面向養(yǎng)老院的健康管理服務設計研究

【摘要】:我國老年人數(shù)量不斷增多,老齡化程度不斷加深,老年人養(yǎng)老已經(jīng)成為社會關注的熱點問題。而機構(gòu)養(yǎng)老作為傳統(tǒng)家庭養(yǎng)老的重要補充,已經(jīng)成為越來越多老年人的選擇。入住養(yǎng)老院的老年人在某種程度上都存在一定的健康問題,大部分老人均患有慢性病。因此,本課題從服務設計的角度出發(fā),運用服務設計的理論與方法,對養(yǎng)老院的健康管理服務現(xiàn)狀進行分析,歸納現(xiàn)有健康管理服務存在的問題,并針對性的提出設計策略,以優(yōu)化健康管理服務流程,滿足老人多層次的健康護理需求。首先,在對兩家養(yǎng)老院的預調(diào)研的過程中,筆者總結(jié)了健康管理服務系統(tǒng)的利益相關人群,及不同護理級別老人所享受的健康管理服務,其次對健康管理服務系統(tǒng)相關人群,包括老人、醫(yī)生、護士、老人子女和養(yǎng)老護理員的現(xiàn)狀進行分析,了解制約養(yǎng)老院健康管理服務系統(tǒng)的客觀因素。對健康管理過程中的入院檔案建立與健康信息管理、健康監(jiān)測與評估、健康指導與干預、突發(fā)緊急狀況處理等接觸點進行分析,了解健康管理的現(xiàn)狀,通過對三家養(yǎng)老機構(gòu)在健康監(jiān)測、健康評估和健康干預的內(nèi)容進行比對,最終得出了優(yōu)化信息反饋和完善健康信息管理兩個設計切入方向。其次,在后續(xù)的深入調(diào)研中設計訪談問題,并和養(yǎng)老院中的老人、醫(yī)生、護士、養(yǎng)老護理員和老人子女等相關人員進行深入訪談,結(jié)合深入訪談、體驗地圖等方法對養(yǎng)老院現(xiàn)有的健康管理服務中的問題點進行挖掘,總結(jié)出健康管理服務系統(tǒng)中的具體需求點,使用卡片分類的方法對具體的需求點進行歸納,總結(jié)出現(xiàn)有的健康管理服務存在的四大問題:⒈服務種類單一⒉智能化應用率低⒊信息傳遞流程長,信息滯后⒋信息的完整度與連續(xù)性差。然后用戶旅程方法對設計機會點進行總結(jié),總結(jié)出設計可介入的問題點。進而,針對以上問題,提出了⒈改變服務模式,增加健康管理服務種類⒉補充養(yǎng)老院的服務功能⒊合理高度地技術(shù)介入4.易用的信息傳遞系統(tǒng)的設計策略,滿足老人多層次的健康護理需求,保證健康管理服務提供的及時性,以及實現(xiàn)健康信息的集中管理與信息共享。最后,筆者提供的健康管理服務設計策略,使得老人、醫(yī)生、護士和養(yǎng)老護理員均能從中獲益。通過重塑服務形態(tài),可以滿足老人多層次的健康護理需求;并行的監(jiān)測機制,不但可以提高自理老人的自我健康管理參與性,而且可以減輕醫(yī)護人員、養(yǎng)老護理員的工作負擔;異常信息反饋機制縮短了信息傳遞流程,保證服務提供的及時性。健康管理系統(tǒng)則保證信息的長久性、連續(xù)性和共享性,利用程序的自動化評估,實現(xiàn)老人健康危險的預警提醒。面向養(yǎng)老院的健康管理服務設計策略的提出,對養(yǎng)老院老人健康管理的研究者具有一定的啟示與借鑒作用,并可促進更多研究者對養(yǎng)老院的健康服務系統(tǒng)進行深入研究和改善,為老人提供更多樣、更全面的健康管理服務。
[Abstract]:The number of old people in our country is increasing, the age of aging is increasing, and the old-age pension has become a hot issue of social concern. As an important supplement to the old-age pension, institutional care has become the choice of more and more old people. The elderly in the nursing home have some health problems to some extent, and most of the old people have chronic diseases. Therefore, from the point of view of service design, this subject, using the theory and method of service design, analyzes the present situation of the health management service of the nursing home, sums up the problems existing in the existing health management service, and puts forward the design strategy to optimize the health management service flow. and the multi-level health care requirement of the old man is met. First, in the course of the pre-investigation of the two nursing homes, the author sums up the benefit-related population of the health management service system and the health management service enjoyed by the old people at different care levels, and then the relevant people of the health management service system, including the old man, the doctor and the nurse, The present situation of the children and the old-age nursing staff of the old people is analyzed, and the objective factors that restrict the health management service system of the nursing home are known. The health management, health information management, health monitoring and evaluation, health guidance and intervention, sudden emergency treatment and other contact points are analyzed to understand the current situation of the health management, and through the health monitoring of the three pension institutions, The content of health assessment and health intervention is compared, and finally the two design cut-in directions of the optimization information feedback and the improvement of the health information management are obtained. Secondly, in the follow-up and in-depth investigation, design the interview problem, and carry out in-depth interview with the old people, doctors, nurses, old-age care workers and the children of the old people in the nursing home, and combine with the in-depth interview, The problem point in the existing health management service of the nursing home is excavated by the method of experience map and the like, the specific demand point in the health management service system is summarized, the specific demand points are summarized by using the method of the card classification, and the four problems existing in the health management service are summarized: The information transfer process is long, and the complete degree and continuity of the information lag information are poor. The design opportunity points are then summarized by the user's journey method, and the problem points of the design can be summarized. In addition, aiming at the above problems, the author puts forward a new method of changing the service mode, increasing the service function of the health management service, and supplementing the service function of the nursing home. The design strategy of the easy-to-use information delivery system meets the multi-level health care needs of the old people, ensures the timeliness of the health management service, and realizes the centralized management and information sharing of the health information. Finally, the author provides the design strategy of the health management service, so that the old man, the doctor, the nurse and the old-age care worker can benefit from it. by reshaping the service form, the multi-level health care demand of the old man can be met; the parallel monitoring mechanism can not only improve the self-health management and participation of the self-care old man, but also can reduce the workload of the medical staff and the old-age care worker; The abnormal information feedback mechanism shortens the information transfer process and ensures the timeliness of service delivery. The health management system ensures the long-term, continuity and sharing of the information, and uses the automatic assessment of the program to realize the early warning and warning of the health risk of the old people. According to the design strategy of the health management service for the nursing home, the researcher has a certain enlightenment and reference for the health management of the old people in the nursing home, and can promote the further research and improvement of the health service system of the nursing home by the more researchers, and provide a more diversified way for the old man.
