【摘要】：目的:探討自體外周血造血干細(xì)胞移植(auto-PBHSCT)過程中造血重建的影響因素。方法:回顧性分析接受auto-PBHSCT的177例病人的造血重建成功率、造血重建時(shí)間和移植后28 d造血重建狀態(tài),探索造血重建的影響因素。結(jié)果:粒系中位植入時(shí)間為12(8-21)d,植入率98.9%,均在28 d內(nèi)植入;血小板中位植入時(shí)間為17(7-420)d,植入率95.5%,移植后28 d血小板累積植入率80.8%。單因素分析結(jié)果顯示,回輸CD34~+細(xì)胞數(shù)量和rh TPO的使用是粒細(xì)胞植入時(shí)間的影響因素;而疾病種類、預(yù)處理方案、回輸CD34~+細(xì)胞數(shù)量是血小板植入時(shí)間的影響因素。多因素分析結(jié)果表明,回輸CD34~+細(xì)胞數(shù)量是粒細(xì)胞植入時(shí)間的獨(dú)立影響因素,而疾病種類、回輸CD34~+細(xì)胞數(shù)量是血小板植入時(shí)間的獨(dú)立影響因素。疾病種類和回輸CD34~+細(xì)胞數(shù)量是移植后28 d造血重建狀態(tài)的獨(dú)立影響因素。結(jié)論:對(duì)于auto-PBHSCT而言,疾病種類、預(yù)處理方案、回輸CD34~+細(xì)胞數(shù)量、rh TPO的使用是造血重建的影響因素。
[Abstract]:Objective: to investigate the influential factors of hematopoietic reconstitution in autologous peripheral blood stem cell transplantation (auto-PBHSCT). Methods: the successful rate of hematopoiesis reconstruction, the time of hematopoiesis reconstruction and the state of hematopoiesis reconstitution 28 days after transplantation were retrospectively analyzed in 177 patients receiving auto-PBHSCT, and the influencing factors of hematopoietic reconstitution were explored. Results: the median implantation time was 12 (8? 21) days, the implantation rate was 98.9%, all of them were implanted within 28 days. The median time of platelet implantation was 17 (7 渭 420) days, the implantation rate was 95.5%, and the cumulative platelet implantation rate was 80.8% on the 28th day after transplantation. The results of univariate analysis showed that the number of CD34~ cells and the use of rh TPO were the influencing factors of the time of granulocyte implantation, and the type of disease, the pretreatment scheme and the number of CD34~ cells were the influencing factors of the time of platelet implantation. The results of multivariate analysis showed that the number of CD34~ cells was the independent factor influencing the time of granulocyte implantation, and the type of disease and the number of CD34~ cells were the independent factors influencing the time of platelet implantation. The type of disease and the number of CD34~ cells were independent influencing factors of hematopoiesis reconstitution 28 days after transplantation. Conclusion: as far as auto-PBHSCT is concerned, the type of disease, the pre-treatment scheme and the use of, rh TPO in the number of CD34~ cells re-infused are the influencing factors of hematopoiesis reconstruction.
