【摘要】：目的了解湖南省第二人民醫(yī)院和長(zhǎng)沙市中心醫(yī)院臨床用血現(xiàn)狀,并分析住院患者臨床合理用血質(zhì)量管理取得的成效。方法對(duì)2011~2014年于湖南省第二人民醫(yī)院和長(zhǎng)沙市中心醫(yī)院住院患者臨床用血數(shù)據(jù)進(jìn)行回顧性分析,統(tǒng)計(jì)異體輸血總量、自體輸血總量、住院患者人均異體血用量、成分輸血率、手術(shù)臺(tái)次、臺(tái)均異體用血量及大量輸血患者人均用血量等指標(biāo)的變化。結(jié)果 2011~2014年在住院人次逐年增加的基礎(chǔ)上,異體輸血總量也有所增加,但臨床用血量的增長(zhǎng)率遠(yuǎn)低于住院人次的增長(zhǎng)率,住院患者人均異體血用量呈逐年下降的趨勢(shì),而自體血用量呈增長(zhǎng)趨勢(shì)。大量輸血例數(shù)逐年減少,大量輸血各成分血液制品用量、大量輸血平均紅細(xì)胞和血漿用量均呈負(fù)增長(zhǎng)趨勢(shì)。同期手術(shù)臺(tái)次雖然增長(zhǎng)了28.77%,但是手術(shù)患者臺(tái)均異體紅細(xì)胞和血漿用量均呈遞減趨勢(shì)。結(jié)論通過(guò)持續(xù)改進(jìn),醫(yī)院輸血工作取得了一定成效,大大提升了臨床科學(xué)、合理及安全用血水平,有效節(jié)約了血液資源。
[Abstract]:Objective to investigate the current situation of clinical blood use in the second people's Hospital of Hunan Province and Changsha Central Hospital, and to analyze the results of clinical rational blood quality management of inpatients. Methods the clinical blood data of inpatients in the second people's Hospital of Hunan Province and Changsha City Central Hospital from 2011 to 2014 were analyzed retrospectively. the changes of the total amount of allogenic blood transfusion, the total amount of autologous blood transfusion, the per capita allogenic blood consumption of inpatients, the component transfusion rate, the operation table, the average allogenic blood consumption of Taiwan and the per capita blood consumption of patients with massive blood transfusion were statistically analyzed. Results from 2011 to 2014, the total amount of allogenic blood transfusion also increased on the basis of the increase of hospitalization number year by year, but the growth rate of clinical blood consumption was much lower than that of hospitalization. The per capita allogenic blood consumption of inpatients showed a decreasing trend year by year, while the amount of autologous blood consumption showed an increasing trend. The number of cases of massive blood transfusion decreased year by year, the dosage of blood products of various components of massive blood transfusion, the average amount of red blood cells and plasma of a large number of blood transfusions showed a negative increasing trend. Although the number of operation stations increased by 28.77%, the dosage of allogenic red blood cells and plasma showed a decreasing trend. Conclusion through continuous improvement, hospital blood transfusion has achieved some results, which has greatly improved the level of clinical science, reasonable and safe blood use, and effectively saved blood resources.
