【摘要】：隨著開封市人口老齡化程度日趨嚴(yán)峻,伴隨老齡化而來的健康問題對現(xiàn)行養(yǎng)老和健康體系帶來重大的挑戰(zhàn),其中失能老人的醫(yī)療護理與生活照料等需求以成為亟待解決的重要民生問題。因此,為破解失能老人養(yǎng)老難題,必須利用機構(gòu)養(yǎng)老形式創(chuàng)新,鼓勵養(yǎng)老機構(gòu)增強醫(yī)護功能,改善失能老人的健康狀況,整合現(xiàn)有醫(yī)療服務(wù)與養(yǎng)老服務(wù)資源,推動新的醫(yī)養(yǎng)結(jié)合形式的發(fā)展。以開封市作為調(diào)研樣本,通過問卷調(diào)查和訪談法,對城區(qū)失能老人醫(yī)養(yǎng)結(jié)合服務(wù)需求與供給現(xiàn)狀進行分析,從中深刻剖析目前城市失能老人醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老所存在的問題及原因。本文從實踐中提取出醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老的創(chuàng)新之處,就是以失能老人為目標(biāo)群體,以養(yǎng)老機構(gòu)下開設(shè)正規(guī)醫(yī)療診所、養(yǎng)老機構(gòu)和醫(yī)療機構(gòu)相互合作等方式為目標(biāo)群體提供養(yǎng)老和醫(yī)療服務(wù),使其增加人性化的專業(yè)醫(yī)療服務(wù)。本文主要從五部分進行研究分析:第一部分為總括,對研究的背景和意義進行論述,梳理當(dāng)前國內(nèi)關(guān)于失能老人機構(gòu)養(yǎng)老研究現(xiàn)狀,以及國外醫(yī)養(yǎng)結(jié)合養(yǎng)老的相關(guān)研究成果,闡述文章思路、方法和創(chuàng)新之處。第二部分回答了“什么是醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老”的問題,以能力貧困、需要層次與福利多元主義為理論基礎(chǔ),從失能老人需求、養(yǎng)老服務(wù)提供主體等方面詳細闡述了醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老發(fā)展的依據(jù)。第三部分包括第三章和第四章內(nèi)容,回答了“醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老發(fā)展現(xiàn)狀與前景如何”的問題,首先從總量上分析開封市養(yǎng)老服務(wù)提供匱乏,再通過實地調(diào)研結(jié)論,并梳理了醫(yī)養(yǎng)結(jié)合養(yǎng)老機構(gòu)的現(xiàn)狀,整理出醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老發(fā)展成就以及面臨的主要困境,引出政策支持的必要性。第四部分回答了“如何破解失能老人醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老的困境”的問題,以政策大力扶持作為解決失能老人醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老困境的突破口,以培養(yǎng)“醫(yī)養(yǎng)結(jié)合”護理人員為基礎(chǔ),依靠居家、社區(qū)養(yǎng)老的方式來減緩機構(gòu)養(yǎng)老的壓力,促使我國城鎮(zhèn)化進程中失能老人養(yǎng)老問題解決。第五部分對全文主要結(jié)論進行了歸納,進一步提出醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老發(fā)展進程中面臨的問題,需要政府與社會力量共同參與解決。因開封市失能老人比重在河南省較高,逐漸推行醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老新形式,在今后所有養(yǎng)老模式中發(fā)展進程中日益突出,也將成為政府和社會處理這類老年問題的新途徑。為更好發(fā)展醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老,需落實政府職責(zé),多部門共同合作促進發(fā)展;鼓勵社會力量參與并提供服務(wù);提升養(yǎng)老機構(gòu)醫(yī)養(yǎng)結(jié)合自身實力,注重醫(yī)療服務(wù)投入等,并在認(rèn)真結(jié)合開封市實情下,摸索出一條適合醫(yī)養(yǎng)結(jié)合機構(gòu)養(yǎng)老發(fā)展的道路。
[Abstract]:With the aging of the population in Kaifeng becoming more and more serious, the health problems associated with the aging of the population bring major challenges to the current pension and health system. Therefore, in order to solve the problem, it is necessary to make use of the innovation in the form of institutional pension to encourage the institution to enhance the health care function of the disabled elderly, so as to solve the problem of providing for the disabled, the needs of medical care and living care for the disabled are urgent to be solved. To improve the health status of the disabled elderly, to integrate the existing medical service and old-age service resources, and to promote the development of a new form of combination of medical care and maintenance. Taking Kaifeng City as a research sample, through questionnaires and interviews, This paper analyzes the demand and supply of medical and nursing services for the disabled elderly in urban areas. In this paper, the author analyzes the problems and causes of the aged in the medical and nursing institutions of the urban disabled elderly. This paper extracts from the practice that the innovation of the aged in the medical and nursing institutions is to take the disabled elderly as the target group. Providing old-age care and medical services to the target groups through the establishment of regular medical clinics under pension institutions, cooperation between pension institutions and medical institutions, etc. The first part is an overview of the background and significance of the study, combing the current domestic research on the disabled elderly institutions for the aged. The second part answers the question of "what is the old-age care system in combination of medical and support", which is poor in ability, Based on the theory of need level and welfare pluralism, this paper expounds in detail the basis for the development of the aged in combination of medical and nursing institutions from the aspects of the needs of the disabled elderly and the main body of providing services for the aged. The third part includes the third chapter and the content of chapter 4th. In this paper, the author answers the question of "what is the present situation and prospect of the development of the aged in the combination of medical and foster institutions". Firstly, it analyzes the shortage of the old-age service in Kaifeng from the total amount, and then through the field investigation conclusion, and combs the present situation of the old-age care institution of the combination of medical and support. This paper collates the achievements and the main difficulties of the development of the aged combined with medical institution and raises the necessity of policy support. In 4th, it partly answers the question of "how to solve the dilemma of the institution for the aged with disability". Taking the policy support as the breakthrough to solve the problem of the aged in the institution of medical care and raising for the disabled, taking the training of the nursing staff of the combination of medical treatment and support as the foundation, relying on the way of home and community old-age support, to alleviate the pressure of the institutional old-age care. In the 5th part, the main conclusions of this paper are summarized, and the problems faced in the process of the development of medical and nursing institutions for the aged are put forward. Because the proportion of disabled elderly in Kaifeng City is higher in Henan Province, the new form of old-age care in combination of medical and nursing institutions has been gradually implemented, which will become increasingly prominent in the development process of all pension models in the future. It will also be a new way for the Government and society to deal with such problems of old age. In order to better develop health care institutions for the aged, there is a need to fulfil the responsibilities of the Government and to work together in various sectors to promote development; and to encourage the participation of social forces and the provision of services; In order to improve the strength of medical and nursing institutions and pay attention to the input of medical services, and combining with the reality of Kaifeng City, this paper tries to find out a suitable way for the development of old-age care in combination of medical and nursing institutions.
