【摘要】：目的探討吸收放散試驗(yàn)及抗體增強(qiáng)方法在ABO血型抗原減弱和血清抗體效價(jià)降低及高效價(jià)冷凝集素引起疑難血型鑒定中應(yīng)用的可行性。方法對30例ABO血型正反定型不相符的血標(biāo)本,采用不規(guī)則抗體篩選、抗體增強(qiáng)方法、吸收放散試驗(yàn)及直接抗人球蛋白試驗(yàn),觀察患者血型抗原及血清中抗體的檢出情況。結(jié)果 30例患者不規(guī)則抗體篩選和直接抗人球蛋白試驗(yàn)陰性,被患者紅細(xì)胞吸收后的人源抗A或抗B血清效價(jià)有所降低;患者紅細(xì)胞放散液與A型或B型試劑紅細(xì)胞出現(xiàn)預(yù)期的血型抗原抗體反應(yīng),凝集強(qiáng)度由常規(guī)方法的混合外觀凝集(MF)~1+增加到吸收放散試驗(yàn)及抗體增強(qiáng)方法的1+~3+,ABO血型正反定型相符。結(jié)論吸收放散試驗(yàn)?zāi)芷鸬綕饪s血清及初步純化抗體的作用,抗體增強(qiáng)方法可增加低效價(jià)血清抗體的凝集強(qiáng)度,在輸血相容性檢測中聯(lián)合應(yīng)用ABO正反定型、吸收放散試驗(yàn)及抗體增強(qiáng)方法,可準(zhǔn)確鑒定ABO疑難血型及解決部分交叉配血不合的問題。
[Abstract]:Objective to investigate the feasibility of absorption and release test and antibody enhancement in the identification of difficult blood groups caused by ABO blood group antigen weakening, serum antibody titer reduction and high valence lectin. Methods 30 blood samples with inconsistent positive and negative typing of ABO blood group were screened with irregular antibody, antibody enhancement method, absorption and release test and direct anti-human globulin test to observe the detection of blood group antigen and antibody in serum. Results the irregular antibody screening and direct antiglobulin test were negative in 30 patients, and the titer of human anti-A or anti-B serum was decreased after being absorbed by red blood cells. The red blood cell release fluid of the patient reacted with type A or type B reagent red blood cells with the expected blood group antigen antibody reaction. The aggregation intensity increased from the mixed appearance of the conventional method to 1 ~ 3 of the absorption and release test and antibody enhancement method. The positive and negative typing of ABO blood group was consistent. Conclusion absorption and release test can concentrate serum and purify antibody preliminarily. Antibody enhancement method can increase the aggregation intensity of low titer serum antibody. ABO positive and negative typing, absorption and release test and antibody enhancement method can be used in blood transfusion compatibility detection, which can accurately identify the difficult blood group of ABO and solve the problem of partial cross matching blood incompatibility.
