【摘要】：目的全面了解該院7年來輸血不良反應的發(fā)生情況,并分析其分布特點和可能的相關(guān)性因素,為深入研究輸血不良反應的發(fā)生機制提供參考。方法通過手工查閱2010年以來所有輸血不良反應的相關(guān)記錄信息,通過電子病歷的輸血管理系統(tǒng)查閱2013年來所有懸浮紅細胞的有效期和發(fā)血日期。結(jié)果 2010年以來,從該院輸血不良反應發(fā)生率來看,總體發(fā)生率為2.38±0.86‰(349次/151 605 U),血小板為5.64‰,紅細胞為3.17‰,血漿為1.28‰,冷沉淀為0.94‰。在非溶血性發(fā)熱反應中RBC占64%(172/212);過敏反應中血漿占38.1%(40/105),血小板占28.6%(30/105)。消化內(nèi)科和血液內(nèi)科分別占輸血反應總數(shù)的32%和21%(110/349和72/349);消化道出血患者發(fā)生的不良反應中RBC占93%(63/68);肝病患者發(fā)生的不良反應中血漿占71%(17/24);白血病患者發(fā)生的不良反應中血小板占57%(20/35)。儲存期為第1、2、3和第4周的懸浮RBC不良反應發(fā)生率分別為5.66‰、4.49‰、3.65‰和4.89‰。結(jié)論 RBC發(fā)生的輸血不良反應以非溶血性發(fā)熱為主,其他血液成分以過敏反應為主;隨著懸浮RBC儲存時間的延長,其輸血不良反應的發(fā)生率出現(xiàn)先降低后升高的趨勢。本研究不能完全排除疾病類型和相關(guān)血液成分之間發(fā)生特定輸血不良反應的相關(guān)性。
