【摘要】：目的探討血小板相關參數與單采血小板輸注療效之間的相關性,為臨床診斷提供指導意義。方法通過回顧性的分析本院2014-2016年輸注單采血小板的患者50例,根據首次輸注1個治療量血小板后d 5復查血常規(guī)結果,血小板計數升至正常范圍為增長組(27人),血小板計數未達正常范圍為降低組(23人)。分別比較分析2組輸注前和輸注后24 h內血小板相關系數包括血小板計數(Plt),血小板分布寬度(PDW),平均血小板體積(MPV),大血小板比率(P-LCR)的改變,同時計算24 h血小板糾正計數指數(CCI)。結果 24 h CCI4.5增長組有21例占77.78%,降低組有11例占47.83%;輸注前血小板相關參數兩組間比較沒有顯著差異(P0.05),輸注后,增長組Plt,PDW,MPV和P-LCR明顯高于降低組(P0.01);其中輸血前后數據相比較增長組MPV,P-LCR全部增高,降低組MPV,P-LCR則全部降低。結論血小板參數檢測特別是MPV和P-LCR的改變趨勢對于血小板輸注的療效觀察與預后判斷具有重要的臨床意義。
[Abstract]:Objective to investigate the correlation between platelet related parameters and the curative effect of platelet transfusion, and to provide guiding significance for clinical diagnosis. Methods from 2014 to 2016, 50 patients with single platelet transfusion in our hospital were analyzed retrospectively. according to the results of routine examination on the day 5 after the first infusion of 1 therapeutic volume of platelet, the platelet count increased to the normal range (27 patients) and the platelet count did not reach the normal range (23 patients). The results showed that the platelet count increased to the normal range (27 patients) and the platelet count did not reach the normal range (23 patients). The platelet correlation coefficient, including platelet count (Plt), platelet distribution width, (PDW), mean platelet volume (MPV), large platelet ratio (P-LCR), was compared between the two groups before and 24 hours after infusion, and the platelet correction index (CCI). Was calculated at the same time. Results there were 21 cases (77.78%) in 24 h CCI4.5 growth group and 11 cases (47.83%) in decreased group. There was no significant difference in platelet related parameters between the two groups before infusion (P 0.05). After infusion, Plt,PDW,MPV and P-LCR in the growth group were significantly higher than those in the decrease group (P 0.01), and the MPV,P-LCR in the increase group was all higher than that in the decrease group before and after transfusion, while the MPV,P-LCR in the decreased group was all decreased. Conclusion the detection of platelet parameters, especially the changing trend of MPV and P-LCR, is of great clinical significance for the observation of curative effect and prognosis of platelet transfusion.
