實(shí)現(xiàn)城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余合理化水平。 第四章用實(shí)證研究方法對(duì)影響城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金的相關(guān)因素進(jìn)行分析,最后深入闡述導(dǎo)致城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余過(guò)多的原因。 第五章選取一個(gè)典型的城市作為研究的案例,介紹了該市城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)制度的演變以及城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金的運(yùn)行現(xiàn)狀。 第六章為本文的總結(jié)章節(jié)。主要闡述了在遵循基金“以收定支、收支平衡、略有結(jié)余”原則的前提下,如何實(shí)現(xiàn)合理化的城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)基金結(jié)余水平的具體對(duì)策建議。
[Abstract]:The basic medical insurance system for urban workers in 1998 over the years since the establishment of non balanced urban workers' basic medical insurance fund income and expenditure growth (fund income is significantly faster than the fund spending) lead to fund balances for the current year and the cumulative balance scale is continually expanding. At the end of 2013, the basic medical insurance fund for urban workers in China and then the balance reached 144 billion 700 million yuan, the cumulative balance as high as 813 billion yuan. In other words, even in the absence of the current year and next year the income of the fund fund expenditures remain unchanged under the cumulative balance of funds alone can guarantee 14 months of fund expenditure. Huge urban workers medical insurance fund balance, has become a hot topic in the field of social security China if one. The fund can maintain an appropriate balance level, can prevent the risk of future payment of medical expenses, but too much can balance fund Can damage the interests of the insured. High balances at the present stage of China's medical insurance fund will be what impact and how to realize the rationalization of urban workers' basic medical insurance fund balance level, this is we need to study the problem. In this paper, from the influence factors of the basic medical insurance fund for urban workers based on the analysis, in-depth exploration leads to medical workers the insurance fund balances high root, according to the origin of fund balances, puts forward how to realize the urban workers' basic medical insurance fund balance policy rationalization proposal.
This paper is divided into six parts. The first chapter is introduction. The main contents of this chapter include the background and significance of this topic, literature review (domestic and foreign literature review), research methods and structure, and possible innovations and shortcomings.
The second chapter is mainly on the basic medical insurance fund to sort out the general law and points out that the basic medical insurance fund for urban workers in the process of operating problems and should follow. Then in the three of China's basic medical insurance fund are compared at the same time, from the starting region in recent years each and the basic medical insurance fund were compared and analyzed.
The third chapter in depth to analyze the urban workers basic medical insurance fund balance, then the future urban workers' basic medical insurance fund balance in the assumption that in order to achieve the balance of urban employee basic medical insurance fund reasonable level prediction.
The fourth chapter analyzes the related factors that affect the basic medical insurance fund of urban employees by using the empirical research method, and finally elaborates the reasons leading to the excessive surplus of basic medical insurance fund for urban employees.
The fifth chapter selects a typical city as a case study. It introduces the evolution of basic medical insurance system for urban workers and the operation status of basic medical insurance fund for urban workers.
The sixth chapter is the summary section of this paper. It mainly expounds how to achieve the rationalization of the balance of basic medical insurance fund of urban workers under the premise of the principle of "fixed income, balance of payments and a little surplus".
